Struck by falling object

Employee was scaling the face when a rock fell from the roof (which was scaled prior to the face) fell and struck EE on the hard hat.

Over-exertion (Not Elsewhere Classified)

There was no missed work or medical treatment on 7/18/23 when EE claimed EE tweaked shoulder climbing onto loader. EE worked until 9/19/23 when EE exacerbated the alleged pre-existing injury. EE was off from 9/20/23 - 9/27/23. EE came back and worked from 9/28/23 - 12/5/2023. EE has been off recovering from surgery from 12/6/23 - current. This was caught in my internal audit.

Fall from ladders

EE just finished greasing a motor at #2 belt and started walking down the catwalk. As EE approached the ladder, EE decided to walk down the ladder facing forward. As EE started to descend down the ladder improperly, EE lost footing and fell forward from approximately 2 feet 8 inches in height and hit head.

The employee was working with two other employees changing plates on the discharge end of the screen. When the employee came out, EE's lower body went down and EE's hands were above EE's head and EE injured EE's left shoulder.

Cone off boom fell off & hit in right shoulder/middle back

Employee was cutting mine belt with a utility knife when the blade slipped and jabbed EE in the left thigh just above the knee. The employee should not have been cutting toward self. EE was taken to Med Express by the on shift EMT foreman (after first aid was rendered) when EE was then referred to the hospital. At the hospital EE received stitches to close the laceration.

Accident type, without injuries

The shift ended at 4pm on July 30th. Arriving to work on July 31st at 5:30pm. I noticed a haul truck had completely burned up overnight. There was no one at the job site overnight. No one injured. Haul truck is outside.

Struck by falling object

Employee was connecting jumper box to batteries on EX1100 excavator. Employee set hard hat beside them so it didn't fall into battery box. Wind gusts were very strong that morning, which blew the battery box lid down and struck employee on top of the head when employee leaned up after connecting the clamps to the battery posts.

Employee was throwing an extension cord up to another employee when lanyard got wrapped in the cord and the lanyard swung and struck the employee above right eye.

The employee was putting a pin back in the hammer boom that worked part of the way out. When stepping down from the dump block he twisted his right ankle.

Employee was checking the tail pulley on the fines collector conveyor on a portable plant. The employee's hard hat fell off. The employee reacted by raising up and hitting the top of his head on a fixed ladder.

Fall from machine

Employee appeared to be working at jaw feed box, without fall protection and fell though the handrail 12 feet 3 inches. He suffered a concussion and fracture of his left thumb. NOTE: Employee was only Kemper Equipment (KE) employee on site and fall occurred in an area we were not contracted to do work. There were no other KE witnesses to the accident.

WHILE UNLOADING STEEL I BEAMS FROM THE BUCKET OF A FRONT END LOADER THE EE WAS STRUCK BY ONE OF THE BEAMS WHEN IT FELL FROM THE BUCKET, STRUCK THE GROUND, AND THEN ROLLED ONTO EMPLOYEE'S LEG.

Struck by falling object

EMPLOYEE WAS FABRICATING DRIP TRAYS FOR OIL STORAGE DRILLS AND USING A GRINDING TOOL. THE GRINDING TOOL SLIPPED OFF THE WORK TABLE AND FELL ONTO HIS LEFT LEG ABOVE THE KNEE RESULTING IN A SMAL L LACERATION. WASN'T REPORTED UNTIL 3-30-01 WHENHE WENT TO THE DOCTOR BECAUSE OF INFECTION.
