Employee was assisting maintenance personal put away tools when the employee stepped over a gutter and onto a curb. The employee placed the ball of their right foot on the curb and took another step with their other leg. The right heel dropped as they place weight on the leg causing a strained calf muscle.

A Maintenance Technician was tasked to remove a piece of angle iron that was stuck in the belt conveyor tail pulley of BC-101. The EE tried to roll the conveyor belt backwards to let the angle iron drop out of the tail pulley by rolling the motor drive belts at the head pulley. In doing so the EE caught three fingertips on left hand. The Employee was wearing gloves.

Employee was using a vacuum hose attached to a vacuum truck to remove sand near a guarded tail pulley. The hose clogged and the employee was picking up the hose to dislodge the material to unclog it when the hose clog released and the hose fell to the ground, the employee then felt a sharp pain in EE's back.

Employee was disassembling pump. When employee removed last bolt the back plate came loose and pinched a thumb causing a laceration. Employee received six stitches to the left thumb. Employee has work restriction from pulling/Lifting no more than 15 pounds and is expected maximum medical improvement by 2/26/2018

Employee was inserting rods into a rod mill. He removed the strap holding the rods together. The rod rolled off the wood platform crushing his left middle finger. He went to the clinic for sutures.

EE felt his shoulder stiffen up due to the repetitive motions. He reported his pain on 6/11/15 to the company and he decided to apply first aid treatment to his shoulder. On 6/16/15, EE felt pain after the first aid treatment and was escorted to the health clinic around 7:30AM. He was diagnosed with a sprain/strain right shoulder and given restrictive duty.

08/01/2014 Employee was shoveling sand in the blending area when he pulled a muscle in his lower back. 08/04/2014 Employee reported his injury to his supervisor. He was sent in for medical treatment and prescribed medication. He was given lifting restrictions.

EE was shoveling at blender, as he stood up he felt a sharp pain in his lower back. EE stated that pain went away quickly and he finished the remainder of his shift. The next morning EE complained of stiffness in his lower back and was taken to the medical clinic where he received a prescription medication.

Sprained left shoulder while lifting a gas cylinder of oxygen up into a storage box on a mechanic's service truck. The sprain was minor pain at first and the employee did not ask for first aid treatment until 04/01/2013.

The operator was cleaning the area around the short head cone crusher at the rock plant when a piece of channel iron weighing about 20 lbs slid down the handrail along the catwalk and landed on his left foot. Two days later (Dec.8), he reported swelling around the ankle and was taken to an urgent care facility for medical treatment.

At the Rock Plant, a 3" diameter shaft approximately 24" in length was being used to pound on the hopper wall. The operator placed the shaft up on the tail pulley guard. He turned to talk to another operator when the bar fell on his right foot.
