Employee was out of the loader manually moving the face drill cable out of their way so that they could clean up. While they were working the cable out of the way they felt a strain in their abdomen that did not ease up throughout the day. They came out and reported the incident around their quitting time.

Showing 50 of 52When attempting to close tailgate after dumping off, driver grasped tailgate bottom and pulled outward. As they let the tailgate fall to latch, they did not ensure fingers were clear of the pinch point. Their right, little finger was smashed. EE was diagnosed with a open fracture of the distal phalanx of the right, little finger and required 14 stitches to close the wound.
Maintenance crew was working to free up an outrigger pad on the UV 11 blasting truck that had seized to the shaft. The crew was using a strap hooked to the maintenance truck to pull on the seized pad. When the pad broke free, it spun approximately 180 degrees on the shaft and mashed the employee's fingers.
Employee was walking over to help clean the beltline when they slipped on ice and landed on their lunch box.
Employee was trying to help install a rotary airlock on the primary separator at kiln 1 baghouse when the airlock shifted on the forklift and pinched EE's right index finger between the flange of the airlock and the chute it was being attached to.
EE was trying to spread forks on the IT CAT Loader Equipment number 1195 when one fork came off of the rail and fell on EE's right ankle. The forks were in a raised position approximately 4 ft. off of the ground in the wire hole (Underground).
There was no missed work or medical treatment on 7/18/23 when EE claimed EE tweaked shoulder climbing onto loader. EE worked until 9/19/23 when EE exacerbated the alleged pre-existing injury. EE was off from 9/20/23 - 9/27/23. EE came back and worked from 9/28/23 - 12/5/2023. EE has been off recovering from surgery from 12/6/23 - current. This was caught in my internal audit.
Employee was performing work under a platform, caught EE's hard hat on the corner of the platform which caused EE's hard hat to fall off. At that time the employee struck EE's head leaving a gash on the top of EE's head in EE's hairline.
Cone off boom fell off & hit in right shoulder/middle back
Employee was helping replace belt on conveyor at the feed bin and felt a pop in right shoulder
While removing the front pad housing steel from the double boom face drill the housing steel jumped up and struck the employee on the left eyebrow.
While cleaning out a duct pipe, the plugged lime "puffed" up. Heat and lime surrounded the employee and lime went around the glasses into eyes.
Employee was shoveling #1 Sand Plant when EE felt a pain in the lower right part of EE's back.
Employee was throwing an extension cord up to another employee when lanyard got wrapped in the cord and the lanyard swung and struck the employee above right eye.

When the spout at the Hydrate Bagging Machine was plugged the employee went to open it while under pressure and lime released from the spout getting into the employee's eyes.
While cleaning frozen mud off of the gate of the diverter at the secondary screen the employee struck left hand against the door of the diverter.
While filling up hydrate lime bags, the machine malfunctioned and had to be reset. While going to reset the machine, the bag being filled burst causing hydrate lime to get into the employee's right eye causing irritation. The employee was wearing safety glasses at the time of the incident.

Employee was checking the tail pulley on the fines collector conveyor on a portable plant. The employee's hard hat fell off. The employee reacted by raising up and hitting the top of his head on a fixed ladder.
While loading a shot at the face the employee felt pain in his back. Although he felt OK the rest of the shift the next day he sought medical care as he states he could barely move.

HYDRATED LINE WAS STUCK IN THE LIME CHUTE. EE TRIED TO LOOSEN THE LIME BY SHAKING THE CHUTE. THE LIME RELEASED ALL OF ONCE AND SPRAYED EE IN RIGHT EYE. RESULT CHEMICAL EYE BURN.
HURT SHOULDER AND RIGHT ARM WHILE LOADING SHOT.
EMPLOYEE WAS INSTALLING A NEW WING PULLY ON #1 BELT WHEN THE PILLOW BLOCK BEARING ROTATED DURING INSTALLATION AND STRUCK HIS FINGER.
EMPLOYEE WAS CLIMBING DOWN OFF OF BC2 BELT AND HIT HIS LEFT SHIN BONE ON THE BELT SCRAPER SHAFT.

EMPLOYEE WAS HELPING TO CHANGE SCREENS IN THE #2MILL SCREENING TOWER AND GOT CAUGHT BETWEEN SCREEN AND SIDE RAIL. HIS LITTLE FINGER GOT CAUGHT BETWEEN.
EE WAS CHANGING CONVEYOR ROLLER UNDER THE #1 CRUSHER WHEN HE GOT A FOREIGN OBJECT IN HIS EYE.
