Two bolter operators had finished bolting the mine roof, backed the roof bolter up and started to hang a section of exhaust tubing. A large piece of rock fell out from the rib striking one employee on the leg causing a deep tissue contusion. IE was unable to return to EE's normal work duties. EE was placed on restricted duty restrictions due to weight bearing on the calf muscle.

IE had right hand placed on top of the miner, when a piece of cap rock came down from the top and stuck the IE on the right hand, causing a laceration and fractured bone.

Employee was operating the CM and was in the process of doing a final clean-up pass when a large piece of roof rock fell and struck him on the top of his right foot fracturing 3 bones.

Major roof fall behind shields caused large displacement of air. The air moving quickly and forcefully blew contractor down the headgate entry causing him to come in contact with the floor, head drive and crib block on the rib.

EMPLOYEE STATES AS HE WAS HANGING A SAFETY STRAP FOR THE VENT TUBE AND SLIDER, A SMALL SHARP ROCK FELL FROM THE TOP AND STRUCK HIM ON THE NOSE. HE RECEIVED A ONE-INCH CUT THAT REQUIRED SEVEN STITCHES.

WHILE EMPLOYEE WAS ON A LADDER ATTEMPTING TO CLEAR A CHUTE, ROCK FELL FROM AN OPEN CHUTE DOOR. THE EMPLOYEE FELL FROM THE LADDER AND ROCK FELL ON TOP OF HIM. HE WAS KOCKED UNCONSCOUS AND TAK EN TO THE HOSPITAL.
