Employee was moving miner cable out of the roadway when he felt a sharp pain in his lower back on the right side and down his right leg. Continued to work up until 4/24/2014 when he was taken off work by a doctor.
Two employees were moving crib block out from under head on miner. Employee went to pick up crib when the other employee tossed another crib hitting him on the left pinky finger pinching it between the crib he was picking up and the crib thrown.
Employee was changing out a belt roller and smashed his right ring finger when pulling roller from belt structure resulting in stitches.
Employee was removing box of soap from metal cabinet. Right hand came in contact with door frame cutting his right index finger resulting in stitches.
Employee and 3 others were loading 4" plastic water line on flat car. Water line was raised over the standards on the flat car and slipped out of the hands of one of the other men. When the end of the water line hit the bottom it mashed his finger between the water line and the standard on the flat car causing a laceration and fracture to the end of his finger on his right hand.
Employee was walking down the steps from the 6th floor to the 5th when he felt his right knee pop.
Employee was retrieving truck sheet from loader. As he climbed up the ladder he felt pain in his right shoulder. He continued to work up until he had shoulder surgery on 3/20/2014.

He moved the booster cable leads on the 4-wheeler batteries and the top of the battery blew off. The employee got battery acid in his eyes and on his face.
Employee was injured while assisting another employee cut a piece of 8" steel. The weight shifted once the steel was cut allowing it to drop approximately 2-3" striking his left hand.
Employee was plastering a brattice and got plaster in his eye.

Employee was removing protection screens and channel over sump while chipping rust off channel. He was punctured by a piece of screen that was extended above the channel he was preparing to remove. The puncture required 2 stitches.
He was removing a 4" piece of PVC waterline that was hung from the roof with chains. As he removed the coupling the water line hit him in the mouth busting his lip and chipping his two top front teeth.
Employee was turning a turnbuckle to align 10B #2 belt drive when he felt a sharp pain in his shoulder. He returned to his regular job duties and on 11/14/2011, after a follow up doctor's visit, he was put on restricted duty.
Employee was bolting in the #2 heading when a draw rock fell. He was trying to move the rock himself and in doing so twisted his right knee.
Employee was watering Belt entry with a 1 inch water line with a metal cutoff valve end. While pulling up slack on the water line, the water hose caught on something, pulling the line through his hand cutting the area between his index finger and thumb requiring Seven (7) stitches.

Air pressure caused the door to close catching his left index finger to get caught between the door and roof bolt, cutting and mashing his finger.

An evening shift electrician was loading tool box on 3-wheeler. He alleges that he felt sharp pain in his lower back. Employee began losing time on 3/10/08.
Employee was hanging a trailing cable, he turned slightly and felt pain in his left knee. He began losing days on 4-1-08.
Employee stated he was exiting the mantrip; when he turned he felt pain in his right knee. Doctor removed him from work on 1-24-2008.
