While lifting a cooler off the tailgate of truck, the employee stepped back and strained EE's knee.

While standing near a loader, the employee jumped back and twisted EE's ankle when the loader moved forward.

Employee lost footing and when they grabbed the rail to steady themselves, they strained their right shoulder.

Accident type, without injuries

While performing maintenance work, an employee was welding a patch on the tub of the shaker when they noticed a fire coming from below the shaker. Sparks from the welding, ignited the conveyor belt.

Over-exertion in pulling or pushing objects

Employee was tightening a bolt on a tail pulley and once EE was done, EE noticed mild pain in right shoulder. Incident was not reported to EE's supervisor until the following week. Diagnosed with right shoulder strain and given restrictions.

Struck by falling object

Employee was in the process of dismantling a gear drive when the steel pivot base frame on which it was setting collapsed onto EE's chest.

Employee was attempting to remove snow when EE slipped and landed on their right hip.

Employee was was working on a final drive (mobile equipment). Employee struck the housing to loosen it. A shard of metal broke off striking employee's right shin.

The employee attempted to break up frozen clumps of sand in a hopper with their hand and while doing so, their hand twisted in an awkward position resulting in a wrist sprain.

Employee was attempting to move a wheelbarrow, when they felt a pull in their back. EE received medical attention at a panel physician the day of the incident and was cleared to return to work with no restrictions but on follow up apt on 7/31/23, EE was put out of work until 8/2/23 when EE had their next follow up apt and was allowed to return to work but with restrictions.

Employee (mechanic) was changing cutting edge on 982M loader. Edge was on the forks of telehandler and employee was prying up edge with a bar to get the right side up. When the edge started to go over employee reached out to grab the edge to keep it from falling and felt a pull in EE's back. Reported immediately, but did not seek treatment until next day 3/21/23.

Employee was doing a plant start up at the wet plant and was shoveling wet material out from manual drop magnet/bucket elevator and felt a pull/sharp pain in lower back.

Struck against a moving object

EE was operating a Caterpillar 992C loader in the South Pit. As they were driving the machine, they reached up with left hand to adjust the airflow coming out of an overhead air conditioning vent. As they reached up, their hand hit the adjacent vent and cut the middle finger resulting in sutures to repair.

Employee inadvertently failed to remove lifting strap from truck crane to excavator bucket. When excavator backed away it raised front wheels of truck with mechanic inside truck. Strap then broke and truck came back down causing employee to strain back.

Employee assisted another employee lifting a drain pan after emptying diesel fuel into pan. When EE lifted pan EE felt a pull in back. Diagnosed with a muscle strain in the back.

Employee was cleaning the tracks of excavator off with a shovel and felt a pop in left shoulder.

Employee was operating a 980G loading a customer truck. While the loader was still in reverse backing away from the truck. Operator depressed the brake / clutch and the loader shifted from reverse to neutral to forward inadvertently. Sudden shift caused pain in lower back.

EMPLOYEE WAS RETRIEVING AN ACETYLENE BOTTLE FROM CAGE WHEN THE BOTTLE SLIPPED AND PINNED EMPLOYEE'S FINGER BETWEEN THE BOTTLE AND CAGE. CONTUSION AND LACERATION TO RIGHT INDEX FINGER.

Employees were welding a patch on a chute and plastic liner caught fire. Employees discharged 1 fire extinguisher and quickly realized they needed to call 911. The fire department responded and extinguished fire.

Employee was working on a CAT skid steer. He was trying to engage the bucket locking handle. Bucket swung out and struck employee on the right ankle.

Employee was unpacking a lighting ballast out of the box when he lacerated his right index finger on a safety latch that was on ballast. Required 2 stitches

Employee was removing a conveyor when he struck his left shoulder on the side of it.

Employee was assisting 3 other employees to lift a set of clothing lockers up 3 steps. During lift he felt a sharp pain in his right bicep area. He was diagnosed with a bicep tendon tear that will require surgery to repair.

Employee was trouble shooting a brake problem on a scraper. Mechanic found a hose that seemed to be leaking air. He loosened the line slightly to try and reseat the line, while doing so the line popped off and hydraulic fluid punctured his right middle finger.

Employee was using a pry bar to unjam a rock from the lump kicker. The pry bar kicked back and struck him in the right side of his face causing a facial fracture.

Employee was hooking up clevises to a lifting hook, which he gained access by climbing up a ladder on the inside. After hooking up clevises he decided to jump off an 8' high tank to the ground. He strained his thigh on his right leg.
