EE was helping make belt move, he was pulling on structure and slipped hurting his back

Employee was suppling the bolter when he walked into a roof bolt hanging from the top and hit his head, spraining his neck.

Using hammer to splice belt. Piece of metal chipped off & hit right wrist.

Unknown. Employee left property. Refused medical treatment. Hasn't made contact with any company officials. Will not return calls.

Employee panicked and drove 4-Wheeler into berm.

Loading tool box on 4-wheeler, he hurt his back.

Employee attempted to pick up a rock that had fallen off the stacker belt & he thinks he pulled a muscle in his back.
