Tech was squatting down to install a drain plug on a wheel loader. As they stood up, pivoting their feet and their left became caught behind right foot. Tech then rolled their right ankle and fell down. Tech sustained a fractured tibia.

The employee was exiting the trailer when they fell and broke their left leg.

The employee was holding the shaft with the crane. When EE raised the hoist, it moved faster than anticipated, causing the synthetic lifting strap to fail. This caused the hook block to jolt and become unattached to the rope cable, then falling to the floor. The hook block then struck the sync motor and then struck the employee's right middle and right ring finger.

Employee was assisting in setting a new back leg lug when some debris fell from the top getting into the right eye. Rust ring was removed from the right eye. Employee was released to full duty.

Associate was assisting in track maintenance near the switch stand, when the backhoe pressed against the switch point, causing the switch handle to swing in the opposite direction striking the associate in the side of EE's face and ear.

Loading chemicals into the truck when a hose fitting broke, sprayed on back and shoulder.

A balloon got caught on the overhead power lines on the south side of belt Y34 and the east side of the turnpike road creating an arc that caused vegetation to catch on fire

Employee was driving a 50 ton, off-road dump truck. Employee claims they became distracted causing them to lose control and steer the truck off the haul road. This caused the truck to roll over.

WORKING ON THE GEAR BOX AND THE SHAFT THAT COMES OUT OF THE GEAR BOX SLIPPED, PINCHING THE LEFT PINKY FINGER TIP OFF.

Employee was getting out of vehicle when EE's hand slipped from the shuttle van door's window pane. EE's finger caught on the door's handle when it slipped

Tech using 2 different size jack stands to support the bucket of a WA-900 and did not relieve the pressure from the hydraulic line. When they broke the control valve loose it caused the bucket shift and fall off of the jack stands. When the bucket fell the dog bone linkage swung down and hit the tech just below the knees. Tech sustained injury to both legs.

Individual was changing v belts on a drive motor when their fingers became caught between the V belts and the pulley causing lacerations to the thumb and middle two fingers

When lifting a bucket of material sample, of around 35 lbs., to put it into the back of a truck, EE felt a pain in right knee and cannot keep balance and fell.

Employee was turning on a water pressure hose when the hose valve flew and struck employee on the face, causing a laceration requiring 5 stitches in the lower lip and two broken teeth.

Employee was welding when a piece of slag burned through pant leg and fell down into boot causing a burn to right foot. Employee was treated with first aid on site until 1/4/16 when they requested medical treatment.

While taking lab samples with a shovel, the employee felt a pain in EE's right knee that prevent EE to supported EE's weight. NOTE: EE has NOT lost any days but is working on restricted activity.

Employee was replacing a dragline hoist cable with a co-worker. While removing the hoist socket with a sledge hammer, the hammer chipped and the piece struck the employee close to the left eyebrow.

Employee strained his lower back while changing grease drums on lube truck. Incident was reported the following day.

A group of employees were working on replacing a cable at the dragline, as they placed the cable through a socket, the cable slipped out of employee's hand and struck him on the nose.

Employee (security guard) stated he injured his back while trying to help mobile garage employee move forklift forks. This case was questionable and under investigation.

Employee was un-clogging chute with pole and struck the top of his eye (eyebrow)with the pole.

Employee stated he was climbing down the dragline ladder and missed the last step, which caused him to bend his knee too far in aggravating a previous knee injury. Pain did not go away and employee sought medical treatment two days later.

Employee chipped his front tooth when a transmission cap slipped from has hand and struck his mouth while servicing plant equipment. Incident was reported late and was under investigation until 04/30/2013. Employee received initial treatment on 04/30/2013.

Employee struck his right hand pinky finger while disposing of old cable floaters.

Employee slipped from the third step of a ladder and injured his left arm/shoulder.

Employee was in the process of placing a comealong when he pushed a piece of conveyor and it cut his finger.

Employee slipped and fell on his left arm while descending off his service truck after completing refueling the air compressor.

Employee was changing rollers and strained his back while lifting one.

Employee was cutting conveyor belt and felt a sharp pain on his left forearm. The following day he sought medical treatment and was placed on restricted duty for strain on left forearm.

Employee was repairing an air conditioning unit when he caught his finger on the fan, resulting in a finger laceration.

