When replacing the windshield on a front-end loader, the employee cut right index finger resulting in stitches.
Employee was getting out of a pickup to throw away trash and when their left foot contacted the ground, they felt a pain in their left knee. Medical attention was sought, and crutches and a knee brace were issued.
Employee was assisting another employee helping to put a clevis on a machine. The clevis rotated and the employee's finger was pinched between the machine and the clevis.
Employee was attempting to dislodge a small tree limb that was stuck behind the blade of EE's bulldozer. When EE let go of the limb, stored energy caused it to snap back, pinning the tip of EE's left little finger against the dozer blade and fracturing the bone.
Employee was replacing an axle shaft of Komatsu HD1500-7 Water Truck. Employee found the axle bushing was stuck and used a glove hand to attempt to lift the sun gear side of the axle to free it. Upon pushing upwards on the sun gear, the employee lost grip on the oily surface and EE's forearm contacted a spline on the sun gear resulting in a 1.5" laceration requiring 5 stitches.
Employee was installing a pin into the back of the dozer arm, the pin moved unexpectedly and pinched two fingers on left hand. This resulted in stitches and time away from work.
Two employees were at the coal barn installing a wing shroud (steel plate) on a dragline bucket. The upper piece was set in place and the crane was lowered. Both employees were going to adjust the piece of steel plate by hand when it fell. It slid down the side of the bucket, hit the ground and fell over hitting the employee breaking two Metatarsal bones in EE's right foot.
While setting up to change the hoist ropes on 102 dragline a pin that secures the shackle of the snatch block was not screwed in. This resulted in the shackle spreading apart and the snatch block popping out hitting the employee in the leg breaking the left tibia.
Employee was maneuvering becket end of pendant lines into a spool for storage. Employee had fingers between cotter key keeper on the pin and becket end. When the becket end was positioned onto the spool, back of pin made contact with spool sliding pin away pinching finger.
Employee was replacing a 2' x 4' x 3/8" piece of steel plate on a bulldozer blade. The employee stood the plate up resting the 2' edge on the floor and begin walking the plate corner to corner to the center of the blade. Once they got to the center of the blade they began laying the plate down on its opposite edge (4' side)when the injured employee felt a pull in left arm.
Air hose reel malfunctioned and stopped rotating suddenly while employee was pulling hose out. When the hose stopped, the employee felt pain in shoulder.
Employee lifted the end of the two cutting edges that were partially on a pallet, when employee went to set the two down they slipped off the pallet causing the two cutting edges to slip pinching employee's right pinky finger.
While on a break from ERT training, two employees were looking at a pocketknife. The injured employee made a motion to point out a function of the knife at the same time that the employee holding the knife happened to flip the blade open. The knife blade cut the injured employees right index finger and sutures were required to close the laceration.
Moving a part, slightly off ground, had a poor grip, hit a toe. Was wearing steel toe boots, injured a toe.
Employee was lifting a 4 inch suction hose off pump truck and when turning on uneven ground felt a pain in EE's back.
While finishing drag rope change out on a 1570 dragline. The winch line dropped out of the horizontal fairlead sheave, employee was guiding the cable back into the sheave. During this process employee's left thumb was pinched between the outer and inner horizontal sheaves fracturing the thumb. The employee was wearing leather gloves at the time of the incident.
Employee was working on rebuilding a planetary. The planetary had 3 gears that were presumed stuck, a pry bar was used to free the first 2 gears. On the 3rd gear the employee was showing the shop planner that the gear was stuck. EE gently wiggled the 3rd gear & unexpectedly it freed up causing EE's finger to pinch in the housing. EE was wearing a latex mechanic glove.
Employee completed a pre-shift inspection on a CAT 785C water truck. Employee was wearing insulated gloves. Operator picked up the wheel chock block and was installing it in the holder located on the front bumper of the truck. The left middle finger tip was caught between the chock block and the metal chock block holder. Finger tip bone was fractured.
Employee was struck by a falling ladder.
Employee was attempting to set a tumbler into position. The employee placed right hand in a pinch point between the tumbler and the brass sleeve. The contact from the tumbler being placed into position resulted in a laceration to EE's right hand middle finger that required sutures. The employee received a regular duty work release.
Employee had removed mud from a loader's hitch pin and was trying to push it down with hands to get it back into place when the pin let loose. The employee grabbed the head of the pin which caused an injury to a finger on both hands. Employee was transported for medical attention and received stitches.
