At Tower 11 the employee was cleaning a jammed magnet separator using a high-pressure water hose. Upon activating the hose, the sudden pressure caused it to slip from their hand. The metal fittings on the nozzle struck and cut their hand.
The employee was operating a manlift and changing out a return roller. When EE turned the basket, EE caught EE's fingers between the rail of the manlift and conveyor structure.
While walking to the S100 hopper to open the guillotine gate, the employee slipped and fell slightly backward, injuring their right ring finger. Employee was treated at urgent care, where X-rays confirmed a dislocation and minor laceration. Medical treatment was provided, and work restrictions were prescribed as of 05/11/2025

On 10/30/2024, EE was working with three team members to load a thermocouple for kiln shelf installation. While positioning the thermocouple inside the Preheater elevator, EE experienced pain in the lower back and waist. The employee initially declined medical treatment but sought care on 11/5/2024, when EE was prescribed medication, physical therapy, and imposed work restrictions.
EMPLOYEE WAS WORKING ON DRAG SYSTEM K446. EE WAS PULLING OUT THE CHAIN THAT WAS WRAPPED AROUND THE SPROCKET. WHILE SETTING DOWN THE CHAIN, EE'S FINGER GOT PINCHED BETWEEN THE CHAIN AND THE BOTTOM LINK.
While unloading an ibeam from a truck using a forklift, the ibeam became unstable and slipped, falling and impacting the employee causing a fracture to employee's left leg. Employee received medical treatment and surgery.
Employee was opening the silo 8 cleaning door located at the top of the silo when the door slipped causing EE's right finger to get caught between the door and the structural beam. EE had a minor cut on the index finger. Employee received prescribed medication.
Employee was using an impact wrench equipment with a swivel and socket to tighten one of the bolts that rend the knives. The accident occurred when the socket separated from the swivel and flew into employee's face causing a laceration on the upper lip.

The employee was entering the office building to go to EE's office. When EE stepped on the rug placed inside the building by the door, the rug moved and EE lost EE's balance and fell to EE's knees. The employee was on self-care measures until 11/30 when EE started receiving physical therapy
The employee was standing 15-20 feet away from the cleaning port located on the kiln seal inlet pipe on the second floor of the preheater tower when there was a flush and a stream of hot material ejected from the port and made contact with the employee's face and wrists. The employee received medical treatment and restrictions to work
Unexpectedly, the material fell from the west side of the TA duct, splashed on the ground and spread widely, creating a dust cloud covering the barricaded area and reaching the worker standing outside the barricade. The worker was extending the barricaded area. The employee suffered from corneal abrasion and received prescribed medication and work restrictions.

On 5/1/2023 the employee sought medical treatment & was prescribed pain medication & was placed on restricted duty.
The employee was working in the screw conveyor at the Process Engineer Fuel plant, adjusting a screw, when the Allen key they were holding slipped, causing a laceration to their middle right-hand finger when it collided with the housing of the screw conveyor.

The employee was cleaning the Pre-heater tower from stage 5. A long lance was used in order to create distance and allow the employee to stand aside while cleaning. At some point, while unplugging the port, material exited the port getting into the employee's eyes.

The employee attempted to loosen a safety bolt that was holding a piece of metal (scraper) to the frame of the secondary shredder using a hammer. The safety pin dislodged allowing the metal piece to fall. The piece hit the hammer and the hammer hit the employee's right ring finger

500 KW Generator was operating in the Finish Mill area of the plant, the generator caught fire and the fire dept. was called to extinguish the fire. The fire was caused by the exhaust pipe running too close to the trailer the generator was contained in. Paint and insulation was burned on the side of the trailer.

Individual went to the south side of the tower to get the water lance and hose. Employee turned on valve, when they turned around they tripped on the hose and fell down. Shoulder hit the ground. This was reported as first aid case, they went to the Dr. on 10-2-18 and it became a reportable injury on that date.

Employee was fixing the seal of an inspection door on the kiln dust bin air slide. The system was down and after sealing it and closed the door, the latch popped open causing dust come out and cover the employee face, including getting access to EE's eyes.
Employee was cleaning the area of the Stage #4 duct. When EE hit the beam nearby, accumulated dust get into EE's eyes. The employee immediately washed eyes with water. EE was using the standard PPE.

The employee was cleaning the riser on the 4th floor West side, with a WAMA gun, when hot dust flew out of the pot hole and get into their left eye. The employee was using all PPE at that time. Immediately, they flushed their eye. They experienced redness in their eye on 08/02/2017.

During the process of cleaning a valve, the employee was in an awkward position, EE make a sharp turn and strained EE's neck.
The employee was removing a snowman, when a built-up fell and hit the lance of the water pressure gun. This caused that EE's left hand turns. EE felt a little pain, but EE thought it was nothing. On October 8, 2016, the employee mentioned to EE's supervisor what happened, but told supervisor that it was nothing. On October 24, 2016, EE's supervisor sent EE to a doctor.
EE was inspecting the bolts condition of a limestone feeder conveyor. EE tripped with the electrical cord when EE tried to prevent falling, when EE tried to pick up an electrical light, & put left hand inside the dolly rail, EE pulled the stop emergency cord but one dolly pinched lt index finger, dislocating it. EE has NOT missed any days from work but is working on restrictions.
The employee was unplugging the bucket elevator discharge chute to pan conveyor when ee hit the handrail with the bar. EE right hand finger (thumb) stayed between the bar and the handrail.
Strained hip trying to move a portable lighting tower to stabilize it. Worker decided to seek medical attention on 1/25/2016.

