The employee attempted to stop a stack of roof mesh from tipping over. He tripped getting out of the way and the mesh struck him as he was on the mine floor.
This employee attemped to move and set a shield by remote means. The shield appears to have set by the auto-set function. The side seal was over lapped and the side seal came off striking the employee.
The employee attempted to move a 7 5/8 drill steel. The steel rolled into another steel pinching the left middle finger.
A shackle broke while pulling on the mobile tailpiece of the longwall with a "miner puller". A piece of the tow chain struck the employee.
The employee was helping to move a steel roof support beam over the stageloader. This employee pinched his right index finger between the beam and the electrical nip on the stageloader.
On 07/22/05 the employee reported C-spine injury from an injury sustained on 07/10/05. The employee was setting secondary support along the stageloader when a piece of roof rock struck him on the head.
A fall of roof prevented travel off of the Head Gate of the B-1 panel at crosscut 40.
Employee stated that as he moved a shield, a rock fell from the canopy, striking him in the right leg.
Fell while moving steel on the door.
On 4/12/05 completion of an investigation and review of medical records, indicated the employee suffered a second hernia while exiting a Wagner Scoop.
The employee was determined to have a hernia on the right side. He believes that it occurred on 02/07/05 while assisting the setting of Big John roof support. (4/18/05 per call - hernia repaired)
Employee was performing work involving spraying of dry mix shotcrete. Employee wore appropriate PPE. After work employee noticed persistant eye irritation from caustic shotcrete dust and received eye irrigations and antibiotic ointment as precaution.

Employee stepped off miner car and when foot touched pad, slipped and twisted knee, ee finished, nitifying foreman at end of shift, that an accident had occurred. Went to ER the next morning.
A ROOF FALL WAS FOUND AT 2:00 AM ON 9/28/04 AT X-CUT 165 ENTRY #0 MAIN SOUTH. FALL WAS IN A 4-WAY INTERSECTION THAT WAS MINED MID-DECEMBER 1996, AND HAD BEEN SUPPORTED WITH 8' FULL RESIN BOLTS. FALL WAS 20' WIDE X 20' LONG X 10' HIGH. NO INJURY OR PROPERTY DAMAGED DUE TO FALL.
EMPLOYEE HAD CUT A SMALL PIECE OF STEEL & WAS USING A STATIONARY GRINDER TO CLEAN THE EDGES. A SMALL CHIP FLEW INTO HIS EYE & BECAME EMBEDDED. EE HAD ON SAFETY GLASSES & HAD THE SHIELD ON THE GRINDER DOWN. EE SHOULD HAVE WORK GOGGLES OVER HIS GLASSES. GOGGLES WERE AVAILABLE AT THE GRINDER.
EE INJURED HIS BACK WHILE HE WAS ASSISTING COWORKER IN HANGING THE CONTINUOUS MINER CABLE FROM THE MINE ROOF. EE CONTINUED TO WORK UNTIL 11/8/04 AT WHICH TIME HE WAS TOLD THAT SURGERY WOULD BE REQUIRED.
While performing routine inspections of fire extinguishers the technician pulled the nozzle away from the tank and came in contact with the retaining hook on the unit. the hook was very sharp on the end and cut the technician's left ring finger, leaving a laceration that required sutures to close. Employee was not wearing gloves at the time. Employee was released to unrestricted
A roof fall was found by examiner doing weekly at at x-cut 258 entry 6 south mains on 9/21/2004. Fall was in a four way intersection that was mined in September 1997 and supported with 8' full resin bolts. No injury or property damage resulted. Travel way will be rerouted.
EE was bent over cutting a piece of metal when he strained his lower back. EE missed 5 days and was on restricted duty until 11-24-04.
While roof bolting the technician had placed his 54 inch drill steel next to the drill mast instead of in the tray to install a 5 ft bolt. steel caught under canopy stop block and sprang out striking the operator int he face leaving a laceration in his nose that required sutures to close. employee was released to unrestricted duty.
While changing a pin on the mast tilt cylinder of a roof bolter the mechanic's finger was pinched between the drill mast and puller bolt leaving a laceration that required sutures. Employee was released to unrestricted duty.
