Bodily reaction, (Not Elsewhere Classified)

Employee stopped their off-road truck to use the onsite porta potty. While standing from the driver's seat to exit, they heard a loud pop in their knee and was unable to bear weight. They sat on the truck steps and eased down. Another employee assisted them to a vehicle and transported them to the ER.

Miner was going to wash the window of the CAT352 and when they stepped outside the Phoenix Plant they lost footing causing them to slip down landing on their right elbow.

Employee was installing drive belts on CV22. EE was adjusting the motor mounts when EE's right index finger tip was pinched between the wrench and the mount.

Struck by falling object

Employee was replacing a Return Roller on CV1. The belt had been locked out and the lower section of the belt chained up to remove the old return roller. While picking up the roller to remove from the frame the roller came out and caused compression injuries to DIP's of Left 1F, Left MF, left RF and abrasion to left MF.

Employee was getting a bottle of water from the side cooler of a service truck when the lid fell in the bed of the truck. Employee went to retrieve the lid and while coming down out of the truck lost footing and slipped causing employee to hit the railing on the back of the truck then fall to the ground.

Employee was going up the catwalk on CV1A and fell and landed on left knee causing a bruise.

Employee was removing a roller frame on a conveyor belt by using a grinder to cut the bolts that held the frame. When EE went to start cutting the grinder bounced back off the bolt and contacted the right leg above the knee twice.

Installing the Shell Separator at the Metso 4265 Gyro Crusher to replace mantle and Main shaft. While inspecting the rams miner went to check that one of the rams was tight and when EE turned it the ram came out of socket and fell approx. 6 ft. and landed on the metatarsal part of EE's left foot.

Maintenance crew was removing pinion sheeve for crusher repairs. While removing off the shaft, Tech's Left index finger was struck by assisting Tech with 2 lb hammer.

Employee was climbing off of the crane - to set up outrigger pads - employee slipped and missed the second step coming down - lost footing and fell to the ground.

Employee was cleaning up shot rock on pit floor - bucket hit a rock that was partially buried under loose material - impact jarred them and caused slight back pain stiffness - thought it would go away on its own.

employee was watching chute load railcars - chute was dripping wet fine/material felt something in their eye - thought it was just dust but was irritated - eye was red the next day - when they came in on 8-29-19 eye was swollen - was sent to dr - rock fines were removed from the eye

Employee was walking down the steps out of the E House. EE's foot hit the side rail throwing EE forward. EE missed the last step and hyper-extended EE's knee when EE stepped to the ground. This dislocated the knee cap. Employee has crutches and is referred to physical therapy.

Am EE was tightening the 3/4 ton come-a-long on a loaded train door to close it. This caused too much tension on the chain. The chain broke and something hit IE in the leg causing a small cut and bruising. Job duties were restricted for healing process.

Taking off hose on spider at Gyro crusher. Line twisted and hit him in the face and eye causing area on face and eye to swell and bruise. Prescribed medication was given for pain if needed.

Employee was trying to close a gate on a bin. He was using a wrench. His hand slipped off the wrench and fell between the gate and the bin. The weather was in the 20's with the wind chill factor. He was not wearing his gloves. He had cut his pinky from the knuckle to the end - requiring a few stitches. He has not missed any work.

Employee was driving the long stick over to tower 5 to clean out around it. He started going down a slight incline. He lost control of the equipment causing it to run into/and under conv #14

Employee was trying to knock the manganese out of the crusher with a sledge hammer. The hammer head came off and fell on his finger. Busting his finger. Finger required stitches. Employee has not missed any days.

EE was changing the trans filter on the 988H. He pulled the filter, turned to move it from it's position. He pulled a muscle in his back. EE was scheduled to be on vacation the rest of the week. He went for follow up yesterday. He had to bend backwards then forwards. His muscles knotted up again unable to work. His 1st official day out of work was yesterday. 11/4/13

Employee was washing counter weight on Conv 1A. while squatting down washing under counter weight a rock fell off the conveyor and hit him in the right thigh.

He was changing a bearing on a shaker, he was rocking the bearing back and forth when the pain began.

Employee was putting the foot on the out rig of the crane. Out rig was extended out catching employees foot between the pipe and the out rig.

EE had been bent over working on a pipe. He was walking across the yard when he experienced severe pain shoot up his back. It was originally diagnosed as muscle spasm but diagnosis has been changed to Sciatica. EE is on light duty.

Fall down stairs

Several employees were carrying a screen deck up a flight of steps. Employee fell backwards on the steps striking the upper and lower back against the stairs. He did not hit his head but he did feel his head snap (whiplash fashion).

Employee was busting rock with the excavator. he turned around to get out of the hammer and saw smoke. He got out with fire extinguisher, heard a pop and saw flames. He stood on the tracks and saw flames in the engine compartment. He tried to put it out with the fire extinguisher but could not so he got on the ground to fight the fire.

putting a roller in a tote, the roller slipped and mashed end of his 3rd right finger. Finger tip required 2 sutures.

Employee was trying to loosen a bolt on a piece of equipment JCD 309 haul truck. He was using all of his body weight. The wrench slipped of the bolt causing him to fall against the equipment. His elbow hit the bracket on the equipment.

He was climbing the stairwell at conv#6. He put his right foot on the top landing and his left foot slipped on the top rung. Employee worked his shift and refused medical treatment at the time. The next day he went to his personal doctor. He later decided to follow up with a company doctor. His 1st day missed was 7/21/11 night shift. He has only missed one day at this time.