FROM EMPLOYEE STATEMENT On 6/8/15, when loading an AC unit over the base at ER 13, I felt something stretching under the abdomen, but I did not give importance. It started swelling a bit, but then disappeared. Second time was at main office moving one table on 6/13/15 and I felt a little pain but nothing else. Employee later had surgery 10/12/2015
A laborer was cleaning an additive stacker job when he slipped and fell. Employee could not recall what he slipped on when questioned. Injury will be questioned through insurance carrier.

Worker standing next to a power supply tray with his foot under the tray. He did not notice and reached over to hand a tool to another worker. His foot got caught, he lost his balance and twisted his left knee. Worker sought medical attention on 05/27/2014.
Employee was cleaning one of the ports at the pre-heater tower feed hood with a water blasting system. He shot water into the port, and that's when hot vapors and dust surged back and into employee's fire protecting hood. Employee sustained burns to his scalp and left ear.

While pulling 54"" steel cable conveyor belt on conv#Y34 - approx. 200' belt had been secured for pulling with 6"" C-Clamps. EE was standing at the approx 230' from roll end where c-clamps were connected to roll. Both clamps released and flew the 230' to strike him on both the right and left upper thigh. He sustained bruising and abrasions on both thighs.

Installing a chute on top of a feeder in the coal mill. The sling got hung up and the foreman told his crew to get a look from the back of the chute. The injured miner did not move(he does not understand english well). There was a problem with the rigging & the chute slid and hit injured below his knees breaking both legs.

Employee stated that he was traveling over a speed bump when the basket of the manlift he was using began bouncing causing him to fall on top of its bolt head.

Erecting scaffold in cyclone when a clinker fell from an overhead section of the cyclone. The clinker landed on the employee's top right side of the hard hat. It also struck his right shoulder, right hip and left thumb causing a fractured shoulder and wrist.
Employee stated that he was cleaning a cooler when he backed up off the platform he was standing on and fell 2 feet to the floor landing on his right foot.
The elevator in the plant dropped momentarily when a piece of scaffold touched the elevator counterweight. The piece of scaffold bounced on the outside of the employee's foot just behind the steel toe which fractured his small left foot toe.

Employee stated that while working with a hand grinder, he lost grip of the grinder and it fell, lacerating his left middle finger.
A demolition crew was working to remove the K3 Coal Firing System (not in use anymore) during the day on 07/05/06. At 4:00 am on 07/06/06, the pulverized coal belt (explosion proof) was found red hot and sparking. Immediately the Fire Department was notified and the fire was extinguished by 5:15 am. No injuries occurred during the incident.
Employee stated that he was turning on the water valve for a high pressure water gun when a burst of hot raw meal came out of the port that was being cleaned. He was getting out of the way when he accidentally collided into the portable platform and struck his right hand.
Employee was cutting plates for a job. He stated that he was working on a metal shear when his left pinky finger got caught by the support plunger, which engages when the shear cuts.

Employee stated that he was walking under the F3 mill when he fell into a ditch that was filled with water and struck his left knee. Employee initially refused to seek medical treatment. Later decided to seek treatment on 01/03/06.
Employee stated that he grabbed a railcar door lock to move it and clear the door. A coworker on the other side of the railcar did not notice him and closed the door, smashing his left middle finger between the lock and door.
Employee stated that he was pushing down on the latch that secures the tail pulley doors when his left hand slipped, bending his thumb backwards. Note: Hurricance Katrina (08/25 - 08/26). We had no phone, fax, or email until Thursday, 09/01).
Employee stated that he strained his back while picking up a 50 lb box of welding rods. Employee will be receiving physical therapy.

The reason for accident is the head ache ball on the crane got hung up on edge of roof transferring some of the weight to the roof. structure. when the load came uncaught the load lurche down and bounce due to boom deflection. Ee was sitting on a beam and was caught between the beam and the load.
Employee stated that his right middle finger was punctured by a nail while he set down a box.
Employee was welding at the raw mill chute when the weld sparks lit up the torch that was on the floor. As a result, he sustained a burn to his right leg.
Employee stated that his right fore finger was caught inbetween a door and cone as he closed an inspection door to Silo 21.
Employee was engaged in helping four others remove a metal chute and transition piece. Employee was unaware that an arm to a diverter valve had been cut loose and when he grabbed the arm it caused the diverter valve to move, mashing his left middle finger between diverter gate and side of chute causing a laceration requiring stitches.

