Employee bent down to pick up 100 lb bag of sand. He didn't bend at the knees. When he lifted the bag he felt a pull in his lower back.

Employee was stacking reject bags on a pallet, noticed another conveyor was having problems. Hurried to complete stacking the reject conveyor by lifting, turning & throwing the bags. Thus extending and straining his back muscles.

EE WAS TRYING TO FIX ELEVATOR ON PALETIZER AFTER BEING TOLD NOT TO TOUCH IT SEVERA TIMES BY HIS FOREMAN. HE TOLD ONE OF HIS CO WORKERS TO START THE ELEVATOR WHILE HE TRIED TO REPLACE A BOLT. H IS LEFT HAND GOT CAUGHT BETWEEN THE ELEVATOR ANDTHE IRON BAR.

TWO TRUCK DRIVERS WERE PLAYING TOPS ON THE LOADING PAD AND WERE OUT OF THEIR TRUCKS, DISOBEYING OUR COMPANY REGULATIONS, FAILED TO FOLLOW RULES ON SIGN ENTERING THE COMPANY. FORKLIFT DRIVER WA S PULLING OUT THE PALLET, TRYING TO AVOID RIPPING ANY OTHER PALLETS, HE WAS KEEPING AN EYE IN FRONT OF HIM, THEN TURNED HIS HEAD BACK AT THE SAME TIME HE HIT DRIVER.DROVE OVER TOES,LEG CAUGHT
