Operator was spot bolting roof bolts, installed bolt while waiting on other drill man. He reached to get an 8 oz of drinking water. Pulled his glove off to pull tab, small pieces rock fell, striking the top of his right hand. Laceration causing cut (8 stitches in top hand).

Was loading supplies on drill or roof bolter. Small rock fell from mine roof approx 18" - rock 6" x 8" x 1" striking left hand on top. Simple fracture #3 finger bone on top hand.

EE WAS OPERATING A SCOOP WHEN A PIECE OF SLATE MEASURING UP TO 8'LONG BY 2"TO 23"WIDE BY 0-8" THICK FELL ON HIM. HE SUFFERED BURST FRACTURE TOVERTEBRA T-6 AND A MODERATE FRACTURE TO VERTEBRA T-5. SURGERY WAS SUCCESSFULLY PERFORMED 5/8/02.
