Employee was lifting a 50 pound speed reducer, he said it slipped in hands and he caught it again, causing pain in stomach. Employee did not report accident until next day, 11-05-08, after he worked half day he said he was going to Doctor. The employee called and said he will need hernia operation.

Injured said he cut his finger while unloading roof bolt plates. On 10-3-07 went to Dr. 0n 10-09-07 with infection on right hand little finger. Reported accident on 10-19-07 to foreman. He worked till 10-10-07 and called in sick, reported to us on 10-19-07 it was the result from cut on 10-3-07.

Conveyor on a bridge unit was stopped up with coal, while removing the coal, EE dialocated his shoulder. He said his shoulder had been dislocated before and it was an injury that happened before. Shoulder sometimes became dislocated.
