Associate was closing gate at equipment storage. While closing the gate a red wasp came towards him and associate's instinct was to jump back, when he did his left ear contacted the razor wire on the top of the gate. He received a laceration to his left ear that required 8 stitches.

Miner was helping another miner to pick up an irrigation pipe and move it to the side when he felt a pin in his lower back. The footing was loose and his foot slip and caused additonal pressure to one side. Strained lower back area.

Associate was unbolting a roller from a scraper without proper support and when bolt was removed the roller twisted and pinch the associate's pinky resulting in it being smashed and lacerated.

Operator was changing 7' cutting edges on road grader. Operator removed all bolts from cutting edge and found it in a bind. Operator removed 3 of the 4 bolts in the outside cutting edge when 7' cutting edge came free and fell striking him in the right shin and right foot.

Employee was unloading scrapper teeth from a loader bucket that was lowered to the ground, and neatly stacking the teeth on a trailer. The employee was carrying one tooth (approximate weight 12.5 pounds each) in each hand, while setting down the tooth with the left hand, employee felt a "pop" in his left arm, that is when he noticed a knot on his arm and felt pain.

Operator's finger was mashed between jack handle and machine floor when chain broke causing operator to lose balance. Finger on left hand required stiches.

Employee was entering mining break room. As he pulled the door closed from inside with his left hand, he was touching the door with his right hand. The hinged edge of the door caught the middle finger of his right hand, ee received a severe avulsion.

2 EE'S WERE LYING UNDERNEATH A CAT 930 TIGHTENING DOWN THE BOLTS TO THE CENTER PIN. ONE WAS SPRAYING WD-40 WITH A SPRAY BOTTLE AND THE OTHER WAS USING AN AIR RATCHET. THE EE USING THE AIR RATC HET WAS ACCIDENTLY SPRAYED IN THE FACE WITH THE WD-40. THE LUBRICANT ENTERED EE'S EYES.

EE WAS WORKING ON THE AIR BREATHER ON A 994 FRONT END LOADER. HE DROPPED A TOOL, STARTED TO CLIMB DOWN. HE SLIPPED STARTED TO FALL GRABBED THE PIPE CONNECTED TO THE AIR BREATHER. THE PIPE AND AIR BREATHER WAS NOT SECURED TO THE MOTOR. THE PIPE AND AIR BREATHER CAME OUT AND FELL ON TOP OF EMPLOYEE.

THE EE WAS MOVING A 55 GALLON BARREL OFF THE PALLET. THE DRUM WAS HEAVIER THAN HE THOUGHT AND IT SLIPPED AGAINST HIS STOMACH THEN HE PUSHED IT BACK ONT HE PALLET. DID NOT WANT TO SEE DR THEN O N 9-10-01 COMPLAINED OF PAIN IN THE STOMACH WHERE BARREL HIT HIM. TOOK HIM TO CLINIC.

EMPLOYEE WAS TURNING A PICE OF STONE WITH THE HELP OF TWO OTHER EMPLOYEES ON TOP OF ROLLERS. ONE EMPLOYEE DROPPED HIS END OF THE STONE CAUSING THE STONE TO FALL, PINCHING OR SMASHING ONE EMPL OYEES HAND BETWEEN THE STONE AND THE ROLLER.