While pulling a dent out of a fender, employee felt pain in the upper arm. This resulted in a strained pectoral muscle.
Employee experienced muscle damage (bicep) while helping another employee lift a shovel dipper tooth.
Employee was adjusting a steel beam that was unbalanced. It shifted and landed on their steel toe boot, causing a fracture to the big toe.

An employee was fabricating a swing case stand while working on a welding table. During the process the employee attempted to flip the piece over to continue welding and in doing so the steel frame fell off the table. The frame landed on the employee's foot, fracturing the foot.
Supervisor was loading water transfer pump fittings into the bed of pickup truck and cut left thumb requiring 3 stitches.
A 500 ton hydraulic ram fell onto the employee's foot causing a fracture.
Employee was riffling a 14 pound sample of coal. As they were dumping the sample they felt a sharp pain in the middle of the lower back. Co Worker helped them sit down and MRT was called. Employee was taken to hospital given pain medication and restricted duty.
Employee was placing a 8"X8"X24" timber underneath a dragline bucket jewelry spreader bar and pinched left index finger. Employee was wearing leather glove at the time of the event. EE left index finger tip was fractured.
Employee pinched finger between synthetic sling and jib crane hook.
Employee was replacing batteries on a track backhoe. EE slipped on the track and the battery fell on hand, causing the injury.
Employee was kneeling down changing a tire on a pickup in the shop, while doing so employee's legs fell asleep. Employee went to pull themselves up by grabbing the side of the pickup and while doing so employee said they felt a tear in the lower abdomen.
Employee got fingers caught in the door frame of a CAT 777D End Dump truck while employee was trying to open the driver's side door which was stuck. Employee was transferred to hospital where employee was diagnosed with 2 fractured fingers on employee's left hand.
Employee was attempting to hang coat in the locker when EE received a static shock from the coat. Employee jerked hand back resulting in a cut on left thumb from the edge of the locker.
Employee opening the rear doors on a 53' tool trailer to gain access to the inside. Employee had release and pulled on the inner latch handle and was attempting to unlatch the outer latch when the handle came toward employee striking employee in the upper right lip. Employee received 8 sutures in upper lip. No equipment damage.
Employee was boarding a Cat 777 Truck when employee felt pain in their lower left leg. Medical treatment followed.
Employee was dropping the belly pan on a D6 caterpillar dozer. The jack under the belly pan slipped as the pan bolts were removed from the frame. The right forearm was contacted as the belly pan fell from the jack. X-rays were negative for broken bones. The cut did not require stitches, however the doctor applied glue to protect the wound.
Tire technician was in the process of dismounting a 33.00X51 tire on the LF Steer of an end dump rock truck. When technician released the lock ring, it fell and rolled around and struck the technician's right big toe.
Employee was aligning a 6" clay valve and strained their back.
A mechanic was installing a pull block on the rear of a scraper. Employee got finger pinched between the pull block and a backup alarm mounting plate. The employee fractured the tip of right index finger.
Employee was in the process of setting up a light plant when a chunk of mud fell from the light fixture striking the employee on the left side of the face between the hard hat and safety glasses. Mud entered the left eye behind the safety glasses.
Operator placed lunch box in cab. As ee stepped back to exit the cab foot slipped off the step. The operator reacted by swinging arm to catch self and the back of right hand contacted the handrail. An x-ray showed a cracked bone in right hand.
Employee was rolling up power cable. EE got off the truck to tie the rope to the cable. While rolling up the rest of the cable/rope the employee was unaware ee was straddling a loop in the rope. As ee rolled the cable up leg got pinched between the rope and equipment, injuring employees left leg. Employee was unable to return to work on next scheduled shift.
Employee was washing a metal Warehouse window with a rag when EE received a cut on EE's left thumb from the edge of the window frame. Stitches were required to close the wound.
Worker was re-spooling cable back into Ericson, P/N # 6123-100, cable reeler in the machinery house space of the 8400 dragline. The end of the cable whipped into his right eye, breaking his safety glasses and caused trauma to the eye. Worker was taken to the Sakakawa Medical Center in Hazen and then to a hospital in Bismarck. He was released the night of 10/23/15
Employee was stowing the safety cable for the box of an overburden truck. The employee had trouble getting the clevis onto the hook. When the cable was moved the clevis pinched the employees finger against the box. The employees finger was cut and finger tip was fractured. Stitches were required.
