Employee overfilled the Diesel Exhaust Fluid tank (DEF), causing it to spill on their clothing & glasses. A drop of DEF entered left eye. They immediately flushed with water, then changed into clean clothes. They requested a medical evaluation to ensure eye was okay. They were taken to ER, where they were examined & medically cleared to return to work without restrictions.

Showing all 12A mechanic was working near a guard that was being set in place by 2 other employees. The guard slipped out of the 2 employees' hands. It slid to the side and pinched the mechanic's left ring finger between another piece of metal. The mechanic had a small puncture wound that went through glove. Injury required 2 stitches at ER.
Mechanic was changing forklift tires. While rolling the old tire away, the tire fell towards them. The tire was too heavy for them to stand back upright. The tire slipped from their grip, landing on their ankle. The ankle became swollen. The incident was reported to supervisor and the employee taken to emergency room. X-rays confirmed left ankle fracture.
A mechanic leaned over to pick up some tools and felt a twinge in EE's back. The employee was seen at an occupational clinic.
Employee was removing small rocks from feeder when larger rock broke loose. It fell and hit EE's outer left ankle.
An employee was moving a large rock from the oversized belt. The rock rolled, pinning the employees left pinky finger against the side of the screen.

Employee reported injury on 2/5/16. Employee states on 12/8/15 she felt a pop on her right knee when closing the door to the scale house.
Employee was helping change the shoes on the vsi crusher. Weather was clear and sunny, about 75°F.
Employee needed to load the air compressor in the back of his truck to take to another location. He had oxygen and acetylene bottle in the back of his truck and needed to move them out of the way. He picked up the acetylene bottle to lay it back down in the bed, when he did this he dropped the bottle of top of his foot.
Was unloading rock screen from a bucket of a loader. As they was lifting the screen the other man lost his grip at this time the screen caught the mans glove and pulled him to the ground causing him to hurt this hand.
