Miner was shoveling rock into buckets at the tail end of the limestone tunnel and hauling the buckets outside the tunnel to dump. Miner hurt their lower back in the process.
The employee was assisting in installing a new drop pipe. The pipe remained secured but shifted from its placement, and the employee on the ground had a hold of it to keep it from swinging and felt the impact of the pipe shifting and felt discomfort in EE's back.
The Employee was caring an electric winch onto the Raw Mill to help with the Discharge grate job. While descending the steps to the mill The Employee trip and fell on top of the mill with the winch in hand. They injured their left ankle during the fall.

Employee was getting out of haul truck to load production and experienced spasms in their back. Called supervisor for assistance and was taken for medical attention. Placed off work for 1 week.
Grinding on duct, raised face shield, debris got in right eye
Miner had pipe on sawhorses to weld on flange. As EE was turning pipe to weld on flange, the flange fell off the pipe and dropped on the edge of EE's foot. The flange sliced through the boot and lacerated the edge of EE's foot. The miner received 6 stitches and modified work duties.
While pulling pieces of conveyor belting by hand 10'- 13' long, a back strain occurred. Doctor's diagnoses: Acute thoracic back pain.
EE was performing maintenance and cleanup (during outage) around the Pan Conveyer. EE was attempting to remove a large piece of clinker without removing the guard and pinched ring finger on right hand between bottom of guard and clinker. EE saw physician 2 days later and had fingernail removed.

Miner making rounds collecting samples for the lab. Strained right shoulder lifting elevator doors.
Worker was sliding scraper liners on a pallet and felt a strain on left side of stomach.
EE attempted to close pump 5 valve when EE felt a 'pop' in chest. EE called for assistance-reporting discomfort in chest-EMS transported to ER for exam.-Was released w/instructed to see PC Provider. 8/16/22 CNP examined released w/10lb lifting restrictions re: strained/pulled pectoral muscle - follow-up Appt. 10 days. No lost workdays to date.
The condition of the site was dry. The employee took a shortcut between 2 end sections of the multi-plate. The edges of the multi-plate are sharp. The employee's hand slipped on the sharp edge of the multi-plate causing a laceration on the right ring finger. Requiring 5 stitches.
Miner broke right ring finger when the coal system baghouse door slammed shut, catching EE's fingers between the door and frame.
Two employees were carrying a pump screw to place in a box. One end slipped out of an employee's hands causing the other end to fall while the other employee still held it. This resulted in a smashed finger between the wooden box and pump screw.
2 employees were moving a water pump down the coal tunnel on a sled. Injured employee was above and holding onto pump with left hand and railing with right hand. While moving downhill, the employee's right arm extended backwards and employee felt a popping in forearm. Diagnosed with torn bicep and placed on restricted work duties.
Employee was unfamiliar with this type of Freight Elevator and while closing the elevator door, smashed EE's ungloved hand between the doors.
Employee was throwing a bag of trash into the dumpster.
Miner was lifting 75lb vibrators to install into vibrator brackets on side of outlet hopper on a railcar. EE placed 3 into their resected brackets but when went to lift the 4th vibrator was struck with sharp lower back pain. Railcar is 37" height and 32" from ground to bracket, and 16" deep. Area was free of debris and without any trip hazards.
Employee did the line change on the 6" line to set up in cement Silo, then came down from Silo to inspect Kiln grease system and proceeded to clean additive system shoveling material and pain got worse. EE attempted to sit in the 320 loader, but the pain intensified. EE immediately reported it to the control room to report it to the supervisor.
While attempting to climb a ladder the employee lifted right leg and felt a snap on right knee
Lowering hand rail on scissor lift and a hand was in the pinch point.
Employee was removing breathing apparatus from the storage case when the unit slipped out of employee's hands. Employee moved quickly downward to catch the unit and while doing so, struck head on the opened lid of the case causing a laceration above the right eye. 5 sutures were required.
Employee was scooping a sample out of the crusher bowl and the bowl slipped and landed on EE's finger.
Employee was pulling a cover off of a trolley line in clinker shed but cover was really heavy and when EE pulled harder EE pulled a muscle in EE's right, upper back.
This incident was not reported to the company until July 7, 2020. The employee was helping lift the core drill in place when EE felt a sting in the groin area.
Employee noticed some rocks in the haul road, while EE began to remove them to prevent a tire puncture EE pinched the right pinky finger between two rocks, lacerating it.

An EE was closing a silo gate, in a vertical motion, and got left pinky finger caught in between the gate and the metal frame as it closed. The EE sustained a small fracture to the small bone in the pinky finger tip.

Employee was doing maintenance on elevator when a large metal pin came loose, bounced off of the door and hit the employee in the face causing a laceration.

Contractor was rolling a conveyor chute out of the way and hand was pinched between the chute and the wall cutting EE's left pinky finger.
Employee was helping to load I-beams on a dolly. EE lifted the end about a foot and a half and set on dolly. EE felt shooting pain during lift and EE's hip to ankles felt bad. EE continued to work until end of shift at 7:00 pm. Management was made aware of the incident on December 23rd.
Employee states was sampling Raw Mix through a rectangular opening. The employee was holding the cover to keep it aligned and it came down and pinched thumb.
Employee was removing a guard from the v-belt on the cone crusher, while doing so the guard got stuck and when it was tugged on the guard became unstuck abruptly from the belt striking EE's finger against the pulley between the guard causing the injury.
Was bending bolt downwards. Gloves were muddy, bolt slipped out of hand, springing upwards and striking them on chin and teeth, causing laceration and chipped teeth.
Employee was lifting a port a power from a box when EE's foot slipped causing EE to drop the port a power back into the box. When the port a power fell it struck EE's left hand resulting in a fractured finger and laceration requiring stitches.
While installing a coal mill top, the lid got stuck and the employee put fingers between the top and the mill and EE shook it. When the lid became unstuck, it came down and smashed EE's fingers.
