While operating a haul truck, a bump was struck resulting in a numbness to left arm.

The employee allegedly was bitten by an insect/spider at some point during the day.

EE was operating the scale house and having to bend over to operate the small bucket machine (4-6 times). EE's lower back became sore. EE has a lower back strain.

EE was walking to pump barge, slipped/fell with right knee striking a rock.

While operating a haul truck, the truck was jarred by a bump. The employee felt a little pain in lower back.

A piece of aggregate material came off a conveyor and struck the employee in the lower back area. The employee has lower back pain and x-rays were negative. EE was released to light duty.

Employee was exiting the dragline and fell off (unknown distance 0'-4') landing on the ground. Employee does not know why they fell. It was a sunny and dry day.

The employee was lifting a skirt board back on to a conveyor system that was being worked on. The skirt board was stuck in the mud so the suction on the object made it heavier than usual. The employee twisted while lifting and pulled a muscle in his lower back. The conveyor that was being worked on was locked out.

Rock was lodged in chute. Employee tried to wrap a Chain around the rock and the rock broke free and trapped his arm in the chute. Pry bar was used to free his arm. Rock put a hole in his arm. Required stitches.