The employee attempted to cut a wire band on a bundle of wire roof mesh to separate the smaller bundles. The mesh sprung and then recoiled back into the employee, pinning him between bundles of mesh.
Swivel socket broke while removing a bolt and cut mechanic's left hand.
USING AIRGUN WHILE WORKING ON FRONT END LOADER AND DEBRIS WAS BLOWN IN FACE.
AN UNINTENTIONAL ROOF FALL OCCURRED AT 39 BREAK IN ONE OF THE INTAKE ENTRIES ADJACENT TO #15 BELT 2 X-CUTS EAST OF SPAD #2990. FALL MEASURED APPROX. 60' LONG X 20' WIDE X 0'-6' HIGH.
While dismounting dozer, he slipped on bottom step and twisted his back. He hurt his back at home and missed several months of work and this is a recurrence of his accident at home.
EE was operating Fletcher roof bolter when a rock fell from the roof approx 3' by 4' by 1.5' striking him in the upper leg causing his knee to hyperextend.
The injured employee was repairing on the unit 2 roof bolting machine. he had removed the rear main control panel and as he was possitiong the cover during reinstallation he felt a sharp pain in his low back. This is incident has been under investigation awaiting information from the dr. ee should always use proper body position and ask for help when handling heavy objects.
EMPLOYEE WAS REACHING ACROSS CAB OF WATER TRUCK TO SHUT THE RIGHT SIDE DOOR AND FELT SOMETHING POP IN HIS LOWER BACK.
WHILE USING A UTILITY KNIFE TO OPEN PACKAGES IN THE WAREHOUSE THE EMPLOYEE WAS CUTTING TOWARDS HIMSELF & SLIPPED WITH THE KNIFE CUTTING HIMSELF IN THE ABDOMEN LEAVING A LACERATION THAT REQUIRED (5)STAPLES TO CLOSE. EE WAS RELEASED TO FULL DUTY (NO RESTRICTIONS).
EMPLOYEE WAS LOWERING THE DRILL STATION CANOPY AND HE LOWERED IT TOO FAR AND CAUGHT HIS LEFT HAND, LACERATING HIS HAND.
EMPLOYEE WAS INSTALLING A FREQUENCY DRIVE IN AN EMPTY SPOT IN AN ELECTRICAL CABINET. AS HE WAS TIGHTENING THE BOLTS, THE WRENCH GOT INTO A BUSS BAR AND CREATED AN ELECTRICAL ARC CAUSING BURNS ON BOTH HANDS.
WHILE PULLING OUT OF #18 HOLE (WEST PIT, X-SEAM), EMPLOYEE WAS PULLING THE CAR PIN. THE PIN BEING STUCK OR IN A BIND, THE EMPLOYEE MOTIONED FOR THE LOADER OPERATOR TO BUMP THE CAR. THE EMPLOYEE'S LEFT HAND WAS ON THE PIN WHILE LOADER MAN BUMPED THE CAR CAUSING HIS RIGHT THUMB TO BE MASHED BETWEEN PIN HANDLE AND THE CAR FRAME.
AN UNINTENTIONAL ROOF FALL OCCURRED AT #52 BREAK, ADJACENT TO #11 BELT, IN THE TRACK ENTRY. FALL MEASURED APPROXIMATELY 35' X 20' X 8'.
Ee was operating diesel personnel carrier traveling outby to the "A""B" power center located outby the working section. As he reached location of "A""B" power box he stepped out of personnel carrier & proceeded to power center. As he was walking he stepped on a loose piece of coal causing him to twist his left ankle & left knee.
Ee was roof bolting in last open crosscut between entry 1 & entry 2 on unit 2. As he was inserting roof bolt a piece of roof rock fell from between the roof bolts, striking him on rt forearm. The incident caused a laceration to his rt forearm that required sutures. He started missing time on Aug 13 & is still under dr's care at this time.
EMPLOYEE STATED THAT WHILE WALKING DOWN CATWALK ON BIN BELT HE LOST HIS FOOTING, FELL AND CAUGHT HIMSELF, INJURING HIS RIGHT SHOULDER. NOTE: HE STARTED MISSING WORK ON 8/20/04.
