Employee at Mechanicsville Sand and Gravel Site sustained a left foot injury sprain/fracture. Injury happed while exiting the plant access point. Employee stepped down from the concrete step/edge on to uneven ground/material and immediately felt pain and discomfort. Employee went to the local ER, and an Xray was taken and confirmed a left foot fracture

Note: Third-party independent over-the-road (dump truck) hauler injury occurred on property associated with sand & gravel extraction activities. Independent hauler was cleaning tailgate off when their finger became pinned between tailgate and asphalt lip.

Employee was cleaning out a pipe on the mud press when he cut his finger. He ended up getting 6 stitches, but did not miss any time from work.

was driving pin (with hammer) into excavator tracks. part of pin chipped and part of pin went in left inside leg...resulting in treatment of antibiotics, tetanus shot, one stitch, and general cleaning...missed one day on 5/14/08...returned to work 5/15/08

Employee was operating a farm tractor when a piece of debris flew up and into his eye. He went to the ER where his eyes were thoroughly cleaned. He had a small cut on his eyelid, which required a prescription for antibiotics. There was no lost time.

The employee was changing a screen in the wash plant. He dropped it and it cut the tip of his finger as well as his fingernail. Employee finished working the rest of the day and part of the next(Friday) until his doctor's appt. The doctor cleaned it and wrapped it and gave him prescriptions for Augmentin and Vicatin. The employee returned to work on Monday.

Employee was driving dump truck, got stuck in mud at pit, loader attempted to pull truck out, jerked truck, resulting in her getting a strain/sprain in her mid-shoulders, middle back, and a stiff neck.

EE is a dump truck driver. There was pressure on the tailgate. When he released the handle, it threw his arm back into the hand rail. This caused him to receive bruising on his left hand and elbow. X-rays revealed no breaks. Orthopaedic doctor gave him a note for 2 days off and light duty for one week. The diagnosis was contusions to the left hand and arm.

WATER PIPE BUSTED, HITTING HIM ON LEFT SIDE OF HEAD. UPON FURTHER INVESTIGATION, IT WAS SUSPECTED THAT EE SLIPPED WHILE STANDING ON A BUCKET, HE TRIED TO STOP THE FALL BY GRABBING A PVC PIPE, THE PIPE BROKE & HIT HIM ON THE LEFT SIDE OF HIS HEAD RESULTING IN A MILD CONCUSSION. HOWEVER, EE REPORTED THAT THE PIPE SHOOK WHEN HE OPENED THE VALVE & THE PIPE BROKE KNOCKING HIM TO GROUND.

WAS OPERATING A RUBBER WHEEL LOADER; FRONT TIRE DROPPED INTO A HOLE THAT HAD BEEN DUG FOR A GRAVEL TEST; WHILE TRYING TO GET THE LOADER BACK OUT OF THE HOLE, THE JERKING MOTION CAUSED HIM TO S TRAIN HIS BACK.

EE WAS IN OUR MECHANIC'S SHOP SERVICING A TRUCK. HE TRIPPED ON A GREASE HOSE AND HURT HIS LEFT KNEE.

SEVERE WEATHER- VERY ICY CONDITIONS- HAD COME OUT OF OFFICE AND SLIPPED ON ICE, THOUGHT HE WASO-K, WENT BACK TO WORK AFTER REPORTING IT TO SUPERVISOR, WENT TO DR. ON 3-29-01. ON 4-16-01 HE TOOK OFF WORK BECAUSE PAIN HAD WORSENED. RTW ON 4-17-01, WAS OUT 4-18-01 TO PRESENT.
