A roof fall was discovered on the 1- North Mains. the roof fall is located in the #5 entry and crosscut at break #5. The roof fall has been dangered off. The fall has not affected the ventilation at this mine. The fall will not be cleaned up. The approx. dimensions of the fall is 20' w. x 20-30' l. x 6' thick.
On 3/16/12, a roof fall was found in the #4 entry 1 bk outby spad #3122 on #1 Northwest Mains. Dimensions were 8' H. X 18' W. X 45' L. MSHA was notified by hotline. A ""J"" order was issued later modified to a ""K"" order to allow area to have additional roof support installed. This is not an active producing mine. No injuries reported, no EQ. involved No inter. of vent occurred.
EE was attempting to remove the leg from a rock box that was attached to the tail piece on a belt conveyor. When the leg was removed the weight was shifted which caused the box to fall striking the EE on the right leg.

The EE had completed trimming excess skirt rubber from the #6 tailpiece located at survey station #10483. When the EE stepped down off the belt conveyor his right knee twisted and possibly came out of joint. The EE seen a phys. on 12-2-2011 and again on 12-05-2011. At this time the EE was taken off work for P.T.
A Roof Fall was discovered at approximately 4:00am on the # 5 belt conveyor. The roof fall is located 1 crosscut inby survey station 3781. The roof fall is approximately 60' feet long x 20' feet wide x 6'-7' feet high. The roof fall was reported to the MSHA Hot-line at 4:10am.
Roof Fall was discovered on 4 North Mains at 3:45 A.M. by Dwayne Maggard Size of fall 30'L X 30'W X 8'H. Fall does not effect ventilation or impede travel, roof fall will not be cleaned up
Employee was lifting a 6"" hollow core concrete block, constructing a ventilation brattice, when he felt pain in his back and grion area.

Fall occurred on 12/14/10 found at approximately 6:45 am by Anthony Miller. Fall occurred on 1 West Mains in the #3 entry 2 cross cuts inby spad station 5605 and extended to spad # 3767. Fall was 100 ft long, 6 ft high, 20 ft wide. Fall did not alter the ventilation or affect the travel way. This fall was timbered off and danger signs were posted to restrict access to this area.
Employee stepped down out of supply building and tripped, fell landing on portable mine phone causing cut on left hand requiring (5) stitches to palm of hand.
Employee Slipped on mine floor and tried to catch himself with right hand. Employee suffered simple fracture to wrist, as a result of the fall.
Employee was assisting to put the section belt back together after it had broken. During the process a rope being used to pull the belt back together broke and struck employee in the face. Employee suffered a laceration to right cheek and jaw.
Employee had stopped miner to pull up miner cable for next cut. After pulling and lifting cable he felt pain in his lower back while walking across section. Resulting in a strain to the lower back.( Employee worked until 8/4/2010 and then became LT)
Roof fall was discovered in #9 entry of 1 North Mains. The fall measured 30' x 19' x 5' thick. Fall was dangered off and will not be cleaned up.

Employee stated that he twisted his left knee while he got up and turned around in the shuttle car he was operating.
Employee was helping put steel beams up using Hieuzman Jacks when the jack slipped out causing the steel beam to strike him on the back.
Roof fall was discovered on 1 Northwest Mains, 140 feet outby SS#3184, in #2 Entry. It measured 18' X 30' X 7'. Ventilation or travel was not affected.
Employee was struck by the roof bolter cable when it got jerked down due to the reel being on while the bolter was backing out of the heading.
While tramming a shuttle car, canopy got into the rib on the corner sending a small piece of coal into employee's left eye. Coal had to be removed by doctor.
Non-intentional, non-injury roof fall was discovered in the Northwest Mains, 90' inby SS# 3040 in the #4 entry. SIze of the fall was 16' wide x 12' long x 6' thick. Fall will be cleaned up and the top will be resupported.
Employee slipped on a flat rock covered in mud and fell. Injured right hip and back when he fell. Started to be Lost Time on 2/27/09 per Doctor's orders.
Employee was throwing a bundle of glue on the roof bolter, when he felt a sharp pain in his groin. He was diagnosed with a hernia.

Employee was putting roof bolt plate on roof bolt to install, and plate cut wrist requiring 2 sutures.
Employee was pulling a piece of drill steel out that had become lodged in the fender of the shuttle car. In the process, he dislocated his right index finger. Employee continued to work not missing any work days after initial accident. Surgery was performed on 10/21/08, and returned to work next scheduled shift, performing his normal duties.
Glove caught burr in drill steel, cutting hand while removing steel out of hole.

A rock fall was discovered during the normal weekly exams. The fall was in the number 1 entry of the south west mains approximately 200 feet inby S.S. number 416. The fall was approximately 20 feet wide, fourty feet long and six feet tall.
A roof fall was discovered by the mine examiner during his regular routine. The roof fall was in the #2 entry of the 1 NW Mains, approx. 60 feet inby spad no. 3020. The roof fall measured 20' wide by 20' long by 4' thick. It does not effect the mine ventilation. The area was barricaded to prevent persons from traveling in the area.

Lowering shuttle car canopy when canopy slipped it fell hitting rt hand (middle finger) between canopy and frame of car.
Roof fall was at Hales Branch Mains, outby spad # 1699 in #5 entry. It was 20' long X 20' wide and 5' thick. No equipment was involved and ventilation was not effected.
Roof fall was found by Preshifter at 2:55pm. It was in #5 entry on 1 north mains, spad # 3772. The fall was 20' wide X 5' long X 4.5' thick. It has been dangered off and will be cleaned up.
Employee put his hand around the base of the drill steel caught his glove between the steel and the chuck. When he started his rotation it twisted his finger resulting in a dislocation of his left ring finger.

OPERATOR OF ROOF DRILL WAS DRILLING A HOLE AND GLOVES HE WAS WEARING GOT CAUGHT IN DRILL HEAD ROTATING DRILL POT.
EE WAS CUTTING THE SUCTION HOSE ON THE ROOF DRILL AND THE KNIFE SLIPPED CUTTING HIS FOREFINGER OF HIS LEFT HAND, REQUIRING SUTURES. EE RETURNED TO WORK AT NORMAL DUTY ON HIS NEXT SHIFT.
A ROOF FALL IN THE PILLAR LINE IN THE #1 PILLAR BLOCK. ABOUT 5' TO 6' AT SPAD #1066. THE ROOF IS SHALE THE BOLTS INSTALLED WERE 4' RESIN. THE FALL CAUGHT A REMOTE CONTROLL JOY MINER THAT HAS B EEN REMOVED FROM THE FALL.
INJURED WAS IN STOOPED POSITION ALIGNING SPADS WHEN THE SHUTTLE CAR APPROACHED WHERE HE WAS WORKING WHEN INJURED REALIZED OPERATOR WAS NOT GOING TO STOP, HE GOT OUT OF THE PATH OF THE SHUTTLE CAR. HOWEVER, HE WAS CAUGHT BY THE SHUTTLE CAR CABLE AND THROWN AGAINST THE MINE ROOF AND CAUGHT AGAINST THE RIB.
WAS HANGING MINER CABLE AND FELT SOMETHING PULL IN HIS GROIN AREA.
