Employee was closing the lids on top of a railcar when they tripped and fell forward. The employee was wearing their fall arrest and using a retractable lanyard. When they fell their arm was caught by the lanyard, resulting in a shoulder separation. Employee had surgery on 11/14/2023. Worked until then.

Employee felt a sharp pain in their right shoulder while pulling ash. Employee continued to work in full capacity until which time surgery was required on shoulder. Surgery was performed on 3/15/2023 to their shoulder.

Employee fell down a flight of 10 metal stairs in the Hydrating building. Employee failed to firmly plant foot on first step and failed to maintain 3 points of contact. Employee suffered minor bruising, lacerations and friction burns to their knees, shins, left cheek, left elbow, left shoulder, neck and right hand.

Employee went to use hand sanitizer and while pushing down on the nozzle the hand sanitizer was forced upward and into the employee's right eye. Employee noted a stinging in their eye. On 12/28/2020 the employee went to an optometrist and was prescribed an antibiotic eye drop.

Per the attached, Graymont maintains that this accident is not MSHA jurisdiction, is not chargeable to Graymont or mining industry, continues to be investigated by OSHA, was not a Graymont employee, & occurred outside of Graymont's privately held mine site.

Employee slipped on ice while exiting vehicle in parking lot. The employee landed on their elbow. Initially, employee did not report an injury. On 3/22/2019, employee reported to work with note stating they could not return to work for 2 weeks. On 4/15/2019 the injury was determined to be work related.
In the process of building a set of steel saw horses and an approximate 4' piece of 4X4 tube steel was standing vertical on the bench when it tipped and struck employee's left thumb between the steel tabletop and the piece the employee was handling.

Employee was on a back side of a cooler repairing steel, grinding, cutting and welding in dusty conditions. He was fine when he left work but the next morning his eye was irritated and felt like something was in it.

Employee opened lid of hydrate plant pugmill to inspect discharge end. When he closed the lid hydrated lime dust blew into his right eye. He was wearing safety glasses at the time. He immediately flushed the affected eye at the plant and was brought to the emergency room for additional flushing. He did not miss any work over the incident and returned to his normal work duties.

Employee was using a front end loader to transfer stone from the stockpile to the reclaim system. The stockpile was frozen and snow covered in several areas. The employee undercut the stockpile attempting to reach thawed and thus free flowing stone. The stockpile overhead sloughed off and engulfed the loader into the cab.
Employee was pulling ash with ash puller and roller rest from the grates using the front pulling door. Employee struck his right hand against a burner pipe joint. The right little finger (pinky) is bruised and sore. Employee did seek medical attention on 1/30/07.

Employee was ""pulling"" ash with ash puller. When he lifted the ash puller, he felt a strain in his left elbow. He sought medical attention on 7-24-06; his left arm is in a sling; restricted to lifting 20#, cannot climb, occasional twisting motion allowed and seldom can squat or push/pull.
Employee was using a metal rod to clean the O2 analyzer. The system pulsed which caused hot air and dust to go onto his face. Lime and hot air combined to irritate both eyes; causing them to become red, puffy and water.

Ee was attempting to dislodge a chunk of lime from the bottom of a baghouse. The hot dust that was hung up poured out onto his left boot. The extreme temperature of the dust caused the boot leather to become burning hot. The heated leather caused burns to his left foot. Ee did seek medical attention on 10-16-04.