Employee was welding overhead when a spark fell on his left shoulder towards the back of his shirt. He stopped welding and noticed that his shirt had caught fire, and sensed a burning sensation.

Employee was searching for a hose in the storage container located in the load-out department when he sustained insect bites on his hands and feet. As a result, the insect bites aggravated a few days later and his foot became swollen.

Employee was in the process of loading some screens onto a shelf when they slipped, and the edge of the screen cut his wrist.

Employee was commuting from primary crusher to plant when he suffered an asthma attack, lost control of his vehicle, and collided with an electrical pole located in median of the the access road. As result, he sustained a contusion in chest wall and closed fracture of rib.

Employee stated that he was removing rigging from punch plates while they were being settled on the shaker platform. In the process, the punch plates slipped, pushing and slighty pinning him between the plates and shaker.

Employee was installing a screen with a pry bar when it slipped off his hands, knocked off his safety glasses and struck his eye, resulting in an eye contusion.

Employee stated that he sprained his knee while climbing off a truck ladder.

Carrying cardboard tubes in windy conditions. Propped on shoulder and felt pain in shoulder.

Employee stated that he was replacing a spring base on a shaker when he struck a bolt with a sledge hammer and a piece of steel struck his left wrist.

Employee stated that he was loading material into a loader bucket, when the operator moved it without notice and struck his left middle finger.

The employee experienced a recordable shift in hearing.

Employee stated that he was using a hammer and chisel to break open a chain when a piece of steel flew out and lodged in his leg.

Employee stated that while he was climbing up on the water truck his right knee hit the bumper.

Employee was welding and a piece of welding slag fell into his ear.

Employee lost his footing and fell from the same level, landing on some steel, resulting in a Contusion/Abrasion to the knee.

Employee stated that he was straddling a pipe to put fasteners in and as he leaned over to align it, the pipe sprung up and struck him in the face.

Employee stated that he was pulling on a water hose and struck his head on an overhead beam. Incident reported on 10/10/06.

Employee is currently unavailable for statement. Apparently, employee's foot got pinched between the knuckles of two rail cars as they were being pushed together to be coupled.

Employee stated that he slipped on some grease and twisted his back while trying to avoid falling to the ground.

Employee stated that he twisted his upper back while cleaning a railcar and track (using hoe - hand tool).

EE stated that a piece of metal fell on his left hand while he was cutting a pipe using a cutting torch (laceration).

Employee stated that he was cleaning the trial scale when water from the high pressure water hose struck his left eye.

Employee stated that he was cleaning the chute on conveyor Y52 with a high pressure water hose. The material on the chute came loose and struck him, knocking him down to the ground.

Employee stated that his right index finger was crushed by the crane block sheaves while attempting to untangle a small chain.

Haul truck operator was coming down the ramp from the primary crusher. It appears that he lost control of the vehicle and as a result, flipped over.

Employee was working on a conveyor structure when he fell through a gap in the catwalk. He fell 36' to the ground below.

Employee stated that he was trying to align a pump coupling while holding the pump with his left hand and the coupling with his right hand. The pump slipped and the coupling lacerated his right index finger.

Employee stated that he was carrying an Acetylene bottle on the second floor of the sand plant. While holding the bottle by it's end cap, the cap slipped, crushing his right thumb on the handrail.

Employee stated that he was cutting a piece of wiper material in the sand plant MCC. As he finished, his right arm hit his right leg, causing the box cutter blade to slightly cut his left thigh.

While attempting to adjust the wiper on the Y49 conveyor (the belt was not moving), employee cut/smashed the end of his left pinky finger.

Employee sprained/strained lower left leg (calf area) while pulling himself up a ladder located on a rail car.

While headed to Pit-G, employee (Truck #70) collided with another employee (Truck #10).

While headed to the primary crusher, employee (Truck #10) collided with another employee (Truck #70).

Employee stated while cleaning the yard area and shoveling dirt, he felt pain in his right leg and lower back. This incident was reported to the Safety Department on 6/24/04 and is questionable as to whether it really occurred at the workplace.

While walking to the Cement scale, employee slipped and twisted her knee.

According to most recent audiogram, ee has suffered recordable hearing loss in both ears. This ee may be exposed to noise above 85 decibles (A-scale) for an 8-hr time weighted average while on the job. Hearing conservation training & hearing protection has been provided to ee upon hire & annually thereafter. In addition, ee's supervisor has been trained in hearing conservation.