A ROOF FALL WAS FOUND AT 10:30 PM ON 8/8/2004. FALL WAS IN A FOUR-WAY INTERSECTION OF ENTRY 4 AT SURVEY STATION 33+90 5RMS (MMU 006). AREA WAS MINED 4/13/2004 AND HAD BEEN SUPPORTED WITH 4' FULL RESIN BOLTS. INCONSISTANT ROCK THICKNESS CONTRIBUTED TO ROOF FAILURE. NO INJURY OR PROPERTY DAMAGE RESULTED. FALL WAS ADJACENT TO TRAVELWAY. NO CLEANUP IS PLANNED.
AFTER COMPLETING BOLTING OPERATIONS, EE WAS FOLDING UP THE MACHINE WHEN A 56" DRILL STEEL CAUGHT BEHIND A SUPPORT POST UNDER THE GLUE STORAGE AREA AND FLIPPED TOWARD THE EE, STRIKING HIM IN THE CHIN. LACERATION ON CHIN REQUIRED SUTURES TO CLOSE. EE WAS RELEASED TO FULL DUTY. CURRENTLY EVALUATING THE SET UP OF ALL GLUE STORAGE AND DRILL STEEL TRAYS.
EE was roof bolting on Unit 3. While on the process of drilling a hole a bolt came out of the pod jack allowing the jack to slide to the side of the pod. The ee reached in to move the jack free from the pod and the pod slid back and caught the ee's left index finger. Incident caused a fracture to the ee's left index finger.
Employee received award of workers' compensation for hearing loss
Employee was working from a ladder, helping take bolts off the bearing cap from the dragline drum drive. As he was using a knocker wrench to help remove the bolts, it struck him on the little finger as it inadvertently fell to the floor.
Employee stated that while moving miner cable against rib a piece of rock fell striking him in head making a small laceration behind rt. ear. Received 3 stitches.
Mechanic was installing a metal cover on the continuous miner when he mashed his index finger on his right hand. The finger had a laceration that required stitches to close.
While cutting a pipe, the piece he cut rolled over and struckhis foot.
EE WAS BACKFILLING A DRILL BLAST HOLE WHEN HE HOOKED A ROCK HIDDEN IN THE DRILL CHIPPINGS. HE FELT A PAIN IN HIS LOWER BACK THAT INCREASED LATER IN THE DAY. HE FINISHED THE SHIFT AND WENT TO THE HOSPITAL ON THE WAY HOME. HE WORKED HIS REGULAR SCHEDULE/REGULAR JOB THE 29TH AND 30TH AND TOOK ANNUAL RE-TRAINING ON THE 31ST. HE STARTED MISSING WORK 8/2/04.
A ROOF FALL WAS FOUND IN A 4-WAY INTERSECTION OF A SET OF WORKED-OUT RETURN ROOMS BY EXAMINER DOING THE WEEKLY AIR COURSE INSPECTION. THE LOCATION WAS ROOM 3 SURVEY STATION 2+40, 5RMS. FALL WAS 20' X 20' X 10' HIGH. AREA WAS MINED 1/15/04 AND HAD BEEN SUPPORTED WITH 6' AND 8' FULL-RESIN BOLTS. TRAVEL ROUTE WAS RELOCATED. NO PROPERTY DAMAGE OR INJURY RESULTED.
Employee states that he was in process of unloading shovel bucket pins from the back end of his service truck. As he was rolling or scooting the pin from his truck, after it landed on the bumper, it either bounced or flipped and struck his right hand. Three of his fingers were mashed with the index finger being fractured.
A ROOF FALL WAS FOUND IN ENTRY #2 SURVEY STATION 39+90 5RMS UNIT. FALL WAS 50' LONG X 20' WIDE X 10' HIGH. THE FALL OCCURRED IN 4-WAY INTERSECTION THAT HAD BEEN MINED MAY 15, 2004. AREA WAS SUPPORTED WITH 6 AND 8 FOOT FULL-RESIN BOLTS. NO INJURY OR PROPERTY DAMAGE RESULTED.
