Employee was removing a 3 by 5 foot piece of steel plate from an old discharge chute. While moving the plate out of the chute, the employee caught EE's right middle finger between the plate and a structural support beam. Employee fractured tip of finger.
Crew was working to remove an old screen. 2 crew members were on a platform below the screen removing old hydraulic lines & storing them on the platform. While trying to roll the hydraulic lines on the platform, one of the connection ends of a line was stuck on a wall. While pulling the line, the connection end came off the wall striking one of the employees in the nose/face.
Employee assisted another employee lifting a drain pan after emptying diesel fuel into pan. When EE lifted pan EE felt a pull in back. Diagnosed with a muscle strain in the back.
Employee was cleaning the tracks of excavator off with a shovel and felt a pop in left shoulder.

Employee was getting up into fuel truck when EE felt a pop in the back of EE's right ankle.

Employee was unloading 50# boxes of explosives from back of truck. Employee claimed "back seized up". Physician diagnosed Lumbar Strain.
Employee was working on a CAT skid steer. He was trying to engage the bucket locking handle. Bucket swung out and struck employee on the right ankle.
Employee was unpacking a lighting ballast out of the box when he lacerated his right index finger on a safety latch that was on ballast. Required 2 stitches
Employee was assisting 3 other employees to lift a set of clothing lockers up 3 steps. During lift he felt a sharp pain in his right bicep area. He was diagnosed with a bicep tendon tear that will require surgery to repair.
Employee was trouble shooting a brake problem on a scraper. Mechanic found a hose that seemed to be leaking air. He loosened the line slightly to try and reseat the line, while doing so the line popped off and hydraulic fluid punctured his right middle finger.
Employee was applying jumper cables to a crane when the clamp slipped off battery post onto his right index finger.
Employee was pulling a wear plate into position to line up holes. The plate snapped up crushing the right hand ring finger between wear plate and bucket.

Employee was climbing off counterweight and strained his right upper arm and shoulder. Employee was wearing fall protection at time of occurence. Employee was diagnosed with strained right arm and placed on restricted work activity.
Employee was doing some maintenance on a piece of equipment. He carried a full 5 gallon bucket to the site, while carrying the bucket he felt discomfort in his lower back. Diagnosed with a lumbar strain and given work restrictions which we will accomodate.
EE WAS TAKING DAMAGED SCREENS OUT OF THE SHAKER. WHILE TAKING SCREENS OUT HE FELT A PULL IN HIS GROIN AREA.
EE WAS WORKING ON CONVEYOR AND HAD TO LIFT THE TAIL WHEEL ONTO WELDING TRUCK. HE HAD ANOTHER HELPING HIM LIFT THE APPROX. 100# TAIL DRUM. WHILE LIFTING HE FELT SOMETHING GIVE IN HIS LOWER BACK . DIAGNOSED WITH A LOWER BACK STRAIN.
EE WAS MOVING A SPEED REDUCER FROM ONE SIDE OF SHOP TO THE OTHER USING THE LIFT & SLIDE METHOD. WHEN HE LIFTED TO SLIDE HE FELT SOMETHING POP IN HIS RIGHT GROIN AREA. HE WAS ALSO COMPLAINING O F PAIN IN HIS LOWER CENTER BACK.
PULLING ON V-BELTS GOT FINGERS BETWEEN V-BELT & SHEAVE CAUSING CUT & FRACTURE TO FINGER.
REMOVING OLD SILT FENCH. PULLING ON SILT FENCE WHEN RIGHT ARM MUSCLE HURT HEN SWELLED UP.