Employee was walking around the bottom of tower 2. Stepped on something and fell to knee, back started hurting. Went to 1A conveyor and was changing two return rollers. Back started hurting worse.

EE (injured)was spraying lubricant on screen deck while installing new screen, another EE was taping in the new screen with a sledge hammer. He missed the screen and hit EE in the left side of face above the left eye.

Employee had just pulled the master key out of the 990 loader. He was starting to walk toward the truck and turned his ankle on unlevel ground. The ground was very wet due to weather conditions so when the 990 parks it leaves little ruts causing unlevel ground.

Employee was changing screen rails in dewatering screen, when he stood up to grab the light and pry bar. He slipped and his feet came out from under him. He tried to grab the spray bar to stop the fall. This caused his arm to be snatched hurting his shoulder.

Initial accident was on 12/8/10 when employee helped lift a barrel of chains. approx 200 lbs. Employee felt a strain. He refused treatment at the time. Next morning manager sent him for a check up - possible hernia. 3 weeks for workman's comp to agree for follow-up with doc. Employee went to doc on 1/4/2011. Dr. suggested surgery on the 17th. no loss time till 1/17/2011.

Employee reported soreness in shoulder after swinging 16 lb sledge hammer. Employee states shoulder has been hurting for some time (a year or more) but is hurting worse now.

Employee was operating the hammer. He noticed several spiders on the hammer and killed them. Apparently one of the spiders crawled up his pants leg and bit him on the thigh. By the 6th his leg had swollen and puss had formed. He went to the Medical Center where the bite was lanced and packed. Not sure what kind of spider it was. He is still undergoing treatment

Employee was torching old wear metal off tail pulley. When torching, He hit a air or trash pocket in steel. That caused hot metal to shoot up and go under his glasses hitting him in the right eye.

This accident injured a contractor who was working by himself at our mine site. His was reparing the hammer (rock breaker). Somehow his hand became caught in the shaft by the cylinder. His hand received major abrasions to all four fingers.

Employees were remooving a step component from crusher. The weight and grease on the part caused it to slip from the hands holding it. It hit the employee in the head and the shoulder.

Employee breaking down crusher, swinging sledge hammer, pulled shoulder. Employee has not missed any time but is still having trouble, may end up missing time.

Employee was flagging trucks on Hwy 90 while our RR crossing was being repaired. He began to feel dizzy, vomiting and headache from getting too hot.

Employee had collected a sample of rock and was crossing the drive. Supervisor was backing out the drive. It is a steep incline. They did not see each other. Employee was hit by the truck causing a bruised foot and a minor chip to his hip bone.

Spider bite - got bite on both legs while cleaning out the lab. He thought it might be an ant or mosquito bite. Both legs were swollen the next day.

Employee slipped off the loader bucket with a piece of 2X1 steel in his hand. The steel smashed his fingers against the bucket.

Performing welding, machining and grinding operations and metal debris entered left eye.

Employee was standing on conveyor #19 shoveling. When he had the belt cleaned, he tried to step off the side of the conveyor. He slipped and fell about 4 feet. It was raining which contributed to the slippery conditons.

Employee was welding during his shift. He woke the next morning with his eyes scratchy, swollen, hurting. His face looked as if he had been sunburned.

Employee on the back of a rail car switching cars around on the tracks. There was not enough clearance between cars to pass without touching. Moving cars made contact with stationary cars causing the hand rail of one car to be pushed up against employee. He was straddling the hand rail when the train stopped. His steel toed boots helped protect his foot.

Employee was putting return trainer back on Conveyor #!. He slipped and fell 3 feet hitting an angle iron. It knocked the breath out of him and bruised his rib cage.

Two employees were on the manlift to check screens in tower 3. A bolt fell off the manlift. They yelled ""look out below"". The contractor looked up and was hit on the left side of the face - left high cheek bone - eye area & chin.

An employee was cutting a bearing off conveyor #8 with welder. He looked up and saw screen deck on fire. Unable to put out with extinguisher; accelerated out of control. Conveyor #11 also caught fire. Fire Dept. was called; they extinguished fire. A fire blanket was being used - located on top of the screen deck. Poss. cause: slag falling onto the rubber screens.

Miner was descending a ladder after patching holes in a chute. Ladder shifted causing miner to fall.

It was raining. Employee was tightening a bolt on the screen deck. He slipped and fell on the step.

Employee was on conv. belt 18 putting in clips. Asked another employee to get drill and bucket of clips. Employee misunderstood instructions and turned on the belt. Lock out, tag out was not observed. E-stop was not pulled prior to employee getting on the belt.

Driving Haul truck too fast and lost control on curve. Truck was loaded with materials. Truck rolled several times.

Employee was filling the water truck. He stepped on the bottom chain step, slipped and fell on the rocky ground. Complaining of hurt shoulder, stiff neck.

Primary stalled out and had to be dug out. Locking the machine out and beginning to dig the crusher out. Employee dropped a rock which landed on the left foot of injured. Left foot is swollen and tender. Ex-rays taken/no fractures.ice pack, foot elevated. Ortho shoe,crutches given. 5 days off- no weight on foot.

The miner was welding on an empty oil drum that was not ventilated. The drum exploded causing the lid to hit his upper body and jaw causing injury.
