Struck by flying object

Employee opening Kiln 2 firing hood door to rake ash. Upon releasing handle, dust entered behind safety glasses and into right eye. Employee rinsed at eyewash station, later sought clinic care and was prescribed eye ointment. Required additional PPE for task was not worn. Employee returned same day, no restrictions; retraining conducted 2/10/26.

Employee got some dust in their eye while standing outside. Was not performing any work at the time. Had all proper PPE on.

Employee was troubleshooting the MODCO Pod in product handling area when an air pulse blew lime dust into left eye. EE rinsed EE's eyes multiple times during the day and after shift change sought medical attention where EE was prescribed eye drops.

Miner conducting a work area inspection. Miner looked up at approaching pickup headlights and slipped on a patch of ice. X-ray taken 12/12/2024 no fractures and released to full duty. MRI referral was made on January 1/24/2025. 1/27/2025 rotator cuff tear was diagnosed. On 2/3/2025 referred to Orthopedic specialist for surgery.

Product loader was on back of trailer when trucker started moving forward. Steps hooked on truck hatch pulling steps under catwalk and employees legs became entangled. Broken right ankle, broken big toe, and lost index toe on right foot.

Employee opened the kiln firing door to check the grates. This caused some dust to stir up and it got in employees eye. Employee flushed eye at work but later went to the clinic. Employee was given prescription eye cream.

Employee was stepping off of forklift, felt slight twinge in right knee. Worked for several hours while knee was sore. After taking a break for lunch employee was unable to bear weight on leg. Waiting for doctor diagnosis.

Employee was changing air pads and laid a impact tool down. The tool got dust in the back of it so when the employee went to use the tool, the air blew the dust out of the back of the tool and into the employees eyes. Employee was wearing safety glasses.

EE was lifting rubber belting out of basket. The basket swung and bumped EE in the back of right calf. At the time the EE stated that it didn't hurt and EE hardly felt the bump. The next morning the EE has a large lump in muscle so EE visited doctor and doctor put EE on restrictions. Contusion to muscle. Blood thinner medicine could have caused bruising.

Removing a piece of brick that was wedged in the end gate of the dump truck. End gate shut catching employee finger between latch and bar of end gate. 19 stitches required.

Employee was using a grinder with a 6"" cut-off wheel. Employee had finished the cut, released the trigger on the grinder and was moving to set the grinder down on the ground. The wheel was still spinning as the grinder was shutting down, employee inadvertently touched it against the back of left hand and it cut though the glove and caused a small laceration. 2 stitches required.

While operating a crane a wind storm suddenly popped up. This storm kicked up a bunch of dust in the path of employee. EE was wearing safety glasses and other personal protective equipment. During the wind storm some dust got in the left eye.

Employee was poking hydrated lime out of the ball mill and stuck his finger on a bolt. Chip in bone and finger splinted.

Employee was moving railcars and the trackmobile jumped the tracks causing the employee to be thrown around in the cab of the trackmobile. Employee experienced pain that did not get better. Had MRI on 10-30-14 that revealed a Lumbar compression fracture.

Employee was removing wet dust from a dribble chute. When employee went to push the flap gate shut the employees his left thumb was caught in between the pipe and the flap gate. X-ray revealed a fracture.

Employee was helping co-worker remove a metal seal from a 55 gallon drum. When employee pried the metal seal apart the seal popped up and cut his finger. Employee received a laceration to his left pinky finger that required 3 stitches.

The employee was preparing to drill holes in the Kiln 1 cooler pipes. Employee went to get additional PPE and when he was getting back into the cooler, employee realized he had something in his right eye. Employee doesn't know exactly how particle got in his eye. Right eye abrasion.

The lime transferring system shutdown, plant personnel tried to reroute the lime. The hot lime sitting on the conveyor caused the belt to start on fire. Plant personnel tried to extinquish fire and call the fire department at 18:06. Plant management was contacted at 18:53, came to the plant to assess the situation and contact MSHA at approximately 19:00. Fire was out around 21:00.

Employee was carrying a portable propane tank when he slipped and fell. Employee has a cut on his right index finger and broke his right leg.

A small fire had started on the night shift but was quickly extinguished with a CO2 fire extinquisher. The burnt gear lubricant reignited and could not be extinquished immediately.

