While employee was driving the haul truck, it appears he lost control of the vehicle and went over a 7 - 9 foot high berm into the lake that was approximately 70 - 80 feet deep.

Tech was squatting down to install a drain plug on a wheel loader. As they stood up, pivoting their feet and their left became caught behind right foot. Tech then rolled their right ankle and fell down. Tech sustained a fractured tibia.

The employee was exiting the trailer when they fell and broke their left leg.

The employee was holding the shaft with the crane. When EE raised the hoist, it moved faster than anticipated, causing the synthetic lifting strap to fail. This caused the hook block to jolt and become unattached to the rope cable, then falling to the floor. The hook block then struck the sync motor and then struck the employee's right middle and right ring finger.

Employee was assisting in setting a new back leg lug when some debris fell from the top getting into the right eye. Rust ring was removed from the right eye. Employee was released to full duty.

Employee was driving a 50 ton, off-road dump truck. Employee claims they became distracted causing them to lose control and steer the truck off the haul road. This caused the truck to roll over.

Tech using 2 different size jack stands to support the bucket of a WA-900 and did not relieve the pressure from the hydraulic line. When they broke the control valve loose it caused the bucket shift and fall off of the jack stands. When the bucket fell the dog bone linkage swung down and hit the tech just below the knees. Tech sustained injury to both legs.

When lifting a bucket of material sample, of around 35 lbs., to put it into the back of a truck, EE felt a pain in right knee and cannot keep balance and fell.

Employee was turning on a water pressure hose when the hose valve flew and struck employee on the face, causing a laceration requiring 5 stitches in the lower lip and two broken teeth.

Employee was welding when a piece of slag burned through pant leg and fell down into boot causing a burn to right foot. Employee was treated with first aid on site until 1/4/16 when they requested medical treatment.

While taking lab samples with a shovel, the employee felt a pain in EE's right knee that prevent EE to supported EE's weight. NOTE: EE has NOT lost any days but is working on restricted activity.

Employee was replacing a dragline hoist cable with a co-worker. While removing the hoist socket with a sledge hammer, the hammer chipped and the piece struck the employee close to the left eyebrow.

Employee was searching for a hose in the storage container located in the load-out department when he sustained insect bites on his hands and feet. As a result, the insect bites aggravated a few days later and his foot became swollen.

Employee was commuting from primary crusher to plant when he suffered an asthma attack, lost control of his vehicle, and collided with an electrical pole located in median of the the access road. As result, he sustained a contusion in chest wall and closed fracture of rib.
