Employee failed to lock out breaker and removed gutter cover in (ER6), it is unknown if the employee inadvertently moved a bare wire into the energized tap blocks causing an arc occurrence resulting in the employee being burnt on hands. Employee was transported to hospital by company truck.

The investigation into the accident is on-going, but as of now, it appears as the three employees were installing ground vault indicator lights when an ach flash occurred, injuring them. The employees were transported to the hospital.

Employee was welding on grizzly bar in feeder, when another employee engaged in horseplay by spraying water on him with a garden hose from above. The employee stopped welding and began walking around. When the employee was asked what was wrong he stated he had been shocked.
