Employee stated that as he was operating the C/M while pillaring, a piece of rib sloughed off and struck him on the lower right leg causing pain in his calf and ankle. Employee returned to work his normal job and shift until 8/10 his job class was changed. The employee did not miss any scheduled days.

Employee was installing a sieve wheel to the mine roof to pull in the belt on 6 West. As the employee started to hang the sieve, a rock on the right corner rib rolled out smashing the employee's right thumb between the rock and the belt structure.

Employee was backing the continuous miner out of a cut when a rock fell out between the rib and outside bolts striking the employee on the left foot.

Employee stated that while tramming the continuous miner out of a place on the 1 North Mains section the boom of the miner contacted the rib. After shutting off the power on the machine, injured employee rested his hand on the boom to check the cable when a piece of the rib rolled off and struck his right thumb causing a fracture.

Employee stated he parked and exited his shuttle car. As he walked between the shuttle car and the rib, the rib rolled/sloughed off and a small piece of coal/rock either struck the employee in the knee, or caused him to fall. This resulted in a knee strain and restricted work days.

Employee was walking along the rib line on the long wall headgate area, the rib rolled out and pinned him against the corner of the head drive.

Employee was hanging a piece of line curtain when a section of rib rolled out striking his back and landing on his left foot.

Employee was exiting the operator's compartment on a scoop when the rib rolled out and struck his lower left leg.

Employee states that he was removing the miner cable from a cable hanger that was hung to the mine roof when the corner of the outby rib fell, striking the miner in the lower left side of his back. Employee continued to work but, then felt pain and was transported to the ER where he was X-rayed and was diagnosed with a bruise and was given restricted duty for 3 days.

Employee stated that he was in the process of installing roof bolts in the top after a belt channel had been cut. He was installing the second row of bolts when a rib/rock rolled out and struck him on the left leg and ankle, resulting in a fracture.

Employee states that he was at the inby corner of the intersection in number 5 entry inby the feeder dump point when the corner of the rib rolled out and struck his right foot causing the steel toe of the boot to mash his toes causing the toe nail of the toe beside his little toe to loosen. The nail had to be removed.

Individual was carrying cable bolting supplies along the 8 right belt at 5 block when the inby corner rolled out and struck his upper chest and right side of his head. The individual allegedly lost consciousness momentarily. The incident was immediately reported to MSHA and WVOMHST.

Employee was operating the roof/rib bolter on the Sandvik miner on the 1 North Mains Left section in the crosscut #4-5. As he retracted the rib protector a large flat piece of rock from behind the rib protector struck his left leg causing bruising.

Employee states that he was operating a continuous miner when a piece of rock fell from between a roof bolt and the coal rib. The rock struck employees RT foot causing a fracture.

Injured employee was working in the #9 heading of 1 North Mains when a piece of rock fell from a rib and struck his left foot resulting in a fracture.

Employee stated that he picked up a bottom roller leaning against the coal rib. As he walked to his ride to load the roller the rib gave way and a section 72" x 45" x 12" rolled off struck him in the buttocks and pushed him to the center of the entry where he fell on the belt roller he was carrying, causing a contusion to his left chest & armpit.

Employee stated while turning a left hand break in the #4 entry on the 1 West section a rib rolled out striking him on the legs knocking him down. Another employee had to help him get the rock off of his leg. When the rib hit him it caused him to twist his RT knee which caused pain between his RT knee and ankle.

Employee was operating the shuttle car. As he exited the car to turn around and drive to the feeder, a piece of rock fell from the mine rib and struck him on the lower legs. This resulted in bruises and abrasions.

Employee was helping put a ram jack in the face conveyor when a portion of the face rolled out and struck him on the right ankle and foot.

Injured employee was operating the continuous miner on the 7 Right section mining coal in the crosscut 2-1. While mining, a section of the wedge cut fell from the #2 face, slid down the coal that had been cleaned to the face and struck the employee's legs knocking him to the ground resulting in a bruised/strained back.

Employee was traveling down the East 3 Belt Scoop roadway and heard a coal rib pop and when he turned to look he was struck by coal on the left hand. He received a laceration to his ring finger that required 8 sutures to close.

Employee stated that while installing a bolt on the left side of the continuous miner a piece of coal fell off the rib and struck his left arm and right knee. The rib protector for the miner was in place and operational.

EE had used the ATRS on the single head bolter to knock the stopping blocks stuck to the mine roof. A block had fell on the drill pot. EE walked up beside the bolter to remove the block off the drill pot when a portion of the rib fell striking EE and knocking him into the bolt machine.

