Showing all 32Employee was shoveling frozen material from under a conveyor belt. Employee went home and their back started hurting. Employee had a strained lower back muscle and was prescribed a steroid pack and muscle relaxer.
Associate was placing a spud pin in the spud when left pinky finger became injured. Associate Received a Transverse Fracture to the left pinky finger. Associate treated and released.
Dust got in the eye of the employee when they went to retrieve a wrench they dropped. The wrench fell onto a covered belt. Employee hit the cover and dust got into their face. No foreign bodies were found when seen at the doctor. Antibiotic drops were prescribed as precaution.
Deckhand was moving a cable when a burr went through their leather glove and punctured left pinky. The burr had to be removed at the hospital. Antibiotic was prescribed and tetanus shot given. Returned back to work. No other treatment given.
The employee was injured while manning a safety line used to hold in a deck barge. The rope slipped from the kevel and caused employee's wrist to jerk, spraining the wrist.
Employee cut by sharp metal pipe. The crew was installing a safety mid-rail on a walkway when the associate was struck by the sharp end of a 1 1/4 pipe. The Pipe contacted the right hand between the thumb and index finger causing a laceration the injury required other than first aid.
Employee was picking up a 50 gal drum, used as a trash can, and dumping it into a dumpster. When EE went to lift the drum, EE hurt left arm. After medical evaluation EE had a torn left bicep tendon that required surgery to repair.
The employee cut EE's finger in two places while removing spud pins from the starboard spud. The cuts required 13 stitches, total. Gloves were being worn at the time. The stitches were done by MedExpress in NLR.
Day Shift Quarry Manager was inspecting a self retracting life line when they experienced shoulder pain. After a period of diagnostic imaging, it was determined that they had a torn tendon in their shoulder. Surgery was needed to repair the tendon. Surgery was completed on 8/30/19.
Associate was removing driftwood from between barges and dock string
Thoracic Sprain pulling wire.
Taking a wire rope off of a barge timber head. Wire rope had a burr on it and caught my finger.
Employee was helping install a cutting edge on the motor grader. EE right hand index finger and ring finger was pinched between the cutting edge and the mold board. Stitches were required.
EE was attempting to remove a line used to secure barge on dredge. The line had become fouled during normal operation. EE attempted to jerk the line free with right hand when the injury happened in the right shoulder area. Management was notified and arrangements were made for a medical evaluation.
Employee was removing a rock from the walkway of loaded barge. Employee reached down with his left hand to slide the rock off when another rock that was resting on top of the rock he was trying to move released and came down on his left index finger.
Employee was cutting the tabs off of Polydeck screens. Employee went to move a ratchet form that was sitting on the stack of Polydeck panel. The knife slipped out of his right hand and cut his left thumb.
It was a cool sunny morning. Employee was walking up the stairs to operate plant when his knee popped.

Pulling on a wire rope from a winch. His right hand was cut by a peice of the wire rope.
Cut the back of his right hand with a piece of wire rope. He cut his hand on June 6, 2007 but did not go to the Doctor until July 23, 2007.
ee was changing out a battery on a D-8 dozer. Battery slipped and ee dropped battery on finger.

LIFTED A ROCK TO UNSTOP FEEDER AND TURNED/TWISTED.
