Employee was getting into the Fletcher 7074 & was in the process of moving his SCSR out of the way past the magnet door sensor. The door closed on his finger at the same time he tried to pull his finger out of the way. Resulted into left ring finger tip amputation.

Employee was positioned in the LW panline conducting chain tension checks on the face conveyor chain. He bent over to pick up the chain tension ram which weighs an estimated 35 lbs. As he raised it up about 1' off the ground he felt a twinge in the right side of his back.
Employee was putting his chock blocks down on the scoop when he felt a pop in his lower right abdomen. Reliance Medical Group diagnosed him with abdominal stomach strain and released him to restricted duty.
Employee was walking and felt a pop in his foot. Diagnosed by Reliance Medical, right 5th metatarsal fracture.
During the process of picking pipe joint from rig to place in single pipe loader, employee picked up joint of pipe, suspended it in vertical position, swung pipe into pipe loader and in process of lowering, placed left hand too close to threads pulling his left thumb into loader with pipe.
Employee's statement: Moving power Cable to Sump Pump#2 caught meter(gas) with cable, pulled on meter which then snapped back upon release striking me in my face, hitting 2 front teeth on upper row. One tooth pushed out of position and one tooth chipped.
An employee at the plant was removing a drive belt from a motor for the feeder pan pulley and pinched right ring finger between the pulley and drive belt. Causing fracture to right ring finger.
Employee was injured while breaking the coupler on a charged nitrogen line. Employee sustained abrasion, lacerations to the face and he received a high pressure injection injury to his left ring finger.
IE and another EE when putting a spreader bar into a steal set. They had the bar "or pipe" between the 2 legs, & when other EE pushed the inby leg out by to put it in place IE's finger was caught between the pipe spreader & the I-beam leg.
Roof operator was drilling a hole for a 8' truss bolt with a 7' drill rod & a 3' extension drill rod. After drilling the hole the 7' drill rod hung up in the screen he was drilling through. Operator reached for the extension & the 7' drill rod came out of the roof smashing his left thumb between the 2 drill steel and resulted into a fracture thumb.
Employee was working on unplugging a slick line with concrete in it for pumping a seal in the longwall. When he unhooked a coupler on the line the concrete sprayed out and some of it got in his eye.
Employee advertently placed his thumb in the hinge point of the door (unit 3571) and his supervisor closed the door on his thumb, resulted into left thumb fracture.
WHILE PUTTING THE SURGE TANK BACK IN PLACE, THE TANK DROPPED. EMPLOYEE TOOK THE FULL WEIGHT OF THE TANK AND FELT A SHARP PAIN ON THE MIDDLE PART OF BACK. WHILE USING THE SHOP FLOOR SWEEPER, THE REAR WHEEL HIT A HOLE IN THE FLOOR. THE JOLT FROM THE UNIT CAUSE PAIN TO FLARE UP.
Employee was unloading a spray boom for the shearer from the back of a flatbed truck. The boom slid off toward the employee and as he tried to get away he tripped and fell. The boom struck him on the foot and leg. X-rays were negative however the employee was held out of service for 2 days.
Employee smashed his right ring finger while handling some belt structure. Resulted into a fractured finger and released to restricted duty.
Employee was helping hang a tube on the bolter and a chuck of coal came out of the tube, striking him in the lip, resulted into a laceration. Went to Reliance and received sutures.
During extraction of emp (w/personal Medical condition) EE injured his hand. Following the extraction he knew it hurt but was focused elsewhere until it worsened on 3-15-14. Went to Med. Ctr on 3-16-14 & xrays were neg. but hand was immobilized & dr recommended not using it for work 3-17 to 3-21-14. 3-17-14 was when he first reported it to supv & 3-18 was released to full duty.
Employee was removing belt structure and placing on skid. While loading a spreader bar the left end dropped and caused the right side to connect with a roller on the skid. The employee's right hand was pinched between the spreader and the roller causing his middle finger on his right hand to be pinched just above the distal joint.
Employee was moving the empty DSI box from the bolter tray after filling it with trash. When he let go of the box into the tray on the trailer, a sharp piece of metal on the box caught his glove and pulled his hand with it smashing it between the box an trailer, causing distal fracture left thumb.
Employee was lowering a dozer belly pan, which stopped an inch from the ground. Employee reached under and grabbed the pan to jarred it loose. The pan loosen and fell an inch. Employee pinky and ring fingers were smashed, to which it required stitches to close the wounds.
(Employee claimed the injury took place on October 2, 2013, but didn't report it until July 14, 2014). Employee claimed he was opening trapdoor to access crane deck. While opening the door made of deck plate, it came down on his head, bending head over to left shoulder.
On 9/26/13 employee reported a lower back stain, he was checked out by a First Responder, who said he was ok, employee didn't want to be checked out at hospital and was released to return to work duties. Employee reported to hospital on 2/28/14 complaining of lower back pain (associated to 9/26/13 incident) he was checked out and prescribed medication and to RTW in 2 days (3/3/14)
Employee smashed left thumb with a drill steel during the bolting cycle. Lost his balance and drill steel fell out making contact with thumb.
Employee was asked to hang 30' feeder cable on the rib mesh. This is usually a two person job and employee did this alone. After lunch he proceeded to throw dust in the LOXC and pain worsen.
Employee states he heard his right shoulder pop and felt pain as he lifted platen on a press. Medical treatment was obtained 8/29/13 and employee was released to full duty. This office was just notified that employee was placed on modified duty on 9/11/13 per orthopedic consult. There is no lost time associated with this injury.
On 8-23-2013 employee reported he carried a 5 gallon bucket of oil to the top floor of the sampling building and his left knee started hurting. Did not report until 8-27-13 he was off as scheduled 8/24 & 8/25. Worked on 8-26-13 and did not report pain or injury.
While draining water separator on LW monorail it came loose and struck employee on the little finger (5th digit) of his left hand, causing a laceration, dislocation and fracture.
Employee was moving fencing material, and pulled a muscle in groin. Employee did not report the issue for several weeks following accident. Diagnosis identified a muscle strain in the abdomen/groin area.

