Employees were cleaning out a plugged conveyor and when the employees lifted the cover for the conveyor up off of the floor the employee felt a strain in his back. Notified of restrictions on April 19th, 2012.

Employee was removing chunks from G289 airlock with a rod. While employee was unplugging the G289 airlock; dust blew out of the pipe and into employees eyes.
While employee was placing a bulk bag on the hydrate bagger, he overextended his reach which caused a muscle stain in his lower back.
Employee was helping take the coal mill apart during a shutdown. While taking it apart, the bottom gear housing became wedged. The employee tried to unwedge the gear housing and strained his lower left back.

Employee was "pulling" ash with ash puller. When he lifted the ash puller, he felt a strain in his left elbow. He sought medical attention on 7-24-06; his left arm is in a sling; restricted to lifting 20#, cannot climb, occasional twisting motion allowed and seldom can squat or push/pull.
Employee was fabricating a roof for the waste oil tank containment. While using the wire wheel, a piece of metal went into his left eye.
Employee was using a shovel to clean under the C32 elevator. He felt something "give" in his lower back while shoveling. (shift started previous night)
Employee was cleaning hot lime out of the #2 kiln basement. He put the hot lime into a barrel to haul out of the basement.
Employee was sweeping the floors in kiln #2 basement. As he was sweeping, dust got into his eye. He was wearing safety glasses at time of accident. He did flush eyes out immediately, but as the night progressed, his right eye became more irritated.
Employee was helping to install a sump pump near the hydrate plant; em'ee moved a pallet and slipped in water. The left boot filled with water, which reacted with lime and caused a burn to the top/left side of left foot.
Employee was setting up the bulk bagger on a bin with a co-worker who was above lowering a power cord to be plugged in. The cord hit the employee's hard hat; a chunk of dust slid off his had and into his right eye. He flushed out his eye but it was still irritated so he sought medical attention.
Employee was helping with the slurry pump maintenance in the hydrate plant. Employee felt irritation in his left eye so he flushed it out at the plant. At the end of the day it was still bothering him so he went to the emergency room.
EE was cleaning out lime build up under a tail pulley so he could unload railcars. EE was kneeling in water and wet lime for approximately two hours. He mentioned to his supervisor that his knees were sore. On 2/11 he told his supervisor he was going to the doctor for sore and raw knees.
Employee was lifting an overhead hoist onto a beam. The weight of the hoist caused a shooting pain in his lower left and upper shoulder area of his back.
Employee was emptying the pebble lime bin; he opened the actuator valve and lime came spraying out. Lime entered his right eye. He rinsed out his eye for 15 minutes at the plant and eye drops were applied also. He did seek medical attention because there were perticles still in.
While ee & coworker were lifting a 200#, 20' long ash rake up to the rotary floor, coworker's back went out causing him to release his grip on rake. This caused the ash rake handle to strike ee's hand: This action caused his middle finger to be pinned between handle & window ledge. He applied 1st aid @ the plant but later was seen at St Agnes ER. His middle rt finger is broken.
Ee & coworker were lifting a 200#, 20' long ash rake up west side of rotary. As it was being placed on rotary floor, ee felt his back go out. This caused him to release his grip on ash rake. He sat down: when he tried to stand up, he fainted, fell & hit his nose & forehead on coal mill exhauster fan base. He cut his nose & bruised his forehead. He was taken by ambulance
EE WAS MIXING REFRACTORY USING A SHOVEL AND HOE. WHILE MIXING SHE FELT A "SNAP" IN HER RIGHT FOREARM. EE DID SEEK MEDICAL ATTENTION ON 5-11-04.
When he was unloading lime, the line became blocked. While dislodging the blockage, he opened the valve hatch without totally depressurizing the tank causing lime to blow back into his face. Lime dust entered both eyes and onto his forehead. His eyes were flushed at the plant but were still irritated. He sought medical attention; received a rx for vicidon and anitbiotic.
Ee was bagging pure cal lime. The bag broke & lime flew onto ee's face. Lime dust got under safety glasses & into his right eye. Eye was flushed out at plant site but eye was still irritated. Ee did seek medical attention. His eye was flushed out again & he was given a prescription for medicated eye drops.