Employee was using a wheelbarrow to clean up in the Modeco building. When the employee went to dump the wheelbarrow full of lime into the skidsteer, he slipped and fell. The wheelbarrow tipped over and spilled lime onto him. Employee had lime burn to right eye.

Employee was helping take the coal mill apart during a shutdown. While taking it apart, the bottom gear housing became wedged. The employee tried to unwedge the gear housing and strained his lower left back.

Employee was emptying the lime out of the bucket of a small front-end loader. While the employee was driving the small front-end lader, lime dust blew back into hif ace. He suffered a lime burn to his right eye.

While accompanied by experienced miner, the injured employee was shutting off the water control valve to shutdown the hydrator process. A pipe thread failure on the 3 way valve released lime slurry onto upper body and face.

Miner was installing new roofing. Miner was adjusting the roof sheet when a piece of metal from drilling entered right eye.

Employee was attempting to clear a bagger spout with a metal rod. While checking to see if spout was open, hydrated lime shot out of spout. The lime deflected off the cross member of the seat causing some lime to get into employee's left eye. Employee flushed eye out but after 45 minutes it still was bothering him. Employee was walking towards the stone screener. While walking, he stepped on a stone and twisted his left knee. ** Employee began missing 2/3/2006 for surgery. He is back but still on restricted duty.**

Employee was sweeping the floors in kiln #2 basement. As he was sweeping, dust got into his eye. He was wearing safety glasses at time of accident. He did flush eyes out immediately, but as the night progressed, his right eye became more irritated.

EE WAS USING A UTILITY KNIFE TO CUT THROUGH AN AIR HOSE ON A RAILCAR. HE WAS PULLING THE KNIFE TOWARDS HIM; THE KNIFE SLIPPED OFF THE HOSE & STRUCK HIS LEFT INDEX FINGER. EE WAS WEARING GLOVES BUT THE KNIFE CUT THROUGH THE GLOVE & CUT HIS FINGER. EE DID SEEK MEDICAL ATTENTION; ""SUPER GLUE"" WAS USED TO SEAL THE WOUND.

Employee was setting up the bulk bagger on a bin with a co-worker who was above lowering a power cord to be plugged in. The cord hit the employee's hard hat; a chunk of dust slid off his had and into his right eye. He flushed out his eye but it was still irritated so he sought medical attention.

Employee was helping with the slurry pump maintenance in the hydrate plant. Employee felt irritation in his left eye so he flushed it out at the plant. At the end of the day it was still bothering him so he went to the emergency room.

EE was cleaning out lime build up under a tail pulley so he could unload railcars. EE was kneeling in water and wet lime for approximately two hours. He mentioned to his supervisor that his knees were sore. On 2/11 he told his supervisor he was going to the doctor for sore and raw knees.

EMPLOYEE WAS ATTEMPTING TO REMOVE LIME BUILDUP FROM THE TUBE MILL WITH AN AIR CHISEL. HE WAS ON HIS STOMACH WHEN THE CHISEL BROKE THRU THE HARDENED LIME. THIS CAUSED A CLOUD OF STEAMY LIME TO GO ONTO HIS FACE AND ENTER BOTH EYES. HE RINSED HIS EYES WITH WATER AND EYE SOLUTION BUT COULD NOT REMOVE ALL THE LIME PARTICLES.

Employee was emptying the pebble lime bin; he opened the actuator valve and lime came spraying out. Lime entered his right eye. He rinsed out his eye for 15 minutes at the plant and eye drops were applied also. He did seek medical attention because there were perticles still in.

While ee & coworker were lifting a 200#, 20' long ash rake up to the rotary floor, coworker's back went out causing him to release his grip on rake. This caused the ash rake handle to strike ee's hand: This action caused his middle finger to be pinned between handle & window ledge. He applied 1st aid @ the plant but later was seen at St Agnes ER. His middle rt finger is broken.

