Employee was installing the finisher steel in hole. The steel got stuck so EE pulled it out some then tried to shove it in the hole. When the steel got stuck again EE felt a pop in left shoulder and experienced pain. Employee started missing time on 6/24/16.
Employee was helping pull the head shaft out of the feeder. The right side slid and came out sooner than the left side causing the right side to drop suddenly and the left side to rise up. The event pinched EE's wrist between the head shaft and the frame of the feeder resulting in a fracture to right wrist.
EE alleges while hanging an empty 2 inch waterline EE felt pain in EE's lower back. EE has not missed work, EE began working restricted on 4/12/16. Alleged incident is under investigation. Due to the reporting deadline, this document is being submitted to comply with reporting criteria until full determination of actual injury and/or incident can be substantiated.
Employee states while throwing a garbage bag full of old boots and trash into the back of the pickup he felt pain and a burning sensation on the top of his right shoulder. He started physical therapy on 2/5/16. *Continued to work until surgery on 4/1/2016.
Bates Contractor was helping hang miner cable. When lifting the cable up to the roof he felt pain in his lower abdomen. This resulted in a hernia.
Employee was moving roof caps (pizza pans) to drill operator station when he twisted around to put the pans on the drill station he felt pain in his lower back and right thigh.
Employee had drilled hole for 10 cable and installed resin. He reached to get cable bolt to start in hole. As he reached for the bolt he heard a "pop" in his left shoulder and felt pain.
Employee was in the process of dropping pinner cable. When he turned, highline hanger hook caught him in the right nostril causing a laceration resulting in 2 stitches.
Employee stated that he was dragging a piece of wire mesh to the #4 when he experienced pain to his hip and leg. Employee has been receiving ongoing medical care for preexisting injury to this area in the previous months.
Employee was working on ventilation controls when setting pogo and it came dislodged striking him in the mouth breaking his left central incisor. The pogo was not under a heavy load but appeared to have been caught in the line curtain causing it to hang.
Bates Contractor was installing wire mesh. He drilled his hole, put the glue and bolt in the hole and dropped it down to the drill chuck. He stated it didn't line up and was fouled to turn by hand so he bumped the rotation and it twisted his hand around the roof bolt. This resulted in dislocation to the left ring and pinky fingers and a laceration requiring stitches to his pinky.
Employee was installing an ATRS valve package on the section roof bolter. He had the valve block in his right hand while trying to connect the hoses from the back side in a confined space. The valve block slipped and caught his right ring finger between the block and drill frame resulting in a laceration requiring seven stitches.
Bates Contractor was loading his drill supplies on the tray of the drill, he was loading glue and struck his right forearm on a 6 inch stack of plates already on the tray resulting in a laceration requiring stitches.
Employees states while unloading drop mast assembly out of the back of maintenance ride, the drop mast got wedged. He reached down to get a bar and the mast slipped catching his right thumb between side of the ride and mast. This is being generated pursuant to the Part 50 audit citation #8209369 issued on 8/4/2015. Management disagrees that a violation of the standard cited exists.
Employee states that he was getting steel from a test hole in the first row and felt pain in his right shoulder going down into his neck.
Bates Contractor: During an attempted retrieval of an immobilized continuous miner, a 2" connecting clevis pin broke away from the two bars it was attached to and the pin passed through the protective wrapping and struck the Bates Contractor in the chest. He was approx. 25' away in the intersection out by.
EE alleges while carrying a toolbox weighing approx. 80-100 lbs he stumbled and fell. He alleges the toolbox landed in his lap. Investigation findings & doctor's reports do not fit the events described by EE. This is being generated pursuant to Cit. #8209357 on 4/2/2015 & management disagrees that a violation of this standard exists. He voluntarily quit 3/19/15
Mechanic was doing a spot check of steering components on a 785 Caterpillar hauler. The mechanic was making his exit from underneath the hauler when the driver turned the steering and caught the mechanic in a pinch point between the hauler tire and the fuel tank of the hauler. Injury resulted in a multiple clavicle fracture with both lungs partially collapsed.
Employee stated that as he moved a loop of continuous miner cable over out of the roadway, he grabbed the cable with his left hand and threw it over on the coal rib at which time he felt pain in his left shoulder and neck area. He continued to work up until a doctor took him off on 3/10/2015.
Employee was bolting on the 003 section and had put up the first row of bolts and had repositioned for the second row. He drilled another hole and was reaching for a bolt when he felt pain in the lower back.
Employee was trying to slide out the forks on a lo-trac. While pulling the fork with his left hand and lifting the front weight with his right, the fork slipped catching his left middle finger behind the fork and slide frame. Resulted in stitches.
