Struck by falling object

He was trying to lift a piece of pipe from the upper level of the pipe rack, however a piece of 1 7/8 inch copper pipe was on top of the stack. As he moved the pipe he was after serveral pieces of pipe rolled off the stack and one piece stuck the inside of his right foot.

EE was done probing a hole. He was putting the probe back into the truck and when he turned with the probe in his hand he felt a pulling sensation in his upper back.

Struck by falling object

About 9 hours after cutting a piece of steel with a plasma cutter he tipped his hardhat and a piece of steel, about the size of a grain of salt, fell into his eye. He tried to irrigate with lots of water. It would not come out. He went to the clinic and they removed it. They gave him a prescription for possible infection on 9-15-2004. No permenant damage was done.

EE WAS USING A PIPE WRENCH TO LOOSEN A PIPE FITTING. AS HE PUSHED DOWN ON THE WRENCH WITH HIS RIGHT ARM HE FELT A POP IN HIS RIB CAGE ONT HE RIGHT SIDE.

Fall from ladders

THE CONTRACTOR VICTIM WAS CLIMBING DOWN AN A-FRAME LADDER WHEN THE FRONT LEFT LEG BEGAN TO MOVE.THE VICTIM COULD NOT REGAIN THE LADDERS BALANCE AND FELL 5' TO THE FLOOR WITH THE LADDER. THE REASON THE LADDER BECAME UNSTABLE IS UNKNOWN.

REPORTABLE HEARING LOSS OF 26.7 DB IN THE RIGHT EAR. PHYSICIAN UNWILLING TO STATE IF HEARING LOSS IS WORK-RELATED. NOISE MONITORING OF EMPLOYEE REVEALED THAT HIS NOISE DOSE IS 6.07% OF THE 90 DBA TWAS. NO OTHER EMPLOYEES IN THE SAME POSITION HAVE DEMONSTRATED A SIGNIFICANT HEARING LOSS DUE TO WORK CONDITIONS.

EE SLIPPED WHILE CLIMBING DOWN A LADDER OFF OF A CEMENT TRUCK. EE STOPPED HIS FALL BY PLACING ELBOW ON RUNG. THIS ACTION STRAINED HIS LEFT SHOULDER AND NECK.

EE INJURED HIS FINGER WHILE STRAIGHTENING DRILL PIPE IN RACK. HE SMASHED HIS MIDDLE FINGER BUT DID NOT GO TO HAVE IT CHECKED UNTIL 7/27/02. THEY TOLD HIM THAT A BONE WAS CRACKED BUT COULD DO NOTHING TO HELP IT.

EE WAS PERFORM,ING MAINTENANCE ON RIG; THE KELLY WAS ROTATED TO TIGHTEN. THE EE'S FINGER WAS PINCHED BETWEEN THE PIPE WRENCH AND THE DERRICK LEG.

EE WAS CONNECTING A 3"" WATER HOSE WITH A CAM AND GROOVE COUPLER TO A TRUCK CONNECTION. THE CAME WAS DIFFICULT TO CLOSE, SO THE EE ATTEMPTED TO POSITION HOSE WITH BODY. THIS EFFORT APPARENTLY R ESULTED IN A LOWER BACK STRAIN.

WAS LIFTING A PUMP JACK INTO A TRUCK WHEN THE ARMS OF THE JACK SHIFTED, PINCHING THE LEFT INDEX FINGER AGAINST THE PUMP CASING. CAUSED A CUT TO BE THE FINGER REQUIRING THREE STITCHES.