Ee & coworker were lifting a 200#, 20' long ash rake up west side of rotary. As it was being placed on rotary floor, ee felt his back go out. This caused him to release his grip on ash rake. He sat down: when he tried to stand up, he fainted, fell & hit his nose & forehead on coal mill exhauster fan base. He cut his nose & bruised his forehead. He was taken by ambulance

EE WAS REMOVING ASH RING FROM #2 KILN WITH A CHISEL. A PIECE OF ASH WENT INTO HIS LEFT EYE. EE DID RISNSE HIS EYE. AFTER A FEW DAYS, HE NOTICED THE EYE WAS STILL RED AND SORE. ON 5-14-04 HE DECIDED TO GET MEDICAL ATTENTION. HE RECEIVED THREE PRESCRIPTIONS.

When he was unloading lime, the line became blocked. While dislodging the blockage, he opened the valve hatch without totally depressurizing the tank causing lime to blow back into his face. Lime dust entered both eyes and onto his forehead. His eyes were flushed at the plant but were still irritated. He sought medical attention; received a rx for vicidon and anitbiotic.

WHILE DESCENDING THE STAIRS IN THE HYDRAT PLANT. EE S FOOT SLIPPED ON THE BOTTOM EE THEN FELL FORWARD ONTO THE HYDRATE FLOOR. HE DID NOT FEEL ANY PAIN AT THAT TIME. ON 12-9-03 HE DID FELL A DU LL PAIN IN THE MIDDLE OF HIS BACK. THE PAIN IS STILL THERE AS OF TODAY 12-11-03.

EE WAS ATTEMPTING TO BREAK THE MERRICK AIRLOCK WITH THE CHAIN STILL ON. HE WAS USING A PIPE WRENCH, HIS RIGHT HAND SLIPPED OFF THE PIPE WRENCH. THIS ACTION CAUSED HIM TO SMASH HIS RIGHT MIDDLE FINGER. HE RECEIVED A SMALL CUT BELOW THE NAIL AND MASSIVE BRUISING OF THE NAIL.

EE WAS TRYING TO REALEASE GUARD OFF THE MERRICK CHAIN. WHEN HE PULLED ON THE GUARD, THE LIME THAT HAD ACCUMULATED ON THE COVER CAME OFF, CAUSING LIME DUST TO ENTER EE LEFT EYE.HE FLUSHED OUT H IS EYE BUT AFTER A FEW HOURS HIS EYE BECAME MOREIRRITATED. EE DID NOT SEEK MEDICAL ATTENTION.

EE WAS DUMPING A HOLDING TUB FULL OF HOT PEPPLE LIME WITH A FORKLIFT. AS IT STARTED TO DUMP, A CLOUD OF HOT LIME DUST FLEW BACK TOWARD EE. THE HOT LIME DUST WENT ON HIS LEFT FACE AREA, LEFT ANKLE AND BOTH KNEES. EE DID RECEIVE MEDICAL ATTENTION: HE RECEIVED TYLENOL W/CODEINE AND MEDICATED BURN CREAM.

EE WAS DESCENDING FROM A FIXED LADDER. HIS LEFT HAND SLIPPED: EE FELL APPROX. 4 FEET TO A CONCRETE FLOOR. EE THOUGHT HE HEARD A ""CRACK"" AND FELT PAIN IN RIGHT ANKLE. TWO FELLOW EMPLOYEES STABI LIZED ANKLE: ICE PACKS PUT ON AND EE TAKEN TO EMERGENCY ROOM.

EE WAS LIFTING A 55 GALLON OIL DRUM (FULL) TO AN UPRIGHT POSITION. THE OIL DRUM STARTED TO TIP OVER, SO THE EE PUSHED ON THE DRUM TO SET IT STRAIGHT. AS HE DID THIS, HE NOTICED A TIGHTNESS IN THE LEFT SIDE OF HIS GROIN AREA. EE WENT TO A DOCTOR ON 5-28-03.

EE WAS USING A SLEDGE HAMMER TO BREAK UP A PIECE OF KILN ASH. WHEN THE HAMMER FACE CONTACTED WITH THE PIECE OF ASH, THE EE FELT SOMETHING POP IN HIS LEFT WRIST AND FELT PAIN.

HE IMMEDIATELY QUIT SHOVELING AND WENT TO SEEK MEDICAL ATTENTION. MEDICAL CENTER DR. PRESCRIBED NAPROFYN FOR INFLAMMATION AND VICODIN FOR PAIN.
