Employee stated that as he moved a shield, a rock fell from the canopy, striking him in the right leg.
On 4/12/05 completion of an investigation and review of medical records, indicated the employee suffered a second hernia while exiting a Wagner Scoop.
The employee was determined to have a hernia on the right side. He believes that it occurred on 02/07/05 while assisting the setting of Big John roof support. (4/18/05 per call - hernia repaired)
EE INJURED HIS BACK WHILE HE WAS ASSISTING COWORKER IN HANGING THE CONTINUOUS MINER CABLE FROM THE MINE ROOF. EE CONTINUED TO WORK UNTIL 11/8/04 AT WHICH TIME HE WAS TOLD THAT SURGERY WOULD BE REQUIRED.
While performing routine inspections of fire extinguishers the technician pulled the nozzle away from the tank and came in contact with the retaining hook on the unit. the hook was very sharp on the end and cut the technician's left ring finger, leaving a laceration that required sutures to close. Employee was not wearing gloves at the time. Employee was released to unrestricted
While changing a pin on the mast tilt cylinder of a roof bolter the mechanic's finger was pinched between the drill mast and puller bolt leaving a laceration that required sutures. Employee was released to unrestricted duty.
The employee attempted to cut a wire band on a bundle of wire roof mesh to separate the smaller bundles. The mesh sprung and then recoiled back into the employee, pinning him between bundles of mesh.
The injured employee was repairing on the unit 2 roof bolting machine. he had removed the rear main control panel and as he was possitiong the cover during reinstallation he felt a sharp pain in his low back. This is incident has been under investigation awaiting information from the dr. ee should always use proper body position and ask for help when handling heavy objects.
Mechanic was installing a metal cover on the continuous miner when he mashed his index finger on his right hand. The finger had a laceration that required stitches to close.
Employee states that he was in process of unloading shovel bucket pins from the back end of his service truck. As he was rolling or scooting the pin from his truck, after it landed on the bumper, it either bounced or flipped and struck his right hand. Three of his fingers were mashed with the index finger being fractured.
Employee states that on July 8, 2004, while moving and handling de-watering pumps and waterline he strained his back. Employee continued to work and then on July 31, 2004, employee started missing work. Note: This injury was not reported to management until August 25, 2004.
While bending over to pick up a piece of paper between seats in mantrip van, ee twisted his back. He has had a history of back problems. Reason being late mine has been on vacation shut down.

EE WAS ASSISTING GROUNDMAN IN MOVING TAIL CABLE FOR THE 2550 DRAGLINE. INJURED EE HELPED TO LIFT TAIL CABLE OVER A ROCK & FELT A PULL IN HIS BACK.
WHILE REPLACING THE REAR SWING BRAKE ON THE SHOVEL THE EYE BOLTS IN THE SWING BRAKE PULLED THE THREADS OUT AND FELL AND STRUCK EE ON HIS RIGHT FOREARM.
EMPLOYEE LIFTED A 2550 TOOTH POINT OFF THE SHOE OF THE DRAGLINE & FELT PAIN IN HIS LOWER STOMACH AREA. BECAUSE THE PAIN DID NOT GO AWAY, EE WENT TO HIS DR. ON 4/30/04 WHERE HE WAS DIAGNOSED WITH A HERNIA. EE CONTINUED TO WORK UNTIL 5/4/04 WHEN HE WENT IN FOR SURGERY.
Employee stated that he was in process of tightening roller cap bolts on a D-10 bulldozer. During the process, he inadvertently struck the trigger causing the impact wrench to thrust his right hand against the roller. He sustained a bruise and later on, was told that he may have a torn ligament in his knuckle area. Note: Employee did not start missing work until May 5, 2004.
EE WAS STANDING IN MUD & WAS LIFTING A TRACK PIN WHEN HE FELT PAIN IN LOWER BACK
EE STATED THAT ON 4/8/04 HE WAS HELPING LIFT A TIMER WHEN HE FELT PAIN IN HIS BACK. HE ALSO STATED THAT ON 4/14/04, WHILE LIFTING CABLE HE FELT PAIN IN HIS BACK. HE STARTED MISSING WORK ON 5/17/04 AS A RESULT OF HIS INJURY AND RET'D TO WORK ON 5/26/04
EE ALLEGES THAT WHILE HELPING LIFT PUMP, HE FELT PAIN IN HIS CHEST.
EMPLOYEE STATED THAT WHILE SHOVELING IN DRILL CUTTINGS IN EXPLOSIVE HOLES, HE FELT PAIN IN HIS SHOULDER. EMPLOYEE DID NOT REPORT THIS UNTIL 4-19-04 TO ANYONE. WE INVESTIGATED THIS CLAIM AND OUR COMP CARRIER DECIDED TO ACCEPT IT. THE REPORT IS FILED LATE FOR THAT REASON. EMPLOYEE WORKED UP UNTIL 4-19-04 WHEN HE REPORTED THE INJURY. HIS FIRST DAY OFF WAS 4-19-04.
EE was working on the belt crew. As he was helping thread the belt around the head roller he felt a pain in his left shoulder. The incident caused a strain to his shoulder. EE will be instructed to always use proper body position and get help when the material is heavy.
