On 7/20/2012 employee was in the process of cutting down some old mesh. While cutting down the mesh he was struck in the head with a loose piece of top. He was seen by a Dr. on 7/27/2012 and was released to work that day. He continued to work while waiting for industrial to accept his claim up until his surgery date of 5/6/2014.

AS THE EE LOWER A SHIELD ON THE FACE TOP ROCK FELL AND CAME OVER PAN LINE AND STRUCK HIS FOOT.