According to most recent audiogram ee has suffered a recordable hearing loss in both ears. WHILE REMOVING A 6"" BEARING USING A 12"" X 6"" BOLT AND HAMMER, EE MISSED THE BOLT AND HIT HIS LEFT HAND. EE REPORTED THIS ON 3/15/04 AND RECEIVED MEDICAL TREATMENT ON 3/16/04.

EMPLOYEE STATED THAT WHILE CLIMBING DOWN THE LADDER OF A HAUL TRUCK, HE FELT PAIN IN HIS BACK. IT WAS A CLEAR, SUNNY DAY. (EMPLOYEE REPORTED ACCIDENT ON 2-9-04)

WHILE TAKING OUT A BOOM CYLINDER PIN, EE FELL BACKWARD ONTO THE GROUND. RAINY DAY, FLOOR WET, & WINDY.

Employee stated that he was removing a conveyor roller from the C10 conveyor. As he began to lift the frame, his left hand struck the lateral which was very sharp, cutting through his glove and lacerating his left middle finger.

WHILE MOVING A 55 GALLON EMPTY DRUM, EMPLOYEE RECEIVED A SHARP PAIN IN HIS LOWER BACK.

WHILE TURNING AROUND, EE'S LEFT FOOT GOT STUCK ON A ROLLER HE PLACED ON THE FLOOR, CAUSING HIM TO TRIP & FALL. HIS RT.THUMB HIT AN ANGLE IRON AS HE FELL FORWARD.

EE WAS TIGHTENING A BOLT UNDER TRUCK #69-2271 WITH A WRENCH. THE WRENCH SLIPPED AND LANDED ON HIS FOREHEAD. EE RECEIVED STITCHES.

EE WAS CLEANING A BEARING USING AN AIR NOZZLE. A FOREIGN OBJECT BLEW BACK AND HIT THE RIGHT EYE.

SUNNY DAY, RAMP IN GOOD CONDITION. WHILE EE PROCEEDED UP RAMP, THE LEFT STEERING CYLINDER TIE ROD BROKE, PULLING THE TRUCK TO THE LEFT. HE THEN TRIED STEERING TOWARDS THE RIGHT BUT THE TRUCK DIDN'T RESPOND AND ROLLED OVER.

THE AREA WAS CLEAR OF ALL MATERIAL AND FREE FROM OBSTRUCTIONS, TRIP HAZARDS AND TOOLS. EE WAS CLOSING A RAILCAR DOOR. A RAILROAD BAR IS NECESSARY FOR THIS TASK. THE BAR SLIPPED FROM THE DOOR L ATCH CAUSING HIM TO LOSE HIS BALANCE, TWISTING HIS RIGHT ANKLE.

GROUND AREA WET DUE TO AFTERNOON RAIN SHOWER, BUT THERE WAS NO RAIN DURING TIME OF INCIDENT. EE WAS EXITING THE CAB OF A FUEL TRUCK, HIS LEFT FOOT GOT CAUGHT INT HE STEP OF THE CAB AND TWISTED AS HE LOWERED HIMSELF TO THE GROUND.

CLEAN AREA; EMPLOYEE WAS HELPING REPAIR/REPLACE A ROLLER. WHILE ATTEMPTING TO LIFT IT HE FELT A CRACK IN HIS BACK.

AFTER SERVICING EQUIPMENT, EMPLOYEE WAS ON HIS WAY DOWN WHEN HE MISSED THE LAST STEP AND FELL.

WHILE GETTING READY TO COME DOWN FROM A DUMP TRUCK BED, EMPLOYEE STEPPED ON SOME CONCRETE BLOCKS BEING USED AS STEPS. THE BLOCKS BROKE AND HE FELL BACKWARDS STRIKING HIS LOWER BACK.

WHILE EE WAS DOING HIS JOB CLIMBING AND KNEELING TO SERVICE EQUIPMENTS, HE STARTED FEELING SOME PAINS IN THE INSIDE OF HIS LEFT KNEE.

NIGHT, UNEVEN FLOOR, SOME MUD. EE WAS CHECKING LOCKS ON EMPTY HOPPERS AND STRIKING THEM WITH A SLEDGEHAMMER WHILE THE RAILCARS PASSED BY. AS HE SWUNG THE HAMMER, HE MISSED THE LOCK & TWISTED HIS BACK.

