Employee working when a 40 lb roller fell 1-2 feet striking him on the side of his hard hat, knocking him off balance. Stated he felt o.k. and went back to work. Later felt stiff and went to FA. At FA he was placed in a cervical spine collar and placed on backboard & transported to CRMC. X-rays & CT were neg. Given presciption meds & placed on restricted duty.

THE CYCLONE HAS 2 DOORWAYS ABOUT 2' SQUARE & 12" DOWN FROM TOP ON OPPOSITE SIDES. THE OPERATOR SAW A LOOSE PIECE OF METAL INSIDE THE CYCLONE. HE WAS REACHING INSIDE TO REMOVE THE LOOSE PIECE OF METAL WHEN A CHUNK OF BUILDUP MATERIAL ON THE VERTICAL WALL ABOVE THE DOORWAY FELL HITTING THE EE'S HARDHAT & CAUSED IT TO COME DOWN OVER THE EE'S FACE & HIT HIM BELOW THE NOSE.

WHILE BENDING OVER TO GET WELDING SHIELD A DROP OF PHOSPHATE FELL BEHIND HER GLASSES AND INTO HER HER EYE AT 10:00 AM. SHE USED EYE WASH DURING THE DAY AND CONTINUED TO WORK. AT 2:30 SHE NOTIF IED HER FOREMAN AND HE TOOK HER FOR MEDICAL ATTENTION. 1ST PCS AND THEN BY THEIR RECOMMENDATION TO WASHINGTON EYE CARE.
