EE was pulling miner cable & strains EE's back. EE didn't tell anyone until the next morning and EE refused medical treatment that day. EE was then taken to Mountain Instant Care on 4/14/16 for evaluation.

Bates Contractor stated he drilled starter hole and laid his starter steel across & behind the drill head. Installed his finishing steel, proceeded to drill the hole and noticed the starter steel was lying on the drill head. When he grabbed the steel it caught the lip of the canopy jack tubing mashing his fore finger against the scissor bar resulting in a laceration and 3 stitches.
Bates Contractor was tramming miner toward face #7 when his right ankle got caught in a loop of miner cable resulting in a sprained ankle.

Bates Contractor: During an attempted retrieval of an immobilized continuous miner, a 2"" connecting clevis pin broke away from the two bars it was attached to and the pin passed through the protective wrapping and struck the Bates Contractor in the chest. He was approx. 25' away in the intersection out by.

Employee stated that as he moved a loop of continuous miner cable over out of the roadway, he grabbed the cable with his left hand and threw it over on the coal rib at which time he felt pain in his left shoulder and neck area. He continued to work up until a doctor took him off on 3/10/2015.

Bates Contractor disengaged the T-bar from the mine roof, when a piece of draw rock dislodged from between the rib and the outside bolt striking him on the left ankle. The contractor (5UF) submitted a 7000-1 on 10/27/14, however it should have been submitted under this MSHA ID.

While trying to exit the lo-trac, the employee stood up raising one leg up to step out when the opposite foot slipped causing him to fall on the handle towards the fork side of the lo-trac bruising his ribs and straining his shoulder. While making a splice on a shuttle car cable, he was separating the strands with a utility knife when the blade slipped out of the insulation causing it to make a laceration across his left thumb. This resulted in 3 stitches.
Employee was walking off section when he came across a water hole. Not realizing there was a drop off in the water hole of at least 12"", he proceeded causing him to trip and twist resulting in a lower back strain. Doctor took him off work on 8/18/14.

While installing a roof bolt, a piece of drill steel fell off the tray. The employee went to catch the steel with his left hand while still pushing up the bolt and caught his left thumb between the boom, the tray and drill steel resulting in stitches and a fracture to his left thumb.

Employee swung off side drill station out to put up first bolt. A rock fell and struck him on the right leg from the knee down resulting in a bruise.
Employee states that while operating the Joy miner, he had a utility knife on his belt. While he was backing the miner out of the place, he knelt down to continue moving the miner back. When he knelt, the remote box set on the knife causing it to cut his right upper thigh resulting in 5 stitches.

When he was trying to put the post back in the bent bracket, he lost his grip of the post smashing his right middle and ringer fingers against the post and bracket. This resulted in 6 stitches and a fracture to the ring finger on his right hand.

Employee was sitting on a plaster bucket on the in by end of the #4 entry block on the corner writing notes when a section of the coal rib dislodged without warning, sliding down the face of the rib and striking him in the back and knocking him to the mine floor. The rock measured 63"" long x 25"" wide and 17"" thick.
Employee stated while lifting spreader chain to hook to loader bucket, he felt pain in his right elbow.
Employee was helping pull miner cable and tripped and twisted his right knee.

Employee was moving a shuttle car cable out of the way when he received an electrical shock which resulted in a burn spot on the palm of his left hand.
Employee was working on a shuttle car cable on the left side of the feeder as he was going to the power center to lock out the cat head he hit the back of his neck on a belt hanger. This resulted in 6 stitches to the back of his neck.
Employee was stepping over shuttle car cable when he was tripped by the cable causing him to land on his back when he hit the ground. Employee was scheduled off on the 19th and was unable to get to work on the 20th because of snow. He missed work on the 21st and was scheduled off on the 22nd and 23rd. He returned to work on the 24th.

Employee was using a lifting jack to raise and level a belt take up. A slate bar was being used to lift take up. As he was using the jack the employee caught his finger between the jack and belt structure over head resulting in a fracture.
Employee was taking the bolts out of a 4"" water line T-coupling. The line was hanging when he loosened the bolts and hit the coupling knocking it loose. When he did this the water line came apart at the joint striking him in the face.

Employee leaned over the side of the shuttle car to look for holes in the bottom when he struck hole. His right side ribs hit the side of the shuttle car deck causing fractured Rib.
Employee was reaching for a roof bolt plate on drill, when it went between glove and arm resulting in cut to right forearm, requiring stitches.

Employee was struck on the shoulders by a piece of rib rock. It pushed him into the miner causing a bruise. He was initially released to full duty, but on 8-7-09 began to lose time.
An unintentional, non-injury, roof fall was discovered in the #7 Entry of the right return aircourse of 001/002 section. The roof fall was 20 feet X 8 to 9 feet X approximately 50 feet. Ventilation was not interrupted.
While tramming a shuttle car, employee hit a hole in the haulway, causing him to hit his head on the canopy. Started losing time on 07/07/09. Employee was released to full duty until that day.
Employee was pulling a tool box off the forks of a Lo-Trac onto a crib block, when he felt a pain in his back.
Employee was operating a shuttle car when he said that he hit a hole causing him to hit his hard hat on the canopy. He continued to perform his regular duties until he was taken off work by a physcian on 8-4-2009.

Non-injury, non-intentional roof fall occurred on the 4d Panel 130' inby SS 3068 in the #2 entry. Fall measured 7' -8' high, 18' wide, and 12' long. Area is dangered off. Ventilation not affected.
While hanging a pinner cable with a rope hanger, employee's finger became caught in the rope hanger pulling finger, causing a fracture to his left middle finger.(Misc Mail 10/29/08 #9-Revisied 7001 submittal to include 5 restriced days and correct job title as mine trainee)
A non- injury roof fall was discovered in the bleeder 45' inby SS #2557 on Panel 3A on the 003 MMU. Fall measured 30' in length, 18' wide, 5' - 6' high. The type of roof support installed in the area was 4' resin bolts, and 8' cable bolts. No equipment was involved. Ventilation was not affected.
Scoop was attempting to push drill up a hill in 6 Rt. when the bucket slipped over the bumper striking the water box which struck the operator in the back of the head and neck causing injury.

As employee was making up bolts, his hand struck another plate and bolt that was laying on the mine floor cutting his glove and his finger that required stitches.
Employee was moving ventilation curtain when he struck a roof bolt plate with his right forearm, causing a laceration that required stitches.
Employee was removing a jack from the belt tailpiece, when his left hand was caught between the jack and tailpiece, causing a laceration to the back of his left hand that required stitches.
Employee stated his right hand was caught between roof bolt plate and drill canopy, when he accidently hit the swing jack on roof bolter, causing a laceration and fracture to his index finger on his right hand.
AS EMPLOYEE PUT TOOLS IN TO A TOOLBOX, THE LID OF THE TOOLBOX CAME DOWN ON HIS RIGHT HAND, CAUSING A FRACTURE AND LACERATION TO HIS MIDDLE FINGER.
Employee felt pain in his lower back as he was loading supplies. He continued to work through 04/27/06 at which time the Doctor removed him from work.
There is no evidence verifying this is reportable. This report is submitted because of citation was issued. EE was opeating shuttle car with limited visibility and uneven floor when another S/C operator struck the EE causing small red spot on his arm Refused to go to Dr. worked shift. EE went to Dr.one month later. MRI showed no problem with elbow but had a shoulder spur.
