Note: Third-party independent over-the-road (dump truck) hauler injury occurred on property associated with sand & gravel extraction activities. Independent hauler was cleaning tailgate off when their finger became pinned between tailgate and asphalt lip.

Sussex Sand & Gravel · Absorption of radiations, caustics, toxic and noxious substances

Injured miner was on a small personnel transport boat at the facility, and was refueling it with a portable gas can. Some gasoline was spilled during the process, and absorbed into the injured miner's pants material. This resulted in a minor chemical burn.

Customer Over-the-Road Truck Driver Injury (not a mining facility employee or contractor). The driver was dumping excess material from truck being purchased & hauled from Sussex facility. Used bare hand to brush excess material away, when finger was pinched between the "dog latch" & "gate closer" of the dump truck tailgate.

Employee was sliding forks in for a Genie GTH-844 Telehandler. After sliding the forks in, he let the front of the fork drop pinching his 5 digit of his right hand between the heel of the fork and the mast of the telehandler

Employee mechanic was at our mine site doing PM to our excavator #18. He was underneath the machine, on the ground in a squatting position, installing the belly pan twisting from side to side and in the process twisted his left knee.

Grates used for guarding were stacked against a wall. He was trying to lift one that fell on the ground. It was partially under the others. As he lifted, the other started to fall forward. He caught them with his hands and thigh. They came to rest on his thighs and knee cap. No cuts, there was bruising and soreness to knee.

EE WAS PICKING UP ODDS & ENDS AROUND PLANT. HE TRIED TO PICK UP SOME 3" REINFORCED HOSE OUT OF THE MUD. THE HOSE WAS STUCK, SO HE HOOKED THE END OF THE HOSE THROUGH HOLE IN LOADER BUCKET. WHEN TENSIONS WAS PUT ON HOSE IT CAME LOOSE FROM LOADER BUCKET AND HIT EE IN THE FACE.
