Employee was installing the finisher steel in hole. The steel got stuck so EE pulled it out some then tried to shove it in the hole. When the steel got stuck again EE felt a pop in left shoulder and experienced pain. Employee started missing time on 6/24/16.

Employee was helping pull the head shaft out of the feeder. The right side slid and came out sooner than the left side causing the right side to drop suddenly and the left side to rise up. The event pinched EE's wrist between the head shaft and the frame of the feeder resulting in a fracture to right wrist.

Employee was working on ventilation controls when setting pogo and it came dislodged striking him in the mouth breaking his left central incisor. The pogo was not under a heavy load but appeared to have been caught in the line curtain causing it to hang.

Bates Contractor was installing wire mesh. He drilled his hole, put the glue and bolt in the hole and dropped it down to the drill chuck. He stated it didn't line up and was fouled to turn by hand so he bumped the rotation and it twisted his hand around the roof bolt. This resulted in dislocation to the left ring and pinky fingers and a laceration requiring stitches to his pinky.

Employee was installing an ATRS valve package on the section roof bolter. He had the valve block in his right hand while trying to connect the hoses from the back side in a confined space. The valve block slipped and caught his right ring finger between the block and drill frame resulting in a laceration requiring seven stitches.

Mechanic was doing a spot check of steering components on a 785 Caterpillar hauler. The mechanic was making his exit from underneath the hauler when the driver turned the steering and caught the mechanic in a pinch point between the hauler tire and the fuel tank of the hauler. Injury resulted in a multiple clavicle fracture with both lungs partially collapsed.

Employee stated that as he moved a loop of continuous miner cable over out of the roadway, he grabbed the cable with his left hand and threw it over on the coal rib at which time he felt pain in his left shoulder and neck area. He continued to work up until a doctor took him off on 3/10/2015.

The beam was fastened by chain on one side and was being lifted up on one end to fasten it to the top when the beam rolled inadvertently catching his right pinky finger which resulted in a fracture and laceration resulting in stitches.

Employee was removing a canopy from a drill that was stuck. The employee was rocking the canopy to loosen it. When the canopy came loose, it slid to the side. He caught the canopy and lowered it to the ground causing pain in his right shoulder. He continued to work up until he had surgery on 7/21/14.

Employee was taking miner water line apart. When the line came apart it was twisted and sprung around hitting him on the elbow and forearm. He went to the doctor on 11/16/12 and an x-ray revealed a fracture on the end of the bone (spur broke completely away from the bone).
