Showing all 33Employee was replacing a Return Roller on CV1. The belt had been locked out and the lower section of the belt chained up to remove the old return roller. While picking up the roller to remove from the frame the roller came out and caused compression injuries to DIP's of Left 1F, Left MF, left RF and abrasion to left MF.
Installing the Shell Separator at the Metso 4265 Gyro Crusher to replace mantle and Main shaft. While inspecting the rams miner went to check that one of the rams was tight and when EE turned it the ram came out of socket and fell approx. 6 ft. and landed on the metatarsal part of EE's left foot.
Changing out screens when sharp pain went down left side

Working on guard that covered conveyor gearbox - gearbox fell with hand inside the guard.
EE was working on a water pump. The inside plate fell on right hand and injured his finger. They were trying to put the pump back together.
He was lifting bearings to put in the man lift.
Taking off hose on spider at Gyro crusher. Line twisted and hit him in the face and eye causing area on face and eye to swell and bruise. Prescribed medication was given for pain if needed.
Two employee's were was setting a liner on a cone head. A telehandler was resting the liner on the head. As it did, the liner slid off of the cone and rolled on the ground pinching an employees leg between it and concrete blocks that the crusher sits on resulting in a broken left leg.
The employee was lifting a skirt board back on to a conveyor system that was being worked on. The skirt board was stuck in the mud so the suction on the object made it heavier than usual. The employee twisted while lifting and pulled a muscle in his lower back. The conveyor that was being worked on was locked out.
Rock was lodged in chute. Employee tried to wrap a Chain around the rock and the rock broke free and trapped his arm in the chute. Pry bar was used to free his arm. Rock put a hole in his arm. Required stitches.
EE was changing the trans filter on the 988H. He pulled the filter, turned to move it from it's position. He pulled a muscle in his back. EE was scheduled to be on vacation the rest of the week. He went for follow up yesterday. He had to bend backwards then forwards. His muscles knotted up again unable to work. His 1st official day out of work was yesterday. 11/4/13
He was changing a bearing on a shaker, he was rocking the bearing back and forth when the pain began.
Taking old toggle seat out of the jaw plant to change it and it was heavier than expected. The seat got stuck, another employee wiggled it to get it loose and when it broke free it fell and the employee that was hurt could not hold it and it came down on the employees leg making a small laceration on left leg.
putting a roller in a tote, the roller slipped and mashed end of his 3rd right finger. Finger tip required 2 sutures.
Initial accident was on 12/8/10 when employee helped lift a barrel of chains. approx 200 lbs. Employee felt a strain. He refused treatment at the time. Next morning manager sent him for a check up - possible hernia. 3 weeks for workman's comp to agree for follow-up with doc. Employee went to doc on 1/4/2011. Dr. suggested surgery on the 17th. no loss time till 1/17/2011.
Employees were remooving a step component from crusher. The weight and grease on the part caused it to slip from the hands holding it. It hit the employee in the head and the shoulder.
Employee was picking up a sheet of metal when he had a pain in his back.
w employees were cleaning out rail cars and shutting the doors. One employee hit the side of the railcar causing a spring to break on the car door. The door popped open hitting the other employee in the right shoulder. Bruised the shoulder
Employee was changing the drive belt on the screen deck. The lifting guide broke out of the top of the motor causing the motor to fall on his right hand. The blow caused his hand to bust requiring stitches.
EE was going to jump-start the water pump. Cables slid off of the backhoe & tire. Cable got under rear tire and the other end wrapped around his left leg.
Changing out crusher, weld holding maganese on one side broke letting the maganese roll over bottom section of both legs of employee.
Primary stalled out and had to be dug out. Locking the machine out and beginning to dig the crusher out. Employee dropped a rock which landed on the left foot of injured. Left foot is swollen and tender. Ex-rays taken/no fractures.ice pack, foot elevated. Ortho shoe,crutches given. 5 days off- no weight on foot.
EE THREW A WELDING LEAD UP TO THE CAT WALK, THE LEAD CAME BACK DOWN AND HIT HIM IN THE FACE.
CO-WORKER WAS PULLING A PIECE OF TRICON OUT OF THE SURGE HOPPER WHEN HE KICKED A PIECE OF METAL DOWN ON EE'S NECK & SHOULDER.
PIECE OF METAL ATTACHED TO I BEAM CAME LOOSE, FELL OFF AND HIT EE IN THE FOREARM. CLEANED WOUND,APPLIED NEOSPORIN AND BANDAID.
INJURED EE#1 WAS LIFTING OFF A CONVEYOR COVER WITH ANOTHER EE#2. EE#2 LIFTED HIS END BEFORE EE#1 WAS READY CAUSING EE#1'S FINGER TO BE PINCHED BETWEEN COVER & A STEEL BEAM.
EE WAS CHANGING SCREENS. HE WAS KNOCKING THE BANANA CLIP & IT WAS UNDER TENSION. THE CLIP SPRANG OUT & HIT EE IN THE FACE ABOVE HIS LEFT EYE AND ACROSS THE BRIDGE OF HIS NOSE.
WHILE MOVING IN SCREEN CLOTH, EMPLOYEE PICKED UPONE PANEL CAUSING THE OTHERS TO FALL (10 OUTER PANELS) THE END CAUGHT HIS GLOVE AND CUT HIS ARMAND HAND.
EE AND COWORKER WERE IN THE PROCESS OF EXCHANGING BUCKETS ON AN EXCAVATOR. COWORKER KNOCKED THE PIN OUT AND EE CAUGHT IT WITH HIS BARE HANDS. THE PIN HAD A SHARP BUR ON IT AND THEREFORE CUT HI S FINGER.
EE WAS REPLACING SOME METAL STEPS FOR A LOADER. HE HAD TACK WELDED THE STEPS TO A METAL WORK TABLE. HE WAS TRYING TO BREAK THE WELD LOOSE BY TWISTING & PULLING ON THE STEPS
WE WERE DISMANTLING A ROCK CRUSHER. A MAN ON THE CRUSHER THREW A PIECE OF ANGLE IRON OFF THE CRUSHER. AT THE SAME TIME, INJURED WALKED OUT FROM UNDER THE CONVEYER AND WAS HIT ON THE HEAD. INJU RED RECEIVED A 3/4" GASH IN THE TOP OF HIS HEAD.
