Employee was filling 2000# bulk bags with two fellow employees. The bag had been removed from the support and was being prepared to be lifted by straps when it started to lean. EE tried to hold it until the forklift could pick it up. EE was unable to hold it and it tipped over. EE was too slow in getting out of the way.

Employee was bagging on the #10 bagger and the bag busted and spewed talc into the air and employee inhaled a large amount of talc through nose and mouth. Employee was not wearing the issued respiratory gear.

Employee cut his finger while working on loader 10. He was removing a cylinder pin. He got his finger pinched and a cut. He reported the injury. First Aid treatment was given. He refused the ER and went back to work. On May 20 the Mine Manager discussed his finger pain. Employee then went to the ER. X-ray was negative. Antibiotic was prescribed. Employee returned to work.
