Operator was spot bolting roof bolts, installed bolt while waiting on other drill man. He reached to get an 8 oz of drinking water. Pulled his glove off to pull tab, small pieces rock fell, striking the top of his right hand. Laceration causing cut (8 stitches in top hand).
Was installing roof bolts when roof fractured thin scale of draw rock causing to break free, hit employee's left hand and cutting left ring finger causing to have 2 stitches in finger.
Was loading supplies on drill or roof bolter. Small rock fell from mine roof approx 18" - rock 6" x 8" x 1" striking left hand on top. Simple fracture #3 finger bone on top hand.

Employee was bolting top when a rock approximately 200 pounds fell striking his left hand, causing injury to three fingers on the left hand
Employe was drilling hole for roof bolt, when a piece of rock fell striking him in the ankle.
Employee was removing drill steel from pot on roof bolter and got struck by a rock on his right thumb causing a fracture in the first joint.
Draw rock fell from top striking employee on right foot. (Started missing work on 8/27/04).
Injured had completed setting survey station (#555) in the face area when a piece of draw rock fell and hit him on his right shoulder.
EMPLOYEE WAS REOMOVING SPILL BOARD AT TAILPIECE WHEN A PIECE OF DRAW ROCK APPROX 2' BY 3' BY 1-4" THICK FELL, STRIKING HIS BACK.
EE WAS OPERATING A SCOOP WHEN A PIECE OF SLATE MEASURING UP TO 8'LONG BY 2"TO 23"WIDE BY 0-8" THICK FELL ON HIM. HE SUFFERED BURST FRACTURE TOVERTEBRA T-6 AND A MODERATE FRACTURE TO VERTEBRA T-5. SURGERY WAS SUCCESSFULLY PERFORMED 5/8/02.
INJURED WAS IN A SQUATTING POSITION APPROX 3' INBY THE LEFT CORNER OF THE PILLAR IN THE NO. 3 HEADING. WHILE INJURED WAS WRITING NOTES IN A NOTEBOOK A RIB ROLL OCCURRED AND THE MATERIAL STRUCK INJURED ON THE LEFT SHOULDER.