EE was operating a roof bolter on the operators side when a piece of the rib measuring approx. 75" l. X 11" w. x 4" t. rolled out and landed on his left ankle.

Injured employee was installing tensar on the overcasts for 17 Left on the 1 South Mains section. While spading the screen into place a section of rib rolled loose and struck the employee's left foot resulting in a fracture to the metatarsal bone.

Employee was walking threw a check curtain when a rock fell between the bolt & rib striking the employee on the right arm resulting in a laceration.

The employee stated he was hanging curtain in 4 cross cut left when a piece of rock fell from between the first bolt and the rib striking the employee in the top of the head. The employee did not begin missing work until 3-14-12.

Check top and rib before taking coal section. IE noticed rock coal shell chunk started coming off rib and IE pushed EE out of the way. Chunk of coal hit EE first, then a piece of coal fell on IE's ankle.

Employee was struck on top of the right foot by a piece of rock that fall from the mine rib, fractures his first right metatarsal.

EE was operating a joy 21 s/c and a piece of the rib rolled off causing a laceration to his left ear that required stitches.

The EE stated he was standing out by the miner between the coalhauler and the mine rib while the miner was loading the hauler. The rib rolled out onto him knocking him down and into the hauler. The rib struck him in the lower leg around the calf area. The EE began missing work on 4/13/11 due to the incident.

Employee was cleaning rock and coal off top of MRS shields when a coal rib fell and struck the employee's right hand.

EE was struck by a piece of rib rock, 6"x6"x8', causing contusions and lacerations that required sutures to close.

Employee stated that had just put up his outside bolt in the seventh row of the cut. He was walking beside the machine to retrieve a half header to support a kettle bottom when the coal rib rolled out striking him in the lower back. He was placed off of work by his treating physician for physical therapy on 2/22/2010.

While walking along rib, hard hat struck roof dislodging a rock from the rib that fell and struck injured employee on the right shoulder blade.

EE had helped setting the MRS's up in the No 5 entry when the right rib rolled off striking him in the right leg.

Miner was walking up to the miner helper to relieve him for lunch and a rib rolled out that struck his side. He later went to the specialist and was placed under doctor's care. The miner was diagnosed with radiculopathy and right foot sprain. The mine site did not receive medical diagnoses until 12-29-09.

Employee had checked the pressure on the mobile roof support and was walking back out of the place when a rib fell and struck him and knocked him to the mine floor.

Employee was standing by No. 4 belt at belt switch and was turning the belt off to spot the belt, when a rib rolled and struck him on his right shoulder.

The roof bolt crew finished bolting the face of #6. The miners backed out of the face and turned the machine off when a rib rolled off from behind the line curtain. The rib measured 6' L, 8" W, and 20 " H. The rib struck the miner in the lower back/legs. The miner had pre-existing back problems and was diagnosed with a lumbar strain. Medical was not released until 8-18-2009.

Injured employee was roof bolting along the left side of the continuous miner on the 6 Right section when a section of rib measuring 9' long x 33" high x 5" thick fell and struck the employee on the back of the right leg just below the knee causing a bruise.

Employee was operating Joy CM-15 when a rock measuring 8'x 2'x 4"-10" fell between the rib pin and rib striking him on his back. Injured back and spine.

Employee was working in the panel of a coal hauler and was kneeled down beside the rib. A small part of the rib rolled off and while moving away from the rib it struck the employee twisting his Ankle.

The employee stated he was standing at the inby corner of break 6 to 7, 100' inby spad 12591. The continuous miner was finishing a lift and the employee was getting ready to operate the MRS. He stated that a piece of coal fell from the rib and struck him on the right side of his head.

Employee stated that he was checking his drill steels at the left side of the continuous miner on the 11 Left section when a rib roll measuring 4" thick x 5' long and 4' wide fell against the employee knocking him to the mine floor. Employee stated he had pain in his lower neck and lower back on his left side.

Employee was struck by a rib roll and bruised his right calf.

The employee stated that he had walked over between the roof bolting machine and the mine rib and was bending over to pick up the line curtain to hand it to the other roof bolter operator, when a piece of rock fell from between the roof bolt and the rib and struck him on his left shoulder resulting in a contusion.

Employee stated that he walked over to talk to the CM helper and was reaching down to move the CM water line out of the roadway when he was struck in the back by a rib roll from the barrier block behind him. He was knocked to the mine floor and suffered bruises and abrasions to his back, left hip and left knee.

Employee was helping change the bits on the continuous miner. The right side of the rib rolled out, knocking a timber out. The timber struck EE on the back of his shoulder. X-rays revealed no fractures and an MRI date has not been scheduled yet.
