As operator approached 5' to 6' pile, his loader was on an incline with bucket full of sand. As he put loader in reverse, a clump of material came off the top of the bucket and shattered the windshield. Loader tipped slightly backward and sand in bucket came into cab, crushing console and steering wheel. Employee sustained cut on right arm and fracture/bone chip in right ankle.

While unloading a 30 foot length of pipe, the end of the pipe toward the front of the vehicle slipped. The rear of the pipe "whipped" around, striking employee in the jaw and face.