Employee was working in fab area during the day. In the afternoon he contacted his supervisor for discomfort in right eye. It was examined and flushed out. He experienced continued discomfort and was taken to a clinic, examined and a piece of metal was found and removed from right eye. He was released back to work.
Employee stated that he was unplugging a gypsum chute and his hand became wedged between the chute and material.
Employee stated that he was driving towards the feeding tower when he drove over a cable, causing the vehicle to overturn.

EE was welding on a discharge chute of a conveyor at night the wind was blowing. he felt something in his eye but feeling went away. he was wearing safety glasses. he was taken to eye clinic after he came to work next day complaining of pain. a piece of metal was removed from surface of left eye. no lost time was incurred.
The employee was attempting to relocate a shackle that had lowered a load to the floor level. While removing the shackle, the load that was leaning against another structure shifted causing the shackle to rotate and tighten striking the employee on the left hand which fractured / cracked the 4th digit.
While employee lifted an oil drum from a forklift, using a hand trolley hoist chain, the drum swung and smashed his right hand index finger against a handrail.
While moving a brick scafold half moon, employee turned the piston into the open position. The piston caught his left hand and crushed it. The conditions were normal.

According to most recent audiogram, ee has suffered recordable hearing loss in left ear. This ee may be exposed to noise above 85 decibels (A-scale) for an 8-hour time weighted average while on the job. Hearing conservation training & hearing protection has been provided to ee upon hire & annually thereafter. In addition, ee's supervisor has been trained in hearing conservation.
EE WAS WORKING IN AN AREA NEXT TO ANOTHER EE WHOWAS USING A GRINDER. EVIDENTLY A PIECE OF METAL FLEW INTO HIS RIGHT EYE. THE NEXT DAY HE WAS STILL BOTHERED IN HIS RIGHT EYE & WAS SENT TO A ME DICAL CLINIC & A PIECE OF METAL WAS REMOVED FROMRIGHT EYE. NO LASTING DAMAGE FROM INJURY. HE WAS WEARING SAFETY GLASSES.
EE DISASSEMBLED A SLURRY PUMP FOR INSPECTION. AS HE BEGAN TO ASSEMBLE THE PUMP, ONE OF THE PARTS LANDED ON HIS FINGER WHILE ATTEMPTING TO INSTALL IT.

AREA IN GOOD CONDITION. WHILE HAMMERING, A PIECE BROKE OFF THE HEAD OF THE HAMMER AND PENETRATED HIS LEFT FOREARM.
EE WAS IN THE PROCESS OF ALIGNING TWO BRACES TO BOLT THEM TOGETHER, AS THE PIECES WERE BEING ALIGNED, THE EE'S FINGER WAS PINCHED BETWEEN THE TWO BRACES CAUSING THE LACERATION.
STAIRS IN GOOD CONDITION, NO AMBIENT DUST OK VISABILITY. AS EE MADE HIS WAY DOWN THE STAIRS HE TWISTED HIS FOOT. EE HAD VARIOUS TOOLS AND MATERIALS WITH HIM.
EE WAS USING A COME-A-LONG TO TIGHTEN A CABLE CONNECTED TO BEAMS FOR ALIGNMENT, THE CABLE SNAPPED UNDER PRESSURE CAUSING THE COME-A-LONG TO STRIKE THE EE IN THE FACE, KNOCKING HIM DOWN CAUSI NG A LACERATION TO HIS LEFT CHEEK /FACE.
EE WAS LOADING A PIECE INTO THE LATHE CHUCK WHENTHE PIECE SLIPPED OUT & HIS FINGER GOT CAUGHT BETWEEN THE PIECE & LATHE BED
EMPLOYEE WAS ASSIGNED THE TASK OF CHANGING OUT THE CENTER BOLT OF A WIND BRACE WITHIN THE RAW MILL STRUCTURE. A COME-ALONG SHOULD HAVE BEEN USED TO SUPPORT THE WIND BRACE BUT WAS NOT. HE LOOSENED THE BOLT AND USED HIS RIGHT MIDDLE FINGER TO PUSH THE BOLT OUT OF THE BOLT HOLE. AT THAT POINT THE BRACE SHIFTED, AMPUTATING THE END OF HIS FINGER NEAR THE FIRST JOINT.
GOOD, CLEAN, CLEAR DAY. EE WAS WALKING DOWN THE STAIR FROM K346 TO K348 ON TOP OF THE CLINKER SILOS, WHEN HE SLIPPED AND TWISTED HIS KNEE.
CONDITIONS GOOD, EE WAS NAILING WHEN HE FELT SOMETHING ON HIS ARM. HE NOTICED HIS ARM BLEEDING, A METAL CHIP MUST HAVE GONE INTO HIS ARM.
CHUTE WAS PLUGGED & OVERFLOWED MATERIAL ONTO THE TAIL PULLEY. EMPLOYEE WAS CLEANING THE AREA WITH A WATER HOSE. IN THE WET CONDITIONS HE SLIPPED & FELL, HURTING HIS RIGHT KNEE.

METAL CATWALK WITH COAL ON IT. EMPLOYEE WAS USING A WATER HOSE TO CLEAN MATERIAL OFF OF CATWALK. WHILE MOVING TO A NEW AREA, HE SLIPPED.