WHILE ATTEMPTING TO INSERT SHAFT UNDERNEATH THE HYDRATE MIXER BOX, LIME DUST FELL INTO EE'S RIGHT EYE. EE DID RINSE HIS EYE WITH WATER AND AN EYE SOLUTION, BUT COULD NOT REMOVE DUST PARTICLES. HE DID SEEK MEDICAL ATTENTION. EE ALSO RECEIVED A PRESCRIPTION FOR ACULAR MEDICATED EYE DROPS.
EE WAS IN THE PROCESS OF ATTACHING THE BOOM TO THE LOADER BUCKET. HIS GLOVED LEFT HAND, LITTLE FINGER BECAME CAUGHT BETWEEN THE BOOM & THE BUCKET. THE LITTLE FINGER HAS A LACERATION. EE DID SEEK MEDICAL ATTN.
EE WAS CLEANING OUT BOTTOM OF ELEVATOR. THE ELEVATOR DOOR CAME LOOSE, SWUNG DOWN AND HIT EE ON HIS HEAD.
EMPLOYEE WAS USING A GARDEN SHOVEL TO LOAD ASH CHUNKS AND STONE INTO A WHEELBARROW. HE FELT PAIN IN THE RIGHT SIDE OF HIS MID/LOWER BACK, WHILE HE WAS LIFTING/ TWISTING.
EE WAS TRYING TO REALEASE GUARD OFF THE MERRICK CHAIN. WHEN HE PULLED ON THE GUARD, THE LIME THAT HAD ACCUMULATED ON THE COVER CAME OFF, CAUSING LIME DUST TO ENTER EE LEFT EYE.HE FLUSHED OUT H IS EYE BUT AFTER A FEW HOURS HIS EYE BECAME MOREIRRITATED. EE DID NOT SEEK MEDICAL ATTENTION.
EMPLOYEE WAS LEANING OVER A RAIL TO PUT BOLTS ON A PLATE. HE HAD A FEELING OF NUMBNESS AND THEN A SHARP PAIN IN HIS ABDOMEN WHEN HE STOOD UPRIGHT.
EE WAS DUMPING A HOLDING TUB FULL OF HOT PEPPLE LIME WITH A FORKLIFT. AS IT STARTED TO DUMP, A CLOUD OF HOT LIME DUST FLEW BACK TOWARD EE. THE HOT LIME DUST WENT ON HIS LEFT FACE AREA, LEFT ANKLE AND BOTH KNEES. EE DID RECEIVE MEDICAL ATTENTION: HE RECEIVED TYLENOL W/CODEINE AND MEDICATED BURN CREAM.
EE WAS LIFTING A 55 GALLON OIL DRUM (FULL) TO AN UPRIGHT POSITION. THE OIL DRUM STARTED TO TIP OVER, SO THE EE PUSHED ON THE DRUM TO SET IT STRAIGHT. AS HE DID THIS, HE NOTICED A TIGHTNESS IN THE LEFT SIDE OF HIS GROIN AREA. EE WENT TO A DOCTOR ON 5-28-03.
EMPLOYEE WAS RE-INSTALLING SIDE GUARDS ON A LIME CONVEYOR (C-157). WHILE DOING THIS, SOME LIME DUST GOT BEHIND EMPLOYEE'S SAFETY GLASSES. EMPLOYEE DID SEEK MEDICAL ATTENTION.
IT IS ALLEGED WHEN EMPLOYEE WAS WEIGHING 70# MORTAR BAGS HE WAS LIFTING THE 70# BAG OF MORTAR AND FELT A SHARP PINCHING PAIN IN HIS LOWER LEFT SIDE BACK AREA. HE HAS PAIN WHEN HE TURNS OR LIFT S HIS LEGS; ESPECIALLY LIFTING HIS LEFT LEG.
IT IS ALLEDGED WHEN EE WAS CLIMBING A LADDER TO FILL A DUST TRUCK, HIS RIGHT KNEE MADE A LOUD POPPING NOISE. APPROX. 1/2 HOUR LATER HIS KNEE WAS SWOLLEN. EE COMMENTED HE FELT LIKE LIQUID WAS G ATHERING AROUND THE KNEE CAP. EE SOUGHT MEDICAL ATTENTION: HAS APPT TO SEE ORTHOPEDIC DR ALSO.
