Job title - Driller. Was placing a drill hammer into the bed of a pick up truck. He pushed the hammer into the bed of the truck. He received a cut on his hand from a sharp edge of the hammer. The injured employee was wearing leather gloves at the time.

Showing all 37While bending down to retrieve parts out of a deep box, employee felt pain in his side.
Removing bend pulley from conveyor. Injured worker grabbed onto lifting strap that was hooked to the pulley that was hooked to crane. Pulley shifted and his left hand was caught between strap and beam. Left ring finger was lacerated, requiring 7 stitches.
EE was replacing teeth and edges on loader bucket in welding shop. The last two edges were flat on the floor and could not be hooked onto overhead hoist. EE attempted to lift an edge up but the other edge slid out and caught his left index and middle fingers. Lacerations on both fingers requiring several stitches on each finger.
While turning over the plate, it slipped, smashing left ring finger between the plate and the welding table. Advised by doctor on 5/2/08 employee was authorized to return to full duty duty, no restrictions, on 4/16/2008
Tugged on case cover when it was hung up and would not lower. Cover went into closed position at the same time employee put hand in pinch point on the cover to maintain his balance. Cut hand which required sutures. Treated and released to regular work with no restrictions.
While unloading a 30 foot length of pipe, the end of the pipe toward the front of the vehicle slipped. The rear of the pipe "whipped" around, striking employee in the jaw and face.
Smashed index finger between the side plate and the other half of the gate. Fracture of right index finger. On modified work. Released to regular work on 8/10/07.
Picked up a 5-gallon bucket of electrical parts. Felt a pain in his groin area. Notified personel at the time. Offered medical care and declined. Reported recurrence of pain over the weekend and requested medical care on 12/4/06. Reported to W/C on 12/4/06.
Pinched left thumb between steel plate and handrail.
EE strained his right elbow when the fire extinguisher did not release from the bracket. Originally considered non-medical event. Recored OSHS and MSHA when medical care required on 5/23/2006.
REMOVING A LATCH DOOR FOR A CHUTE AT THE KRUPP. AS EE WAS SETTING THE LATCH DOOR DOWN, HE PINCHED HIS HAND (LACERATION).

EE CLAIMED INJURED HIS BACK ON FRIDAY WHILE SHOVELING AT THE SAND STOCK PILE. REPORTED INCIDENT TO ANOTHER EE ON MONDAY AM. POSSIBLY WILL BE MANAGED AS 1ST AIE IF NO FURTHER TREATMENT NEEDED AFTER 9/29/05 APPOINTMENT
EE REPORTED VISION PROBLEMS AFTER EXERTING HIMSELF WHILE ASSISTING REMOVAL OF CHUNK OF CLAY FROM BELT. DID NOT SEEK MEDICAL CARE UNTIL AFTERNOON OF 9/7/05 AT END OF SHIFT. PER DR. OK TO RETURN TO REGULAR WORK NEXT SHIFT 9/8/05
Employee was climbing the ladder on the loader, carrying an impact gun, when his lower back started hurting. Reported at the time but did not seek medical care. Reported on 4-25-05, back continuing to hurt & req medical care.
LIFTING THE LARGEST BLADE INTO PLACE, FELT PAIN IN LOWER BACK AT THE WAIST AREA.
Bent at the waist to collect the sample. Employee's back went into spasms.
BEARING HOUSING FELL ON EMPLOYEE'S LEFT FOOT. LEFT 2ND METATARSAL FRACTURE.

Safety vest caught on truck door handle as hubie was exiting the truck. in a ttempting to steady himself. he caught his thumb in the windlash handle and twisted the thumb to the side.
HE FELT PAIN IN HIS LEFT SHOULDER WHILE ATTEMPTING TO CLOSE A RAIL CAR DOOR. HE REPORTED THE INCIDENT BUT DECLINED MEDICAL CARE AT THE TIME. HIS SHOULDER STILL HURT ON 12/5/03 AND HE WANTED ME DICAL CARE. SENT TO MAR MONTE. POSSIBLE TORN ROTATOR CUFF.

BELT ABOVE HIM MOVED AND DUMPED ROCK INTO SCREEN WHERE HE WAS LOCATED. HE TRIED TO MOVE OUT BUT FOOT WAS STUCK AND HE WRENCHED HIS RIGHT KNEE WHILE TRYING TO GET OUT OF THE WAY AND WHEN WORKERS TRIED TO PULL HIM OUT, HAD TO DIG HIM OUT TO GET HIM FREE.
BACK PAIN INCREASED OVER TIME WHILE WORKING AS A MACHINIST
MATERIAL UNKNOWN WHAT CAUSED PAIN IN SHOULDER AND COLLARBONE.
REPETITIVE SHOVELING AS A LABORER AT THE WILSON QUARRY.
CONTACT WITH HOT EXHAUST PIPE. JUMPED ONTO TRUCK AND GRABBED EXHAUST PIPE FOR BALANCE.
STRAINED WRIST WHILE SHOVELING.
MATERIAL PICKED UP HEAVY BAR AND RING FINGER, LEFT HAND, BENT BACKWARD DUE TO EXCESS WEIGHT OF BAR. FINGER WAS EXAMINED AND FOUND TO BE FRACTURED ON MAY 15, 2000.
