Employee was installing the finisher steel in hole. The steel got stuck so EE pulled it out some then tried to shove it in the hole. When the steel got stuck again EE felt a pop in left shoulder and experienced pain. Employee started missing time on 6/24/16.

Employee was lapping up approximately 50 feet of line curtain when they felt a sharp pain in their lower back. The curtain was covered with mud and water at the time of the incident.

Employee was dragging a vent tube when they felt a pop in right groin area. The employee continued to work till 6/22/2016 when they were diagnosed with a right hip flexor strain and taken off work.

Employee was helping pull the head shaft out of the feeder. The right side slid and came out sooner than the left side causing the right side to drop suddenly and the left side to rise up. The event pinched EE's wrist between the head shaft and the frame of the feeder resulting in a fracture to right wrist.

Employee stated was lifting a CM cable to drag another cable underneath. EE finished their shift and did not seek medical attention at that time. On 5/13/16, EE visited ER for evaluation and was released at own discretion. EE continued to work until 5/20/16 when EE returned to doctor. EE was given time away from job until 6/1/16.

Employee stated was throwing CM cable out of roadway when felt a pain in right shoulder and bicep. Employee refused first aid and finished shift and sought medical treatment the following morning. Employee visited RGH ER and referred to ortho doctor. Employee was excused from work through 5/6/2016 and remains away from work.

The employee was carrying a 5/8 inch breasting cable by the eye when employee felt pain in left groin area. The employee was taken off work on 5/5/2016 when diagnosed with a hernia.

Employee was removing an oil pump from a dozer with a chain hoist. As EE was holding it steady with EE's hand, the pump slipped from the chain hoist, pinching EE's finger. This caused a laceration that required 9 stitches to EE's ring finger

Employee was cleaning up the back of the warehouse around a separator drum. While cleaning up employee reached down to pull a piece of metal from under the drum when the magnet in the drum pulled the piece of metal to the drum mashing employee's thumb between the drum and the metal.

As EE was putting some hyd. hoses into a scoop bucket a couple of 9' timbers that were lying in the bucket became dislodged and fell onto EE's right hand. X-rays at the ER were negative for a fracture. EE continued to have swelling so returned to the doctor on 5/17/2016 where X-rays revealed a small fracture and the doctor recommended physical therapy

The employee was removing the miner cable from a hanger when the hanger swung over and chipped one of their teeth. This was originally a report only accident. The employee has since been treated by a dentist and is now being reported as a Medical Treatment accident.

The employee was in the process of removing a rock from ATRS on a Fletcher DDR roof bolting machine when EE felt a pain in stomach. EE was diagnosed with an abdominal hernia on 4-4-16 and began missing work on 4-5-2016.

EE was unhooking an Add car cathead. EE placed EE's foot on the top of the cars push arm. EE's foot slipped off and was caught between the push arm and the flipper jack. EE quickly removed EE's foot and received a stretched tendon attached to a bone in the top of EE's foot. Medical evidence did not revealed injury until EE received an MRI and it was read (4/22/16).

EE alleges while hanging an empty 2 inch waterline EE felt pain in EE's lower back. EE has not missed work, EE began working restricted on 4/12/16. Alleged incident is under investigation. Due to the reporting deadline, this document is being submitted to comply with reporting criteria until full determination of actual injury and/or incident can be substantiated.

Employee was in the process of plugging the continuous miner cathead into the designated receptacle at the section power center when EE felt a strain in the lower right portion of their back. Employee continued to work until 5/17/16 when EE was diagnosed with a herniated disc on 5/18/16.

The employee was in the process of securing a tow bar onto the end of a Brookville 15 ton motor when the bar shifted and pinched the employee's right hand. This resulted in a laceration to the right hand requiring 7 sutures.

EE was moving the miner cable out of the roadway when the bolt machine was backing up the bolter cable caught the employee's left leg and caused it to bend backwards injuring EE's left knee. This employee did not seek medical attention at the time of the incident. EE went to the doctor on 3/14/2016 in which it was determined EE could not return to work pending on an MRI.

Employee was removing pin from steering cylinder to the bell crank. As pin came out, cylinder shifted forward and pinched finger between cylinder and bell crank. This smashed the finger.

Mechanic was putting away a tie rod off of a CAT 789 haul truck. Six days later they informed their supervisor that they thought they received a back injury putting away the tie rod. The EE's medical records indicate a history of back pain and sciatica which is what the EE is being treated for. Medical reports also indicate that sciatica can be caused by herniated/slipped discs.

Employee was moving shuttle car anchor and was letting down the slack that was hung by a rope hanger, the end of the rope hanger hook swung up hitting employee on the nose and when employee jerked head back the hook caught on the inside of nose resulting in 5 stitches, employee returned to work the next scheduled shift.

The employee was struck in the right cheek with a metal wedge from a wedge socket that was being used to pull belt onto belt structure. The wedge socket connection failed causing the wedge to fly thru the air and struck the injured causing a laceration and broken orbital bone.