EE WAS OBSERVING ANOTHER EE PERFORMING MAINTENANCE ON THE NORTH BAGGER SPOUT. THE OTHER EE ACTIVATED THE SPOUT WHICH RELEASED MORTAR. THIS MORTAR SPRAYED ON THE BAGGER FRAME & INTO EE'S RIGHT EYE. EE DID SEEK MEDICAL ATTENTION.
EMPLOYEE WAS USING A RAKE TO PULL ASH CHUNKS FROM THE COOLER GRATES. EMPLOYEE FELT PAIN IN LOWER BACK FROM BENDING PULLING ACTION.
EE WAS BAGGING LIME.LIME DUST WENT THROUGH HIS CLOTHING, THE COMBINATION OF LIME DUST & THE HIGH TEMPERATURE CAUSED A CHEMICAL BURN TO HIS LOWER RIGHT BACK
WHILE PULLING 2X8 BOARDS OUT OF FEED END OF #1 KILN, THE BOARD FELL INTO THE HOPPER. THIS CAUSED THE OPPOSITE END OF THE BOARD TO BOUNCE UP. THE BOARD HIT EE IN THE CHEST AND PINNED HIM AGAINS T THE DOOR. EE EXPERIENCED CHEST PAINS AND SORENESS.
WHEN EMPLOYEE WAS EMPTYING THE #1 DUST BIN, HE FELT PAIN IN HIS LOWER MIDDLE BACK WHEN HE PULLED ON THE BIN. HE SOUGHT MEDICAL ATTENTION.
WHILE CLEANING OUT THE BAGGER PIT, LIME GOT INTO EE'S EYE RIGHT.
EE WAS RESTACKING 50# BAGS. HE PICKED ONE OFF THE FLOOR AND FELT PAIN IN HIS LOWER BACK. EE DID SEEK MEDICAL ATTENTION.
EE WAS ATTEMPTING TO STOP A BAG OF MORTAR FROM GETTING CAUGHT IN THE APRON OF THE MORTAR PLANT BAGGER. HE USED HIS RIGHT FOOT TO ROLL THE BAG SO IT WOULD NOT GET CAUGHT. IN DOING SO HIS RIGHT FOOT BECAME CAUGHT BETWEEN THE ANGLE IRON BY THEBAGGER SEAT AND THE BAG OF MORTAR.
EMPLOYEE WAS CLEANING OUT THE SILO PRODUCT BINS, WHEN LIME FELL ON THE BACK OF HIS NECK. EE DID SEEK MEDICAL ATTENTION. HIS SHIFT STARTED ON 8/15/01 AT 6:30 PM UNTIL 6:30 AM ON 8/16/01.
WHILE PULLING HEAD PULLEY ONTO C-114 CONVEYOR, EE FELT PAIN IN HIS LEFT SHOULDER.
EMPLOYEE WAS USING A TWISTING MOTION TO MOVE EMPTY BAGS FROM THE LEFT SIDE OF THE BAGGER TO THE RIGHT SIDE. HE FELT A SHARP PAIN IN HIS LOWER BACK. EMPLOYEE IS SEEKING MEDICAL ATTENTION.
EE WAS CLEANING UP A PILE OF LIME THAT SPILLED IN THE #1 BASEMENT. WHILE LIFTING, HE FELT A SHARP PAIN IN HIS LOWER BACK.
IT IS ALLEGED EE INJURED HIS MIDDLE/SIDE OF HIS BACK WHILE SHOVELING MUD/DIRT IN THE RECLAIM AREA. WHILE SHOVELING, HE STATES HE FELT A SHARP PA9IN IN THE MIDDLE AND SIDE AREA OF HIS BACK. HE IMMEDIATELY QUIT SHOVELING AND WENT TO SEEK MEDICAL ATTENTION. MEDICAL CENTER DR. PRESCRIBED NAPROFYN FOR INFLAMMATION AND VICODIN FOR PAIN.