Not securing the pin while trying to remove it from a load block for repairs. The pin fell to the ground smashing the employees finger.

While employee was repositioning the antenna on GBV trailer (PUD0767), he smashed his finger between the mounting beam & 3way adapter, caused left 5th finger laceration. received sutures and released to full duty by Reliance Medical Group.
Employee lifted a piece of sheeting while assisting in securing a load to a fork lift when they noticed a pain in their lower back.
Employee was moving tertiary crusher bin on to bin 2. Switch bar loosened and fell onto employees right shoulder. No immediate injury observed. at 1:20am employee noticed pain while using wheel barrel. Escorted to hospital where diagnosed with contusion on right shoulder. Employee on restricted work.
Individual completing pre-op inspection was mounting a forklift. While maintaining three points of contact and stepping up into the cab, the employee felt a pop in the right elbow. Resulting injury initially diagnosed as tear in right distal biceps tendon.
Installing tooth on LD995 corner adapter, tooth slipped off and dropped approximately 4", injuring left middle and ring finger. Diagnosed with small fracture to fingertip, one fingernail removed by doctor. Fingers were bandaged and employee was released on light duty.
Employee was in torque Converter compartment removing 16GPM pump from T/C went to get down swung leg over fender well. Felt a pop and sharp pain in lower back. Reliance Medical Group released him to restricted duty.
Employee closed his finger in his truck door in the parking lot, he had not started his shift. he reported the incident to his supervisor & drove himself to the ER @ San Juan Regional. He declined to have someone drive him. Doctor noted:12-25-12 to rtn to wk in 5 days & no heavy lifting until condition improves. 12-31-2012 released to restricted duty by Reliance Medical Group.
EMPLOYEE WAS AT MINE REPROGRAMMING RADIOS. HE PUT HIS WIRE CUTTERS IN FRONT JACK POCKET AFTER CUTTING TIE TO REMOVE RADIO. HE TRIED TO CLEAN OFF SOME DIRT ON SLEEVE OF JACKET AS HE DID HE CUT HIS PALM ON THUMB SIDE. HE WAS TAKEN FOR SEVEN STITCHES. HE RETURNED TO WORK THE NEXT DAY WITH NO RESTRICTIONS
while lifting 50# rock dust bags, employee strained mid-back. release to no lifting restriction.
Employee's finger was caught in a pinch point while verifying that the belt pulley was not binding
Employee was dumping trash using a self dumping steel forklift hopper, released lock lever and the hopper bounced back smash his finger.

while removing a dust mask from his face a strap from the mask snapped and coming back and striking ee in the eye. this caused irritation and redness to the effected area
Employee was hanging tubes in 400 Sec in LOX. While pulling strap, the strap broke and the hook hit employee's tooth, causing it to fracture.
