He was trying to lift a piece of pipe from the upper level of the pipe rack, however a piece of 1 7/8 inch copper pipe was on top of the stack. As he moved the pipe he was after serveral pieces of pipe rolled off the stack and one piece stuck the inside of his right foot.

Showing all 11EE was done probing a hole. He was putting the probe back into the truck and when he turned with the probe in his hand he felt a pulling sensation in his upper back.
About 9 hours after cutting a piece of steel with a plasma cutter he tipped his hardhat and a piece of steel, about the size of a grain of salt, fell into his eye. He tried to irrigate with lots of water. It would not come out. He went to the clinic and they removed it. They gave him a prescription for possible infection on 9-15-2004. No permenant damage was done.
EE INJURED HIS FINGER WHILE STRAIGHTENING DRILL PIPE IN RACK. HE SMASHED HIS MIDDLE FINGER BUT DID NOT GO TO HAVE IT CHECKED UNTIL 7/27/02. THEY TOLD HIM THAT A BONE WAS CRACKED BUT COULD DO NOTHING TO HELP IT.
OPERATOR RUBBED HIS LEFT EYE WITH HAND AND APPARENTLY CAUSED A SMALL OBJECT TO ENTER EYE/EYELID.COULD NOT WASH IT OUT. WENT TO PHYSICIAN WHERE IT WAS DETERMINED THAT NOTHING WAS IN THE EYE BUT THE LENSE WAS SCRATCHED.
EE WAS CONNECTING A 3" WATER HOSE WITH A CAM AND GROOVE COUPLER TO A TRUCK CONNECTION. THE CAME WAS DIFFICULT TO CLOSE, SO THE EE ATTEMPTED TO POSITION HOSE WITH BODY. THIS EFFORT APPARENTLY R ESULTED IN A LOWER BACK STRAIN.
EMPLOYEE WAS PERFORMING MAINTENANCE ON DRILL RIG DRAW WORKS GEAR BOX. ANOTHER EMPLOYYEE THOUGHT WORK WAS COMPLETE AND RELEASED THE BRAKE ON THE DRUM. CABLE END CUT RIGHT WRIST, REQUIRING FOUR SUTURES.
INJURED AND ANOTHER EMPLOYEE WERE RIGGING UP PULLING UNIT TO PULL A WELL. INJURED REACHED FOR THE SANDLINE BUT GRABBED MAIN CABLE INSTEAD. RIGHT 5TH FINGER WAS PULLED INTO CABLE SHEAVE. OTHER EMPLOYEE STOPPED OPERATION AND REVERSED MOTOR FREEING SHANE'S FINGER.
EE WAS RACKING 2 7/8" DRILL PIPE, HE LIFTED UP ON A STEEL BAR AND HURT HIS BACK.
WAS LIFTING A PUMP JACK INTO A TRUCK WHEN THE ARMS OF THE JACK SHIFTED, PINCHING THE LEFT INDEX FINGER AGAINST THE PUMP CASING. CAUSED A CUT TO BE THE FINGER REQUIRING THREE STITCHES.
EMPLOYEE WAS PULLING AND MAKING UP WIRING IN A HEADER HOUSE MOTOR CONTROL CENTER WHEN HIS RIGHT HAND BRUSHED AGAINST THE DOOR EDGE (STEEL) CUTTING HIS KNUCKLE. MINOR LACERATION RESULTED WHICH LATER BECAME INFECTED AND RESULTED IN MEDICAL TREATMENT.