DRY AREA, HAMMER IN GOOD CONDITION, CLEAN CONVEYOR BELT. WHILE EE USED A HAMMER, IT BROKE TO PIECES AND LACERATED HIS ARM. A PIECE IMBEDDED HIS ARM.

CLEAR, SUNNY DAY. EE STATED HE WAS WORKING AT TOWER 80 WHEN HE LIFTED A HEAD PULLEY COVER AND FELT SOMETHING IN HIS LOWER BACK.

CONDITIONS WERE FINE; AREA WAS CLEAN; DAY WAS CLEAR; NO BAD WEATHER. EMPLOYEE WAS DESCENDING STAIRS WHEN HE STEPPED ON A ROCK AND TWISTED HIS RIGHT ANKLE.

CLEAR & SUNNY; NO UNUSUAL CONDITIONS. EMPLOYEE WAS USING A WRENCH TO INSTALL A RADIATOR WHEN IT SLIPPED AND JAMMED HIS RIGHT RING FINGER.

ENVIRONMENTAL CONDITIONS NOT A FACTOR. EE WENT TO PAINT THE TOP SIDE OF DRAG MOTOR NUMBER TWO; HE CLIMBED UP PUTTING ONE FOOT ON COMPRESSOR AND THE OTHER ON THE MOTOR BASE. EE LOST HIS BALANCE AND FELL FROM A HEIGHT OF 2 1/2 - 3 FEET. HE WAS WORKING WHILE MACHINE WAS OPERATING.

EMPLOYEE PULLED HIMSELF UP ONTO THE RAIL CAR AND FELT A POP ON HIS LEFT ELBOW.

GOOD COOL TEMPERATURE. EE WAS CUTTING THE DUMP CABLE WITH A BLOW TORCH WHEN THE CABLE SWUNG AND STRUCK HIM IN HIS RIGHT HAND.

DRY, CLEAN, DAY LIGHT. EMPLOYEE WAS PICKING UP PIECES OF METAL AND PLACING THEM INSIDE A LOADER BUCKET TO BE THROWN AWAY. WHILE PICKING UP A PIECE OF METAL, ANOTHER PIECE ON TOP OF IT SLID AND TOOK OFF HIS LEFT HAND MIDDLE FINGERNAIL.

BRIGHT SUNNY DAY, EE WAS USING A SLEDGE HAMMER TO REMOVE A PAIN MIST CAUSING HIM TO PULL HIS RIGHT SHOULDER.

GOOD WEATHER MUDDY ROAD CONDITIONS, LIGHTING WAS FAIR EQUIP CONDITION GOOD. EE WAS PUSHING INTO THE FEEDER AND THE CONTROL TOWER OPERATOR MOVED THE TRIPPER CAUSING ROCKS TO FALL ON TOP OF THE DOZER AND WHILE OPERATOR ESCAPED HE HURT HIS RIGHT KNEE.

BRIGHT, WARM, SUNNY. EMPLOYEE WAS ADDING OIL TO THE LOADER AND SOMETHING FELL INTO HIS LEFT EYE.

MUDDY AREA, NIGHT DARK AREA, PRY BAR CAME LOOSE FROM WHERE THE EMPLOYEE PLACED IT CAUSING HIM TOFALL BACKWARDS.

NORMAL, STICKY MATERIAL, EMPLOYEE WENT TO PUSH IN THE FEEDER. IT CAME DOWN AND DOZER WENT INSIDE CAUSING HIM TO CUT HIS RIGHT ELBOW, BRUISE THE RIGHT SIDE OF HIS FACE AND BREAK A TOOTH.

NIGHT, PLENTY OF LIGHT, SPOTLIGHTS. WHILE ENTERING TEMPORARY TRAILER STAIRS, EE MOVED OR SHIFTED & FELL DOWN.

OVERCAST DAY, SAND & ROCKS AROUND THE FUEL TRUCK, EE BENT HIS KNEES AS HE PULLED ON THE HOSE WHEN HE FELT PAIN.

THE MATERIAL LOADED IN THE BACK OF THE TRUCK WAS DONE POORLY AND WAS VERY DIFFICULT TO WORK WITH.

THE EMPLOYEE WAS GETTING INTO HIS SERVICE TRUCK AND PULLED A MUSCLE IN HIS BACK.

