Employee was installing cutting edges on Cat D11R dozer. He turned one of the edges over and it fell on a pry bar and the bar came up striking right lower leg causing lacerations that required 11 stitches.

Showing all 28Employee was helping air conditioner repairman change the coils on a/c unit in 777 rock truck. Repairman tossed a gallon tub of cleaner to employee and it hit the handrail and the lid came off and splashed into employee eyes
Employee was assisting welder lift a piece of metal out of the back of truck. Metal weighed approx. 150 lbs. Felt sharp pain in lower back just as metal got to the ground.
EE was removing cap from air line and failed to completely turn off valve allowing air pressure to be under cap. When cap was removed it struck EE on hand. EE was operating Cat 994 endloader.
Helping clear a blockage in pipe in washer plant, employee was reaching and pulling and felt a pain in his right side. Went to doctor on 3/14/11, pulled muscle in lower back.
Employee was laying a 2" cable to pull a tooth onto a 994 bucket with the help of the customer's mechanic. The customer's mechanic let go of the cable without warning, causing the cable to fall across employee's finger, cutting the finger.
EE was helping move an electrical cable when the cable suddenly moved after being caught on something and pinned EE's hand between the cable and the end loader bucket. This caused a laceration which required stitches.
Employee was installing a muffler system on CAT 245 Excavator when he strained a muscle in his left shoulder. MRI is scheduled.
Employee was helping install a bed on water truck. He was helping guide bed down into place and the bed shifted and he sprained his wrist while trying to keep bed in place.
Mechanic was removing D10 engine. The nose guard had been rolled over and 2 hydraulic lines were still attached and under tension. When 1 line was removed the remaining line could not hold the load and pulled loose striking mechanic on the hand.
Employee notified company on 12/14/06 that he injured his back on 12/5/06 climbing on or off a D9L bulldozer. He went to doctor and had an MRI done. Test show ruptured disk in lower back. Doctor is saying surgery is necessary.
Employee was doing a routine check on caterpillar 785 rock truck and as he exiting from underneath the truck he hit his left shoulder on a pull hook under the bumper and then hit the side of his head on the bympen. He wearing hand hat.
Employee was helping remove engine cover plate on 980C endloader and got his thumb caught between the cover and another piece of metal when setting the cover down.
Employee was working on the emergency brake on a Caterpillar TC-30 Forklift and strained a muscle in his neck. He reported it on 2/15/2006 and went to the doctor on 2/15/2006 and a follow up visit on 2/17/2006 when the doctor put him off duty till further notice.
Employee was climbing up on a Caterpillar 785 Rock Truck to perform some mechanical duties and as he pulled himself up on the ladder he felt a pain in the lower part of his back. He is scheduled to take physical therapy for a strained muscle in the lower back and is not released to come back to work yet.
Employee said he was starting a Honda Pump to fuel a 992C Endloader and when he pulled the starting rope he felt a pain in his lower back. He did not go to the Doctor until the following week and Doctor told him to take off work starting 04/20/2005 until further notice.
EE was bent over putting an oil pump on the air compressor of a drill when he felt pain in his lower back on the right side.
EE WAS UNLOADING A ROLL OF BELT LINE OUT OF PICKUP TRUCK AND FELT PAIN IN LOWER BACK. HE KEPT WORKING UNTIL 10/21/03. PAIN HAD NOT GOT ANY BETTER AND HE WENT TO DOCTOR. TOLD BY DOCTOR TO STAY OFF WORK UNTIL RESULTS OF MRI WAS BACK.
EE WAS STARTING A HONDA PUMP TO WASH EQUIPMENT AND WHEN HE PULLED ON ROPE HE FELT A SHARP PAIN IN HIS BACK. HE WORKED THE REMAINDER OF THE SHIFT AND ON MONDAY 8/18/03 HE SAID BACK WAS WORSE AN D WAS GOING TO THE DOCTOR.
EMPLOYEE WAS CLIMBING OUT OF MACK 800 WATER TRK AND WHEN HE STEPPED ON THE GROUND HE TWISTED HIS KNEE.
EE HAD HELPED CHANGE CUTTING EDGES ON D-11 DOZER & BUCKET TEETH ON 944 ENDLOADER WHEN LOWER BACK STARTED HURTING. WENT TO EMERGENGY ROOM. X-RAYS NEGATIVE - PRESCRIBED MUSCLE RELAXERS & SENT HO ME FOR A DAY.
EMPLOYEE LIFTED SAMPLE BAGS AND COMPLAINED OF BACK PAIN. WENT TO DOCTOR ON 1-9-02 AND WAS TOLD TO TAKE OFF UNTIL 1-11-02 AND RETURN FOR TESTS.
EE HAD HELPED MECHANIC FIND AND REPAIR AN AIR LEAK THAT WAS ALLOWING DUST INTO THE DUCT WORK AND CAB AREA. EE REMOVED LOUVER FROM DUCT AND WAS CLEANING DUCT WORK WHEN FINGER GOT INTO FAN BLADE.
EE WAS HELPING INSTALL A RETURN ROLLER ON C BELTLINE. WHEN HE PICKED UP THE ROLLER HE FELT A PAIN IN LOWER BACK.
EMPLOYEE REMOVE BRAKE CALIPER FROM FRONT WHEEL ON 777 ROCK TRUCK WHEN THE CALIPER STARTED TO FALL EMPLOYEE TRIED TO CATCH IT AND IT PULLED HIM TO THE GROUND INJURING HIS BACK. CALIPER WEIGHED APPROX. 150 LBS.
EMPLOYEE WAS PUSHING OPEN A GARAGE DOOR TO INSTALL SOME LIGHT AND FELT PAIN IN HIS RIGHT FOREARM. HE REPORTED IT TO COMPANY OFFICIAL BUT DID NOT GO TO DOCTOR. PAIN GOT WORSE AND EE WENT TO DOC TOR ON 3-31-01. DOCTOR PRESCRIBED MEDICATION ANDSPLINT TO HELP IMMOBILIZED ARM. RETURNED TO WORK.
EMPLOYEE SHOVELED COAL FOR TWO HOURS WHEN A BELTLINE STALLED OUT AND WHEN HE GOT UP THE NEXT MORNING HIS BACK WAS HURTING. IT GOT WORSE THE 2ND DAY AND HE WENT TO THE DOCTOR WHO TOLD HIM TO BE OFF FOR TWO OR THREE WEEKS.
EE WAS INSTALLING A BELT SCRAPER RUBBER ON BELT LINE AND WAS LINING THE BOLT HOLES UP WHEN HIS HAND SLIPPED AND HIT A PIECE OF TING COVERING THE BELT LINE, CAUSING A SEVERE CUT TO THE TOP OF T HE RIGHT HAND.