WHILE ASSISTING IN THE RECOVERY OF A KENNEDY STOPPING, A LOOSE PANEL SLID UNEXPECTEDLY AND CUT THROUGH EMPLOYEES GLOVE, LACERATING 3 FINGERTIPS ON HIS RIGHT HAND.
EMPLOYEE WAS LOADING A BELT WINDER CORE INTO A PERSONNEL CARRIER AND CAUGHT HIS RIGHT FORTH FINGER BETWEEN THE BELT CORE AND THE PERSONNEL CARRIER FRAME. THE INCIDENT CAUSED A FRACTURE TO THE RIGHT FORTH FINGER. THE EMPLOYEE WILL BE INSTRUCTED TO ALWAYS WATCH FOR PINCH POINTS AND BE AWARE OF HAND PLACEMENT WHEN WORKING IN CONFINED PLACES.
While shoveling coal from catwalk on left side of launch pad, ee said after 2nd shovel of coal he felt 2 pops in lower back.

EE WAS LIFTING A BELT ROLLER WHEN HE FELT A PAIN IN HIS LOWER BACK AND LEFT LEG. HE HAS CONTINUED TOWORK UP UNTIL MAY 11, 2004 WHEN HE STATED THAT THE PAIN IN HIS LEG HAD GOTTEN WORSE.
EMPLOYEE WAS LIFTING A 5 GALLON CAN OF HYDRAULIC OIL WHEN HE FELT A SHARP PAIN IN HIS LOWER STOMACH. HE WAS DIAGNOSED AS HAVING A HERNIA.
EMPLOYEE WAS LOADING ROLLERS WHEN ONE SLIPPED STRIKING EE IN RIGHT LEG. RETURNED TO WORK THE SAME SHIFT.
EMPLOYEE WAS CHANGING A STABILIZING JACK ON A STAMLER BELT FEEDER WHEN HE GOT HIS FINGER CAUGHT BETWEEN THE JACK AND THE FRAME OF THE FEEDER. THIS RESULTED IN A LACERATION THAT REQUIRED STITCH ES.
EE WAS USING A FRONT END LOADER TO PICK UP SCRAP METAL. HE SAID THAT HE INJURED HIS BACK WHILE SLIDING A PIECE OF SCRAP METAL INTO THE BUCKET OF THE LOADER. MISSED WORK 1-604.
EMPLOYEE WAS PUTTING THE RIGHT FORK BACK ON THE FORKTRUCK. HE LIFTED THE FORK TO SLIDE INTO PLACE WHEN IT SLIPPED AND FELL TO THE FLOOR, PINCHING THE MIDDLE AND RING FINGER ON HIS RIGHT HAND.
THE INJURED EE WAS WORKING IN THE PREPARATION PLANT CLEANING UNDER A SCALPING SCREEN. AS HE REACHED UNDER THE SCALPING SCREEN HE FELT PAIN IN HIS LEFT SHOULDER. THE EE WILL BE INSTRUCTED TO ALWAYS USE PROPER BODY POSITION WHEN PERFORM-ING A TASK.
EE WAS IN THE PROCESS OF CLIMBING DOWN FROM SCRAP METAL BIN WHEN HIS RING FINGER GOT CAUGHT ON THE CORNER CUTTING HIS FINGER. THE LACERATIONREQUIRED 6 STITCHES.
USING 2 COME ALONGS TO LIFT TILT CYLINDER FOR INSTALLATION. CYLINDER SLIPPED THROUGH CHAIN ON 1 COMEALONG AND FELL. LEFT THUMB WAS CAUGHT IN THE CHAIN OF THE OTHER COME ALONG CAUSING A LACERATION
EE STATED THAT AS HE TRIED TO TIGHTEN A CHAIN BOOMER IT SLIPPED OPEN AND STRUCK HIM IN THE MOUTH.
EE WAS WORKING IN THE OLD UNIT 3 RECOVERY AREA. HE WAS IN THE PROCESS OF SECURING A ROLL-UP CURTAIN WITH THE ATTACHED CHAIN. AS HE WAS SECURING THE CURTAIN HE CAUGHT HIS LT.HAND BETWEEN THE CU RTAIN & THE CHAIN. THE INCIDENT CAUSED A CONTUSION & STRAIN TO HIS LT. HAND. HE MISSED A TOTAL OF 13 DAYS, HE IS STILL UNDER THE DR.'S CARE AT THIS TIME. THIS INCIDENT IS UNDER INVESTIGATION.
EE STATES HE WAS IN PROCESS OF OPENING A DOOR ON THE WET BAG POWDER TRAILER AS HE TOOK HIS HAND OPEN THE DOOR THE HANDLE SUDDENLY CAME AROUND AND STRUCK HIM ON THE UPPER LIP, CAUSING A LACER ATION NOTE: PLEASE SEE ATTACHED MEMO
WHILE REPLACING DAMAGED CONVEYOR BELTING, OLD BELT TORE UNEXPECTEDLY, STRIKING EE IN STOMACH. INITIAL DIAGNOSIS WAS BRUISED ABDOMEN, FOLLOW UP CONSULTATION INDICATED 2 FRACTURED RIBS. RELEASED TO UNRESTRICTED DUTY.