GOOD CONDITIONS, WHILE EE WAS GETTING A KNIFE TO CUT THE COVER OFF AN ELECTRICAL CABLE, THE KNIFE SLIPPED AND CUT HIS FINGER.

THE EMPLOYEE WAS CLEANING AROUND A CONVEYOR WITH A WATER HOSE. HE ATTEMPTED TO MOVE A PIECE OF CONCRETE WITH HIS HANDS AND FELT A SHARP PAIN IN HIS LOWER BACK AND SIDE.

SUNNY, DRY, AREA WAS A BIT MUDDY, WINDY. EE BELIEVES THAT WHILE HE BENT DOWN TO REACH FOR SOME BOLTS, AIR FROM AN AIR GUN BLEW SOMETHING INTO HIS EYE.

EMPLOYEE WAS CUTTING METAL WITH A TORCH. A PIECE OF METAL FELL DOWN AND INTO BOOT, AGAINST HIS RIGHT ANKLE. THE RUBBING OF THE BOOT AGAINST IT MADE IT WORSE.

COVERED ROOF STRUCTURE WITH NO SIDES WHERE WELDING, CUTTING, AND GRINDING IS PERFORMED. DAY TIME, BRIGHT SUNNY DAY. EE FELT SOMETHING ENTER HIS EYE WHEN WALKING ACROSS THE WELDING SHOP.

EMPLOYEE WAS PERFORMING HIS DAILY CLEANING. WHILE PULLING A HEAVY HOSE HE FELT A SUDDEN SHARP PAIN IN HIS RIGHT SHOULDER.

CONDITIONS NORMAL TO SITE-NO WATER, MUD, GREASE. EE WAS REPLACING AN ELECTRIC MOTOR USING TWO COMEALONGS TO PUT MOTOR ON A BASE, MOTOR SLIPPED OFF TWO INCHES ABOBE THE BASE.

WHILE LIFTING A REINFORCEMENT PIECE ON THE BED IT SLIPPED. EE CARRIED IT TO THE FLOOR SO IT WOULD NOT HIT HIM AND HE HURT HIS BACK WHILE PUTTING IT DOWN.

EE WAS WORKING ON FIXING LIGHTS AND LIFTED A HEAVY TOOL BOX & WHEN HE LIFTED THE BOX UP HE FELT A SHARP PAIN IN HIS LEFT BACK SIDE.

MECHANICAL, EE WAS TIGHTENING BOLT ON SPRING BASE OF Y-12 FEEDER. THE SWIVEL ON THE AIR WRENCH CAME LOOSE AND STRUCK HIS LEFT INDEX FINGER.

RAINY; WHILE TRYING TO CENTER HOLE WITH PINCH BAR, EMPLOYEE SLIPPED OFF THE SPRING BASE AND HIT HIS LOWER BACK WITH THE FLOOD GATE HANDLE.

EE WAS CLOSING A HOPPER DOOR WITH A PRYBAR WHEN HE FELT A SHARP PAIN ON HIS LOWER RIGHT SIDE BACK AREA.

EE WAS CLEANING TOP OF THE TRUCK WITH PRESSURE WASHER NOZZLE AND LOST HIS BALANCE CAUSING HIM TO FALL ON TOP OF GREASE PUMP AND THEN TO THE FLOOR HURTING HIS RIGHT RIB CAGE.

EMPLOYEE WAS GOING UP THE STAIRS TO 620 SAND SCREW WITH TWO OTHER PLASTIC PANELS APPROX 20 LBS, AS HE CONTINUED TO INSTALL THE PANELS HE FELT PAIN IN HIS LOWER BACK AND LEG AREA.

EE TRIPPED OVER AN OLD ELECTRIC PIPE THAT WAS ON THE FLOOR.

CLEANING BROKEN CONVEYOR WITH HOSE WHEN DIRT SPLASHED INTO HIS EYES AND MOUTH.

PIECE OF MATERIAL FELL INSIDE HIS BOOTS WHILE HEWAS WELDING.

MOVING STEEL BEAM AND CAUGHT INDEX FINGER.

EE WAS COMING DOWN FROM C-1 TO Y-62 ON LADDE3R WHEN HE SLIPPED & FELL HITTING HIS LEFT ARM

WAS WORKING UNDER HAUL TRUCK AND HIT HIS HEAD ON TRUCK FRAME.
