Showing all 38EE tried to start the pump located in the rear compartment on the passenger side of the fuel truck and hit the back of EE's hand on a bolt on the mud flap located beside EE cutting hand. The cut EE received from this bolt required 10 stitches in the right hand.
The employee stated that he was working on a motor that was suspended above a walkway. The motor fell, striking the employee's foot and collapsing the walkway.
EMPLOYEE WAS PICKING UP A BAG CONTAINING COAL USING HIS LEFT ARM WHEN HE STRAINED A MUSCLE IN HIS LEFT ARM.
While trying to dislodge the frozen pulling lashing from the loaded barge, something ripped loose and popped in my right shoulder socket.

Roof bolting with Fletcher RR2 twin head bolter, he had 4' resin bolt in hole and it was partially bent. He was pushing on bottom end of roof bolt with his body weight trying to straighten bolt when bolt twisted in hole causing him to fall into the bolt hurting his shoulder area.
Injured was making a splice on a trailing cable and bent over to pick up roll of tape and twisted lower back.

Injured person was pulling shuttle car cable to a new location on section when injured stomach muscle's. Diagnosed as hernia. This person worked light duty until 6/12/07-day of surgery.
Was putting a bucket of mortar over a stopping wall. Being submitted due to Part 50 Audit.
Employee was removing 12.00 x 24 tire from a truck. He tried to lift a tire, he strained his lower back, left side. He worked the rest of the day. On Monday he had severe pain in his back and did not report to work. On Tuesday he came to work but could not punch in. He went to the chiropractor.
Pulled a muscle in his back (left side) while lifting CM cable & water hose.

Employee picked up a 4 ft section of 6" schedule 80 pipe, twisted his back and pulled a muscle in his lower back.

ON 7/25/04 EE WENT TO THE HOSPITAL - PAIN HAD INCREASED TO THE POINT OF UNBEARABLE. X-RAYS WERE TAKEN: SHOWED BULGING DISC IN L-3. FIRST DAY MISSED WAS 7/26/04.
EE WAS WATERING BATTERIES ON #3 SECTION SCOOP. HAD LID WEDGED AGAINST TOP. LID FELL DOWN SMASHING & CUT RIGHT THUMB.
FOUND ROCK ON JABKO CABLE 4 BELT 1 1/2 X-CUTS OUTBY 4160 X-OVER SIZE 4 TO 4 1/2' LONG 2 1/2'WIDE 5"THICK TAPERING TO 1". INJURED LIFTED ROCK TO ROLL OUT OF WAY WHEN IT BROKE ON LIFTING END FAL LING ON VICTIMS'S RIGHT FOOT.
EMPLOYEE WAS HELPING REGULAR SCOOP MAN PLUG UP BATTERIES WHEN HE TURNED AROUND GOT HIS FOOT CAUGHT IN MUD CAUSED HIM TO TWIST AND FALL RESULTINGIN INJURY.
EMPLOYEE WAS KNEELING DOWN MOVING MINER CABLE OVER TO RIB, AND STRAINED A MUSCLE IN UPPER BACK.
EE WAS HELPING BELTMAN TO PUT TOP BELT ROLLER BACK UP ON BED RAILS WHEN HE PINCHED HIS RIGHT MIDDLE FINGER BETWEEN BED RAIL AND BELT ROLLER STRUCTURE.
EE STOOD UP AND HIT HEAD ON PIPE.
EMPLOYEE WAS LOADING BELT STRUCTURE INTO A SUPPLY CAR. WHEN HE TWISTED HIS BACK ABOUT 7:00 PM CONTINUED TO WORK THE REST OF THE SHIFT. EMPLOYEE NEVER SAID ANYTHING ABOUT IT TO HIS FOREMAN AT THE TIME OR AFTER THE SHIFT. IT WAS REPORTED TO ME AT 3:00 PM ON 9-13-01. HE HAS NOT REPORTED SINCE.
EE WAS LIFTING PUTTING BELT STRUCTURE IN AND PULLED MUSCLE IN LOWER BACK.
HE TWISTED TO PICK UP ROOF BOLT TO PUT PLATE ON IT & HURT BACK.
EMPLOYEE WAS CLEANING WET MATERIAL FROM SHOOT, WHEN A ROCK FELL FROM THE FEEDER BIN STRIKING THE EE ON THE RIGHT HAND CAUSING INJURY TO THE RIGHT THUMB AND HAND.
