The equipment was locked out and electrical energy was isolated. During routine V-belt alignment and tensioning, the employee manually repositioned the belt. EE's right pinky finger entered the pinch-point area between the belt and pulley, resulting in injury.
At Tower 11 the employee was cleaning a jammed magnet separator using a high-pressure water hose. Upon activating the hose, the sudden pressure caused it to slip from their hand. The metal fittings on the nozzle struck and cut their hand.
Employee was assisting in setting a new back leg lug when some debris fell from the top getting into the right eye. Rust ring was removed from the right eye. Employee was released to full duty.
On 10/30/2024, EE was working with three team members to load a thermocouple for kiln shelf installation. While positioning the thermocouple inside the Preheater elevator, EE experienced pain in the lower back and waist. The employee initially declined medical treatment but sought care on 11/5/2024, when EE was prescribed medication, physical therapy, and imposed work restrictions.
EMPLOYEE WAS WORKING ON DRAG SYSTEM K446. EE WAS PULLING OUT THE CHAIN THAT WAS WRAPPED AROUND THE SPROCKET. WHILE SETTING DOWN THE CHAIN, EE'S FINGER GOT PINCHED BETWEEN THE CHAIN AND THE BOTTOM LINK.
Employee was opening the silo 8 cleaning door located at the top of the silo when the door slipped causing EE's right finger to get caught between the door and the structural beam. EE had a minor cut on the index finger. Employee received prescribed medication.
The employee was standing 15-20 feet away from the cleaning port located on the kiln seal inlet pipe on the second floor of the preheater tower when there was a flush and a stream of hot material ejected from the port and made contact with the employee's face and wrists. The employee received medical treatment and restrictions to work
Unexpectedly, the material fell from the west side of the TA duct, splashed on the ground and spread widely, creating a dust cloud covering the barricaded area and reaching the worker standing outside the barricade. The worker was extending the barricaded area. The employee suffered from corneal abrasion and received prescribed medication and work restrictions.

Employee was in a boat, pulling on electrical cable that was stuck in the mud. Electrical cable is utilized to power dredge. While tugging on the cable employee strained their back.
The employee was preparing to do a job in the main baghouse when the material exited the inspection door that was open above. The material got into employee's eyes
Operator had climbed into the seat of the loader and went to shut the door. As EE pulled hard on the handle, EE experienced a sharp pain from the left elbow to the hand. Operator was swapping loaders with another employee.
The employee adopted an uncomfortable position switching the valve to silo-13 due to a scaffold located in front of the valve and hurt lower back
hot material exited and landed on the employee left boot, the material burned through the left side of EE's boot, burning EE's left feet
During the course of removing a net from the preheat tower, the employee strained EE's back and then went to the doctor
Employee was measuring the level in the cement silos. When EE opened the latch on the silo measuring port, dust came out and got into EE's eyes.
Individual was changing v belts on a drive motor when their fingers became caught between the V belts and the pulley causing lacerations to the thumb and middle two fingers
Individual went to the south side of the tower to get the water lance and hose. Employee turned on valve, when they turned around they tripped on the hose and fell down. Shoulder hit the ground. This was reported as first aid case, they went to the Dr. on 10-2-18 and it became a reportable injury on that date.
Delivery driver making a fly ash delivery to the cement plant, washing out the bed of the truck. The water hose hit ee in the leg, breaking the leg. Degree 9, not a Titan employee. This was a delivery driver.
employee was replacing guard on tail pulley on Belt F606 when EE lost EE's grip and felt cutting to EE's lower left arm
Routine measurement of measuring #5 silo. Employee opened port and moved to the side while inserting the tape measure into silo. Silo puffed out cement and covered employee with cement dust affecting both eyes
Drive belts on the secondary crusher twisted. In an effort to straighten out the belts, employee caught right pinky finger between drive belts and sheave.
Employee was fixing the seal of an inspection door on the kiln dust bin air slide. The system was down and after sealing it and closed the door, the latch popped open causing dust come out and cover the employee face, including getting access to EE's eyes.
Employee was cleaning the area of the Stage #4 duct. When EE hit the beam nearby, accumulated dust get into EE's eyes. The employee immediately washed eyes with water. EE was using the standard PPE.
While loading 3/4" cable into a work boat, the miner lifted several bound wraps of cable over the handrail and let go. When the loose end of the cable went over the rail, it whipped up and struck the miner in the right elbow. This resulted in a contusion on the elbow and tingling in the right hand. The miner returned to work, but with medical restrictions on right arm.
Employee was getting down a vertical ladder, when EE felt pain in EE's left shoulder and part of EE's left back.
During the process of cleaning a valve, the employee was in an awkward position, EE make a sharp turn and strained EE's neck.
EE was inspecting the bolts condition of a limestone feeder conveyor. EE tripped with the electrical cord when EE tried to prevent falling, when EE tried to pick up an electrical light, & put left hand inside the dolly rail, EE pulled the stop emergency cord but one dolly pinched lt index finger, dislocating it. EE has NOT missed any days from work but is working on restrictions.
While washing down the plant, the operator had water spray come under the side of safety glasses. Operator wiped sandy water from eye, but a grain of sand got into left eye, causing an abrasion. Employee was taken to clinic where sand grain was removed. Antibiotic drops were prescribed and operator was released back to work with no restrictions.
Strained hip trying to move a portable lighting tower to stabilize it. Worker decided to seek medical attention on 1/25/2016.
Two mechanics were installing the kiln door seals, employee placed his hand on an angular bracket to push the seal forward, when the seal was set high it came back and hit his finger
FROM EMPLOYEE STATEMENT On 6/8/15, when loading an AC unit over the base at ER 13, I felt something stretching under the abdomen, but I did not give importance. It started swelling a bit, but then disappeared. Second time was at main office moving one table on 6/13/15 and I felt a little pain but nothing else. Employee later had surgery 10/12/2015
Employee strained his lower back while changing grease drums on lube truck. Incident was reported the following day.
Employee was cleaning one of the ports at the pre-heater tower feed hood with a water blasting system. He shot water into the port, and that's when hot vapors and dust surged back and into employee's fire protecting hood. Employee sustained burns to his scalp and left ear.
A group of employees were working on replacing a cable at the dragline, as they placed the cable through a socket, the cable slipped out of employee's hand and struck him on the nose.
Employee (security guard) stated he injured his back while trying to help mobile garage employee move forklift forks. This case was questionable and under investigation.

Employee was removing wooden spool from stand. Upon removing the center pipe, spool reel fell and cut the tip of his finger.

Employee was relocating elevator buckets by placing them into pallets when one of the buckets from another pallet fell over, punctured glove and caught his hand. Employee suffered a laceration to his left hand.
While pulling 54" steel cable conveyor belt on conv#Y34 - approx. 200' belt had been secured for pulling with 6" C-Clamps. EE was standing at the approx 230' from roll end where c-clamps were connected to roll. Both clamps released and flew the 230' to strike him on both the right and left upper thigh. He sustained bruising and abrasions on both thighs.
Employee was replacing the idlers on the conveyor belt when he pinched his finger between the idler assembly and conveyor frame.
Employee injured his right knee while attempting to catch chipboards falling off pallet.
