Service Technician was performing maintenance on an impact crusher, rotating the impact blow bars. While handling one of the bars EE lost grip and it slipped out of EE's hand. A sharp edge on the blow bar cut the palm of EE's hand, requiring 5 stitches to close the wound. It was a very hot day and EE decided not to wear protective gloves.

Miner was doing maintenance on one of the conveyors down on the stone crushing plant. EE was standing on the ground reaching slightly up to push over the portable electrical box, and as ee stretched and pushed off with left leg, ee heard a pop and immediately had shooting pain on back side of left ankle.

MINER WAS BENDING OVER TO LIFT A PIECE OF LIMESTONE APPROX 30 LBS WHEN EE FELT A SUDDEN SEVER PAIN IN EE's BACK AND SIDE AREA

Miner was sliding/pulling a piece of limestone on cut belt to trim it, and ee had fingers below the stone as ee was maneuvering it into correct position to be cut, and the stone broke from a weak spot and it fell onto Left pinky finger causing contusion.

Miner was doing handstack to the conveyor. As they were placing a piece of limestone onto the conveyor table, miner must have misjudged the height, smashing the stone in their hand (pinky finger) against the conveyor table.

Miner was banding a pallet of limestone when they lacerated their thumb when adjusting the bander.

Miner was picking up a piece of limestone and they felt a stab sensation in groin area, left side

Miner was reaching across conveyor belt, and as the stone was coming up EE reached out and over to grab a small piece EE thought was going to get stuck in the turn. As EE was stretching out across the belt EE felt a pain on the backside of the left knee.

Miner was lifting a piece of limestone approx. 70lbs with the help of a co-worker. He felt a strain on his right side and couldn't finish lifting.

Miner was lifting a piece of limestone, approx. 75 lbs from knee level while turning to place it onto a pallet. He heard a "crack" and immediately felt his muscles on the right side of his back tighten up.

Miner was cutting limestone on a Hydra-Split machine. Between cuts, he lifted the extraction plate with his right hand & used his left hand to clear debris out from area underneath. The plate slipped out of his right hand, and fell landing onto his left hand/fingers which were still below in the area of the stone chip & debris causing crush injury to left index finger.

Miner reported on 3/24/15 that he had been seen in a clinic on 1/26/15 due to having pain in the groin area since Mid December. He was working throughout winter as weather would permit, while being on partial layoff. He was informed he had a hernia. He remained working on & off lifting no more than 10 lbs.

Miner was lifting a piece of limestone 4-6" thick weighing approx 100 lbs with the assistance of another miner. He continued working and later stated he didn't feel right in the groin area.

Miner is a quarry Block Worker. He was ground stacking pallets when a piece of limestone slipped from his hands crushing his fourth left finger

Employee was climbing up to get into a Quad Axle dump Truck and as he was reaching and pulling he felt a sharp isolated pain in his low back.

Minor was cutting limestone on Hydra-Split Machine. He grabbed a piece of limestone to turn it into position for cutting and a piece broke off and his arm slid across the sharp stone edge cutting his wrist.

Minor was in quarry stacking limestone on a pallet and he struck a sharp edge of a stone piece causing a laceration to his left arm

Miner was working on block in quarry splitting a piece of limestone with a chisel & wedge, and an underneath piece fell loose landing on the top of his left foot just in front of the edge of the steeled toe boot, causing a contusion and laceration

Miner was working in the saw shop - placing limestone on pallet. The limestone slid out of his hands and the sharp edge lacerated his finger.

Miner was working on the quarry block and as he was walking to transfer a piece of limestone to the pallet, the stone must have split on an underneath seam, and the piece broke off & fell striking his leg that was stepping out.

Minor was transferring a piece of limestone from a pallet to feed the cutting machine. As he placed the stone onto the rollers of the splitter he flipped the stone and caught his fingers between that piece and another piece of stone

Miner was transferring limestone to a pallet when a piece slipped from his hand. It fell striking his left ankle area.

Minor was lifting & pushing veneer stone onto the conveyor to be cut by the saw, when he experienced wrist/hand pain and was having trouble holding onto anything.

Minor was transferring limestone from a pallet to rollers on th Hydra-split machine to be cut into product. When the minor picked up a piece the one under it fell off the pallet and landed on his right foot causing a contusion. Minor was wearing all required PPE.

Bending & Lifting - low back pain

Miner was carring a piece of limestone to be transferred onto a pallet, when he twisted wrong to set it in place.

While stacking a piece of quarry limestone on a pallet, miner lost his footing and slipped on the uneven quarry floor. He then dropped the stone on his left leg just above the knee and twisted his left leg.

Employee reported lower back pain on 9/19/06.

Employee was lifting a piece of limestone when he felt a pull in his lower groin area.

Employee was feeding the conveyor with limestone from a pallet when the pallet gave out, the stone fell onto left leg & foot resulting in a large, deep laceration to his left leg & a contusion to his left foot.

Employee was tranferring limestone from pallet to conveyor, when limestone dropped on right leg causing laceration

While transferring stone from one pallet to another, stone broke and fell on knee and upper leg.

While tranferring limestone from wood pallet to landscape truck, stone spun pinching finger between the top limestone & limestone underneath.

while transferring limestone slab from pallet to feeding the conveyor, the pallet shifted and the stone was farther out on pallet and leg brushed against causing an abrasion

while feeding the conveyor with limestone slabs, a piece gave out and it dropped on left foot

Repetitive lifting of limestone caused lumbar strain

while stacking limestone on pallets employee dropped a limestone slab on right foot

EE WAS LIFTING PIECES OF STONE FROM A PALLET AND PUTTING THE PIECES ON A CONVEYOR BELT TO BE CUT. EE FINISHED THE WORK DAY. THE NEXT DAY HE SAID HIS BACK WAS SORE & WENT TO CHIROPRACTOR. HE SH OULD NOT HAVE LIFTED PIECES OF STONE THAT WERE TOO HEAVY. A FORKLIFT IS AVAILABLE AT ALL TIMES.

EMPLOYEE WAS TAKING PIECES OF STONE (APPROX 20 LBS) EACH OFF A PALLET AND PLACING ON THE CONVEYER BELT WHEN HE PINCHED HIS FINGER BETWEEN ANOTHER PIECE.

EMPLOYEE WAS STACKING STONE ON PALLETS WHEN HE CUT HIS LEG ON A PIECE OF STONE THAT WAS ALREADY ON A PALLET. EMPLOYEES ARE ENCOURAGED TO WEAR LONG PANTS.

EMPLOYEE WAS TRYING TO SPLIT OPEN A PIECE OF STONE WITH A HAMMER AND WEDGE WHEN HE HIT THE STONE A PIECE BROKE OFF AND HIS NATURAL INSTINCTWAS TO TRY AND CATCH IT. HE SHOULD HAVE LET THE PIECE FALL TO THE GROUND OF LET THE LOADER SPLITTHE STONE.

EMPLOYEE WAS REMOVING STONE FROM CONVEYOR & STACKING ON PALLET. HE CUT ARM ON A PIECE THAT WAS STICKING OFF PALLETS. EMPLOYEES WERE REINFORCED TO MAKE SURE STONE IS STACKED SQUARELY ON PALLETS SO NOT TO CATCH ANY SHARP EDGES & ALSO MAKE SURE THERE IS AMPLE ROOM BETWEEN PALLETS OF STONE.
