Employee was filling 2000# bulk bags with two fellow employees. The bag had been removed from the support and was being prepared to be lifted by straps when it started to lean. EE tried to hold it until the forklift could pick it up. EE was unable to hold it and it tipped over. EE was too slow in getting out of the way.

Employee was bagging on the #10 bagger and the bag busted and spewed talc into the air and employee inhaled a large amount of talc through nose and mouth. Employee was not wearing the issued respiratory gear.

South Door of mill by the ACM- Rollers were hung up

Employee cut his finger while working on loader 10. He was removing a cylinder pin. He got his finger pinched and a cut. He reported the injury. First Aid treatment was given. He refused the ER and went back to work. On May 20 the Mine Manager discussed his finger pain. Employee then went to the ER. X-ray was negative. Antibiotic was prescribed. Employee returned to work.

Employee was filling bulk bags. It was a long step down from the platform he was standing on. When he stepped down, he felt a pinch in his abdomen.

Employee was cleaning up. He lifted and moved a 50# bag to empty it. As he did this he felt a pain in his groin area.

Employee was lifting an empty wooden pallet and felt a sharp pain on his side.

Employee was checking a line that was plugged with product. When he took the line apart, dust blew into his face and eyes.

Accident reported 4/22/2008; after employee had been off for illness. Employee reported he had lifted 20 pallets weighing approximately 45 lbs each over a shift. Employee reported this caused internal injuries.

Left lower leg was caught under bagger roller stand at time of turn of body to set bag on pallet.

NEW BELTS WERE BEING INSTALLED ON ELECTRIC MOTOR AND GEAR REDUCTION BOX, ELECTRIC MOTOR WAS STARTED AND GEARS WERE NOT TURNING IN GEAR REDUCTION BOX.
