Employees were pulling a vertical turbine pump, had placed a joint of column pipe on lay down timbers. As the pipe reached the last timber, it was slightly higher, so employee attempted to lift the pipe and exacerbated a recently repaired hernia. Surgical repair slated for 05/21/26. Currently ONGOING.

Employee was walking towards the centerline during shift change using the designated path to the ladder. As they were walking towards the ladder the employee noticed that the pathway sloughed causing the employees left leg to sink up to the knee. This position caused the employees ankle to experience discomfort. The employee went restricted duty 5/5/2026.
A Shovel/Drill employee was removing snubber pins to perform a re-shim of the snubber. While using a digging bar to drive out the pins, the pin suddenly released, causing the snubber to drop. This resulted in the employee's left pinky finger being pinched between the dipper bore and the digging bar. Employee was given restrictions on 4/25/2026.
Employee used Brakleen to remove hydraulic oil from their hands; after doing so they took a smoke break causing burns to their hands.
Employee was walking crouched down in a low overhead clearance area and felt a pop in EE's lower back, followed by pain. EE was taken to the clinic, given work restrictions the company is accommodating. CURRENTLY ONGOING

Mill Operations Employee was cleaning the discharge cylinder at the Lime Plant. When the valve on the hose was opened, compacted lime discharged back toward the Employee and went under the face shield and safety glasses, contacting the eyes.
An employee was operating a drill press in the maintenance shop when metal shavings became lodged in the equipment. The employee stopped the drill press and attempt to manually remove the obstruction, and a sharp metal shaving penetrated the glove and caused a laceration to the employee's left index finger. The employee received stitches for their injury.
Employee was rolling up mantos on a leach stockpile, when their right forearm became irritated by the raffinate solution. The employee wash off arm with 4 bottles of water and reported incident to supervisor. The employee was taken to clinic to be evaluated.

A Morcon ops employee was washing off material behind the ball mills area when they took a break. Upon returning from the break they went to bend over to pick up the water hose to continue spraying down area when they felt a pain in their back.
The employee had noticed red on the under part of their eyes 2/18/2026 at the end of their shift and over the next couple of days a rash appeared and on 3/4/2026 the employee reported the injury when coming back to work due to the condition not improving.

While changing rollers in an impact zone a roller frame assembly fell smashing employees left thumb causing a laceration requiring 12 sutures.
Employee was picking up parts for their task and one of the required parts the employee needed was a cylinder for the dust curtain on the drill. While lifting the cylinder, the employee stood upright and experienced a sharp pain on the left side of the body.
During a maintenance activity, a maintenance employee was replacing some dust collector ducting in the tunnel when they pinch their right index finger.
Employee sustained a laceration to right hand ring finger when it was cut by a burr on the inside of the drill pipe and received 5 sutures. Employee identified one of the burrs, but not the second. EE had Impact Cut 5 gloves on at the time of occurrence.

Mechanic sustained a hand laceration while replacing air filters on haul truck within the bay.
Crane Operator was preparing the rig to be strapped to the truss. They placed a shackle on top of the metal beam to position themselves for rigging. The shackle off the beam and fell approximately four feet, striking the employee on the back of right index finger.
Employee was lifting a desk with other employees into the back of a pick-up truck. When the employee bent down to lift and stood up, EE felt pain in EE's back and down EE's right leg.
Employee was attempting to open the back door of a UTV, and the front seat passengers opened their door to assist. Employee was able to get their door open before front passenger exited the UTV and placed their hand on the door jam. During this time the front seat passenger closed their door and smashed employees' thumb in door.
While walking to the HT629 at Shannon Q-Point, the dayshift operator tripped on a rock and fell forward. At the time of incident, the operator was wearing their backpack, carrying a water bottle and an additional bag full of cleaning supplies. The operator stated they normally use a flashlight while walking but were not at the time of this incident
Employee experienced Personal Medical causing them to injure their foot while stepping out of shower.
During an attempt to remove rigging from a hook on a forklift, the operator extended the boom to assist the rigger. Unfortunately, this action caused the rigger's hand to become trapped between the knuckle of the mat and the item they had just lifted, resulting in three broken bones in EE's right hand.

when entering the lube room a 1" shackle fell and stuck EE on top of right foot behind steel toe protection. EE does recall where it fell from when EE entered the lube room.
Employee was taking apart a 3-inch hose and the camlock struck EE in the lower lip, causing a laceration.
EE was installing belt on pulley. EE's finger became caught between the pulley and the belt, amputating the tip of EE's right ring finger. EE was taken to the hospital for treatment. Taken off work.
