Employee was closing the door behind employee and got finger caught in the door latch.

While driving haul truck, employee felt pain in lower back.

Employee was pulling a piece of steel out with a come along to divert a material when the steel plate tilted and caught employee's left hand between steel and angle iron. Employee fractured left metacarpal shaft in hand.

Employee was removing a 3 by 5 foot piece of steel plate from an old discharge chute. While moving the plate out of the chute, the employee caught EE's right middle finger between the plate and a structural support beam. Employee fractured tip of finger.

Crew was working to remove an old screen. 2 crew members were on a platform below the screen removing old hydraulic lines & storing them on the platform. While trying to roll the hydraulic lines on the platform, one of the connection ends of a line was stuck on a wall. While pulling the line, the connection end came off the wall striking one of the employees in the nose/face.

Employee was unloading 50# boxes of explosives from back of truck. Employee claimed ""back seized up"". Physician diagnosed Lumbar Strain.

Employee was searching for the filter of the air conditioning in his loader when he contacted the moving fan blades in the ceiling. Lacerated right hands middle and ring fingers

Employee was walking on a blacktop surface, when he went to turn his leg turned but his ankle didn't, causing an ankle sprain to his right ankle.

Employee was unjamming the primary crusher, while doing this, he picked up some oil on the soles of his shoes from a leaky hydraulic line. When he went to walk down a set of stairs, he slipped and overturned his left ankle. Diagnosed with a sprained ankle.

Employee was doing some maintenance on a piece of equipment. He carried a full 5 gallon bucket to the site, while carrying the bucket he felt discomfort in his lower back. Diagnosed with a lumbar strain and given work restrictions which we will accomodate.

EE WAS DRIVING A HAUL TRUCK WHEN HE ACCIDENTALLY RAN INTO A SINK HOLE WHICH WAS NOT PRESENT ON HIS LAST PASS THROUGH THAT AREA. HE JARRED HIS BACK AND WAS DIAGNOSED WITH AN UPPER BACK STRAIN AND GIVEN RESTRICTIONS WHICH WE DID ACCOMODATE.

OPERATOR WAS PUTTING ANTI FREEZE IN RADIATOR OF LOADER. WHEN FINISHED HE WAS STEPPING DOWN ON STEP TO EXIT MACHINE. HIS FOOT SLIPPED OFF OF STEP CAUSING HIM TO FALL AND HIT HIS TAIL BONE ON MA CHINE.