Height condition changed from 6.5 feet to 5.5 feet. Employee struck head on top, causing pain in neck and lower back. Employee continued to work until 7/18/2014, when doctor took him off work.
Employee was in the process of installing a beam on the mine roof. The beam was fastened by chain on one side and was being lifted up on one end to fasten it to the top when the beam rolled inadvertently catching his right pinky finger which resulted in a fracture and laceration resulting in stitches.
Employee states he was trying to get finisher piece of steel out of hole. When he pulled, the steel didn't come out but he came off the steel with hands and felt pain in his left shoulder and fingers.
While putting pack of glue into tray on bolter employee pulled or strained something in his lower back. He stated he had pain in his lower back and left leg.
Employee was removing a canopy from a drill that was stuck. The employee was rocking the canopy to loosen it. When the canopy came loose, it slid to the side. He caught the canopy and lowered it to the ground causing pain in his right shoulder. He continued to work up until he had surgery on 7/21/14.
Employee states that he and his helper were taking lid off of diesel scoop. The lid slipped and he tried to catch it smashing his finger between the scoop and the lid. This resulted in 4 stitches to his left index finger.
Employee states while getting bag of dust off the scoop, his hand came in contact with a bow saw causing a cut on his finger resulting stitches.
Employee had raised the cover over reel to check the cable and left it in a raised position. He dropped the remote box for the MRS and when he bent over to pick up the box he placed his right hand on top of the MRS frame. The lid fell catching his right little finger between the frame and lid causing a laceration resulting in stitches.
P&P Contractor began hanging feeder cable when he felt a strain in his left shoulder.
Employee had taken all of his work apparel off and placed it in his basket. He pulled his basket up and shook another basket beside his causing a hammer to fall out striking him on top of the head resulting in a small cut. This is being generated pursuant to the Part 50 audit citation #8209365 issued on 8/4/2015. Management disagrees that a violation of the standard cited exists.
Employee was putting cover back on pump compartment of shuttle car when he felt pain in left shoulder.
Employee was repairing the cable guard on a shuttle car at the canopy. When he was trying to put the post back in the bent bracket, he lost his grip of the post smashing his right middle and ringer fingers against the post and bracket. This resulted in 6 stitches and a fracture to the ring finger on his right hand.
Employee was pulling miner cable and strained his back. He continued to work up until a doctor took him off work on 5/16/2013.
Employee states that he was hanging MRS cable when he felt pain in his lower back, hip, groin and left leg.
Employee was assisting two other employees remove a belt clamp. The belt moved catching his hand between the clamp and belt roller stand. This is being generated pursuant to the Part 50 audit citation #8209368 issued on 8/4/2015. Management disagrees that a violation of the standard cited exists.
Employee was helping another employee clear miner cable for roof drill when he felt pain in his right pelvic region.
Employee was in the process of cutting a single rope hanger down while lifting up on the miner cable. When the cable dropped it jolted him resulting in pain in his back.
Bates Contractor placed a crib block on top of an AL Lee man trip and he bent down to pick up a second crib block. His right shoulder bumped the crib block lying on top of the man trip causing it to fall off and strike his fingers. Resulted in a fracture to his right pinky finger.
Employee was lifting miner cable when his lower back gave out.
Employee was taking miner water line apart. When the line came apart it was twisted and sprung around hitting him on the elbow and forearm. He went to the doctor on 11/16/12 and an x-ray revealed a fracture on the end of the bone (spur broke completely away from the bone).
Bates Contractor was in the process of advancing belt and power when he strained his back while pulling cables or handling materials.
Employee was lifting roof bolt plates onto scoop and felt pain in lower back. He continued to work up until 9/6/2012.
Employee stated while lifting spreader chain to hook to loader bucket, he felt pain in his right elbow.
Employee had mounted hauler by using a front ladder. He was pulling the ladder up with chain and felt a sharp pain in the center of his lower back.
Bates Contractor was hanging bottom roller hanger and mashed finger between hanger and rail resulting in the tip of his finger on his left hand being broken.
Employee was in the process of helping others pull high voltage cable. As he grabbed hold of the cable to pull, the others pulled suddenly which caused him to strike his left little finger against the canopy and between the high voltage cable resulting in a fracture.
Employee was trying to remove a shear pin from a rock duster using a 6' bolt. He struck his small, ring, and middle fingers on augar mashing and causing a laceration resulting in 3 stitches to his middle finger on his right hand.
Employee got out of loader to check trans oil and stated when he got back in the cab and reached out to pull the door shut, he pulled his back.
Employee was setting bits on miner when he turned his head and felt a sharp pain that went down his left shoulder causing his left arm to go numb. He returned to his regular duties until 2/3/2012 when he had surgery on his shoulder.
