EE middle left finger fractured when struck between bucket tooth and shank of WA800 loader
The associate was drilling through a rubber sheet and the drill bit went through the rubber sheet and contacted EE's hand resulting in a laceration.
Metal plate fell onto employees foot. Incident did not become reportable until 2/16/2026 when employee sought metal attention and was diagnosed with fractures.
EE was struck by belt from behind causing EE to come in contact with belt rolling frame.
Upon completing repairs to a 336 excavator, then descending from the machine, left foot touched the ground, the Technician felt a pop in their Left knee. Pain increased over the next few days, and Technician decided to seek medical attention. All PPE was properly worn at the time of the incident.
Employee injured left arm while fusing pipe
EE was cutting wedge bolt from stationary jaw dye. While cutting the wedge bolt, the bolt sheared and made contact with EE's welding hood that they were wearing. The frame that holds the lens in the welding hood broke loose and lacerated EE above their left eye. EE received 12 stitches above left eye.
Due to personal medical illness, employee fell out of pickup truck parked at electrical room in the secondary plant. They suffered a laceration to the forehead and received stitches.
Employee was operating a 992K loader while loading customer trucks when EE heard a "pop" in EE's right wrist while using the control handles.
Loosening a bolt with with an impact wrench and socket wrench. Hand slipped off wrench and was struck in face.
The employee was operating a WA600 loader and, while moving backward to forward on level ground, experienced neck & shoulder pain. This became reportable on 12/30/2025 after the employee was placed on restrictions following an orthopedic evaluation.
Employee was injured when they fell walking to the troop carrier. Injury became reportable on 11-18-25 when an orthopedic specialist diagnosed a left shoulder partial rotator cuff tear and left shoulder proximal humerus fracture. Physical therapy was also ordered.
Employee for C & L Machine and Welding and foreman for Martin Marietta were standing on-site looking at a tooth on a bucket to see which one had to be removed. Employee tapped the tooth with a hammer and a metal splinter went into EE's groin area.
While installing a cylinder in a scaling machine, a lifting strap failed. The failed strap section hit the employee in the eye causing a scratch to the cornea. All proper PPE was worn.

Employee was adjusting the bowl on Omnicone 1560 crusher when EE's right thumb was caught between two adjusting cylinders. EE's right thumb was fractured.
Employee rolled left ankle resulting in a fracture
Employee was struck by foreign object in the eye during maintenance. Foreign object was removed from eye.
(Illness) Employee was standing and passed out, fell to the ground, and received a cut on EE's head from the ground. (total time passed out was less than a minute). EE was taken to the hospital, received 3 stitches to close the cut along with IV's. EE was then released, told to stay home one day, permitted by the doctor to drive self home. EE has been working since then.
A piece of loose material contacted the employee on the right arm and left leg. Required stitches.
Employee smashed finger removing rock from crusher
The employee was removing metal from a conveyor, and while going to dispose of it, the employee lost control of the metal object and pinched their hand between the metal and the handrail. This resulted in a laceration and a broken finger.
While removing an expanded metal guard bent around a motor shaft, the guard flexed back to a straight position after the bolt was removed striking and cutting the employee's nose. employee received 5 stitches.
It appears that an employee experienced a personal medical event while operating a haul truck. This medical event is believed to have resulted in a loss of consciousness, leading to a loss of vehicle control and a rollover incident. Available evidence suggests that it may be attributable to a personal medical condition rather than factors related to the work environment.
Employee was climbing ladder into equipment and felt a pop in their right knee. Injury did not become reportable until 7/1/2025 when employee was prescribed and attended physical therapy.
Trespasser entered mine property and while attempting to climb down from bench 5 to bench 6, slipped and fell approximately 40 feet breaking both legs.

EE's left middle finger was smashed & lacerated between two parts of Komatsu 605 haul truck during removal of dampener.
Technician was working with a man lift and it became stuck. Technician was pushing at the end of the man lift to get it unstuck and the slide was out a bit and when pushing on that it slid in and it cut/split their finger while servicing a 775 Cat at the start of the shift.
Mid American Electrical insulator failed causing an arc flash, power wire fell to the ground. No damage/injury to the mine site or employees.
A train car wheel sparked on the tracks while leaving the site, starting a brush fire on the mine property.
Trying to dislodge a rock 17' down a bore hole with loading poles. The rock was dislodged and the poles rebounded and hit employee under the right eyebrow. Cutting EE with a 1' cut. EE was wearing safety glasses. EE was taken to the emergency care unit. for stitches.
Left hand caught between tube and chute wall- worker was removing old scrape cleaner- tube was stuck, then broke free, worker did not expect tube to slide forward toward left hand- when it slid tube caught left hand between chute wall and tube- received 7 stitches between left index finger knuckle and middle finger knuckle- no broken bone
Employee's hand was pinched between a plate of steel and a block of wood resulting in a laceration approximately 2.5 cm that required stitches.
Upon arrival at the mine site a brush fire was located on the back side of the property between the pit and rail line. This area is away from the plant and consequently did not expose miners to the hazard. 800 number called. Fire extinguished and monitored by local fire department.
A tree fell on mine property due to high wind, causing it to fall on a powerline, causing it to arc. This resulted in two brush fires to start burning on the mine property.
03/18/2025 Employee was standing near the Deister screen when the wind blew dirt into the employee's right eye. First Aid measures were provided on site. The employee was seen by a Banner Health clinic on 03/18/2025. Prescription eye drops were prescribed by the physician. The employee did not have any lost time or restrictions.
Arsonist suspected of setting fire that expanded onto quarry property. 911 called. Fire was confirmed to be on quarry property and 800 number was called. Local responding agencies contained and extinguished the fire. They confirmed they were seeking an arson suspect.

Employee was found entangled in conveyor 12 tail pulley. Responding authorities pronounced the employee deceased. Incident is still under investigation.
Employee was struck by excavator bucket
Night shift EE decided to retrieve EE's RC car from the employee parking area. The EE retrieved the RC car, placed it in the company truck, and stopped by the granite sand pile. EE began operating the RC car and during this process, EE lost control of the RC car and it jumped off the ramp at approximately 20 mph and struck the EE in the face, causing a laceration on EE's chin.
Employee rolled their left ankle while getting out of a pickup. The injury became reportable after a doctor placed employee on restrictions on 02/26/2025
